건강 형평성의 관점에서 본 제3차 국민건강증진종합계획 평가

국문 초록 (Abstract)

국민건강증진법에 따라 정부는 매 5년마다 국민건강증진종합계획을 수립하고 있으며 2005년 수립된 계획부터 건강 수명의 연장과 건강 형평성의 제고를 총괄 목표로 제시하고 있다. 이 중 건강 형평성은 사회 정의, 인권의 측면에서 중요할 뿐 아니라 건강의 사회적 결정 요인과 관련하여 최근 보건 정책에서 핵심적 개념으로 자리 잡고 있다. 본 논문에서는 2010년 정부가 수립한 제3차 국민건강증진종합계획(2011-2020)을 건강 형평성의 관점에서 집중적으로 탐색하였다. 이를 위하여 세계보건기구와, 미국, 영국 등 주요 선진국들에서 건강 형평성을 개선하기 위한 정책적 노력들을 살펴보고 우리나라의 현 수준과 비교하였다. 제3차 국민건강증진종합계획에서는 과거에 비해 건강 형평성과 관련한 하위 목표 수의 증가, 지역 간 불평등의 해결을 위한 지표 제시 등 몇 가지 개선이 있었다. 하지만 건강 형평성 제고에 대한 총괄 지표가 없고, 16개 대표 지표 중 건강 형평성과 관련한 지표는 제시하지 않았다. 뿐만 아니라, 건강 형평성을 개선하기 위한 구체적 사업들은 여전히 제한적이고 근거 역시 불분명하였다. 결론적으로 우리나라의 건강 형평성을 개선하려는 노력은 그 중요성에 대한 인식에도 불구하고 여전히 수사학적 표현에 머물러 있으므로 건강형평성 제고를 위한 국가 수준의 별도의 실행 전략과 계획을 수립하는 등 적극적 정책적 노력이 필요하다.

다국어 초록 (Multilingual Abstract)

According to the National Health Promotion Act, South Korean government started evolving Health Plan every five years. In 2005, extension of health adjusted life expectancy and improvement of health equity became overarching goals of the plan. Health equity has become not only a critical issue in health policy for achieving social justice and human rights, but also the central concept for the health policy related to social determinants of health. In this paper, we focused on the third Health Plan (2011-2020) formulated in 2010 by government from the perspective of health equity. Political efforts of WHO, US and British for the improvement of health equity have been analyzed and compared with those of South Korea. In the third Health Plan, some improvements were noticed such as increasing numbers of health equity objectives and encompassing objectives for reducing health inequalities among geographic areas. However, there is no overarching objective or leading health indicator related to health equity either. Moreover, policies and programs for reducing health inequalities are still limited and their evidence bases are weak. In conclusion, efforts for improving health equity are just lip services for now despite their importance has been recognized, so more active political efforts are needed such as establishing the independent national strategy and action plans for the improvement of health equity.
