재가 치매노인 주부양자의 부양부담 실태 및 영향요인 = Influencing Factors and Caregiving burden among primary caregiver for Demented Elderly

국문 초록 (Abstract)

본 연구는 재가 치매노인 주부양자 196명을 대상으로 하여 첫째, 재가 치매노인 주부양자의 부양부담 실태를 파악하고 둘째, 재가 치매노인 주부양자의 부양부담에 영향을 미치는 요인들이 무엇인지를 규명해보고자 함이 주된 연구목적이다. 이를 위해 선행연구를 통해 부양부담에 영향을 미치는 요인으로 파악된 치매노인과 주부양자의 특성 외에 또 한 가지 중요한 요인으로 언급되고 있는 사회적 지지 특성을 정서적 지지, 정보적 지지, 도구적 지지 등 대표적인 세 가지 유형으로 나누어 각각의 영향력을 살펴보고자 하였다. 또한 종속변수인 부양부담은 보다 구체적으로 총 부양부담 및 신체적, 사회 활동적, 정서적, 재정적 부담과 가족관계의 부정적 변화 등 다섯 가지 하위차원별 부양부담에 대해 살펴보았다. 본 연구결과 첫째, 치매노인을 돌보는 주부양자의 부양부담 실태는 총 부양부담 정도가 평균 2.12점으로 비교적 높은 부양부담을 나타냈다. 하위차원별 부양부담 실태를 살펴보면, 사회 활동적 부담이 2.33점으로 가장 높게 나타났고, 그 다음이 신체적 부담( 2.20점), 정서적 부담(2.15점), 재정적 부담(2.06점)의 순이었으며, 가족관계의 부정적 변화가 1.88점으로 가장 낮게 나타났다. 둘째, 주부양자의 부양부담에 영향을 미치는 변수로는 치매노인의 ADL, 문제행동, 배우자유무, 월평균 가구소득, 1일평균 부양시간으로 밝혀졌으며, 특히 치매노인의 ADL과 1일평균 부양시간은 거의 모든 부양부담에 유의미한 영향을 크게 미치는 것으로 나타났다. 한편, 사회적지지 특성을 지지 유형별로 나누어 살펴본 결과, 정서적지지 만이 총 부양부담에 유의한 영향을 미쳤을 뿐, 대부분의 영역에서 유의한 영향을 미치지 않는 것으로 나타났다. 이상의 연구결과를 바탕으로 선행연구결과에 비추어 논의를 전개하였으며 재가 치매노인 주부양자의 부양부담 경감을 위한 나름대로의 함의를 제시하였다.

다국어 초록 (Multilingual Abstract)

The main purpose of this research is to investigate caregiver's burden and the factors influencing caregiving burden in the primary caregivers of patients with dementia. Data was collected by questionnaires from 196 dementia elders' primary caregivers that have been caring for the dementia elders at home for more than 6 months and have not resorted to any hospitals or in-home services. Independent variables included dementia elders' and primary caregivers' general characteristics and social support characteristics, and the caregiving burden, a dependant variable, includes 5 sub-categories that was measured separately: total caregiving burden and physical burden, social burden, emotional burden, financial burden, and negative change in family relationships. The results are as follows : The burden of caregiver's was relatively high. With regard to the issues of the caregivers' burden, the social burden is the most significant one among the five dimensions of the care giving burden, and the next are physical, emotional, financial burdens and negative change in family relationships in this order. The degree of care burden of the primary caregiver was significantly dependent on the level of ADL, problem behavior, marital status, family income, the hours of caregiving per one day. Among three types of social support, emotional support appears to be the most important in diminishing the caregivers' total burden. These findings provide some implications and suggestions for the development of policy in order to relieve primary caregiver's burden.
