고령화연구패널자료를 이용한 노년기 건강불평등의 구조 분석 = Structures of Health Inequalities of Korean Elderly : Analysis of Korean Longitudinal Study of Ageing

국문 초록 (Abstract)

이 연구는 노년기 건강불평등에 대한 통합적인 이해를 위해서 건강상태에 대한 다차원적 개념을 적용하여, 우리나라 노인들의 삶의 조건을 구성하는 사회적 불평등의 요인과 이들 요인이 건강격차를 만들어 내는 기제를 규명하고 이로부터 노년기 건강불평등의 완화를 위한 정책적 함의를 모색해 보고자 한 것이다. 연구모델에 대한 검증을 위하여 한국노동연구원에서 제공하는 「2006년 고령화연구패널 제1차 기본조사」의 원자료를 활용하였다. 확률표집에 의한 45세 이상 중고령자를 대상으로 하는 원자료에서 만 65세 이상 4,155명을 이 연구의 분석대상으로 하였다. 65세 이상 노인인구의 전반적인 건강상태를 정신적, 기능적, 의료적 및 주관적 차원에서 분석한 결과에 의하면 노인들의 건강상태가 단순히 생물학적 또는 생리적 특성의 차이 뿐 만 아니라, 성별, 교육, 소득 수준 등 사회적 불평등의 요인에 의해 차별화된 삶의 조건이나 경험의 결과적 양상인 것을 알 수 있었다. 사회적 불평등 요인이 건강상태에 영향을 미치게 되는 중간기제를 분석해 본 결과에서는 자녀 및 친족관계, 사회단체활동 등 심리사회적 자원의 긍정적 가설이 확인되었으며, 건강위험 행동은 이론적 가설과는 상반되는 결과가 나타나 건강위험성이 배태되어 있는 사회문화적 특성이 고려되었다. 또한 건강의 4개 하위 차원 및 통합적 개념에 대한 사회적 불평등의 요인의 효과를 비교 분석함으로써 건강의 다차원성을 확인하는 한편, 포괄적 건강상태에 대한 주관적 건강상태의 예측 효과를 확인할 수 있었다. 이상 노년기 건강상태에 영향을 미치는 사회적 요인들의 효과에 근거하여, 노년기 건강증진을 위해 신체 또는 생리적 기능에만 초점을 두는 의료적 접근을 넘어 건강문제의 근본적인 맥락이 되는 사회적 조건에 대한 이해의 중요성을 강조하였다.

다국어 초록 (Multilingual Abstract)

This study attempted to examine the causal mechanisms of health disparities among elderly Koreans in terms of social inequalities. Secondary analysis was done for 4,155 probability sample aged 65 years or older from 「2006 Korean Longitudinal Study of Ageing」 by Korea Labor Institute. Analysis results presented that health disparities among elderly Koreans would not be simply regressed on the biological or physiological differences but more widely related to the unequal social conditions constrained by gender, education or income level. As intermediate mechanisms between social status and health disparities, the positive effect hypothesis of psycho-social resources including relationships with children and relatives or informal social organization participation was supported; whereas the negative effects of health-risk behaviors were not clear or somewhat contrary to the theoretical hypothesis. Upon these inconsistent results, the traditional Korean culture which were generous to drinking and smoking were discussed as risk factors. Confirming the gender or socioeconomic status as fundamental causes of health disparities, this study suggested the development of health promoting strategies for elderly women in the lowest ladder of SES as the first step of policy intervention to challenge the health inequalities among Korean elderly population.
