일 도시 지역 노인여성의 요실금, 우울 및 삶의 질의 관계 = The Correlation among Quality of Life, Depression, and Urinary Incontinence of Elderly Women in a Urban City

다국어 초록 (Multilingual Abstract)

The purpose of this study was to identify general characteristics and the prevalence of urinary incontinence(UI) and depression and to investigate the correlation between depression and quality of life of elderly women with UI. The sample of this descriptive-correlational study consisted of 181 elderly women in A-city. For data collection, the Questionnaire for Urinary Incontinence Diagnosis(QUID), developed by Bradley, et al.(2005), the Korean form of Geriatric Depression Scale(KGDS)(Jung, et al., 1998), and the Incontinence Quality of Life Instrument(I-QOL), developed by Patrick, et al.(1999) were used. For data analysis, SPSS 12.0K for Windows program was use for descriptive statistics, ANOVA, and partial correlation analysis. As results, the average age of the subjects was 72.70 years and 59.7% of them lived in apartments. The monthly income was below 500 dollars in 32.0%. The prevalence of UI was 29.8% and the prevalence of depression was 38.1%. 33.1% of the subjects without UI reported above mild depression, conversely, 50.0% of the subjects with UI. Elderly women with UI reported average I-QOL score 83.81±22.65(maximum 110), moreover, there was a significantly negative correlation between KGDS score and I-QOL in women with UI(r= - .300, p= .000). These results suggested that it is imperative that the intervention appropriate for the elderly suffering depression and UI should be developed. For instance, programs for the elderly dwelling in community could develop and be carried out for detecting and improving depression and UI.

국문 초록 (Abstract)

노인여성은 다른 연령층에 비하여 신체상태와 기능이 저하되어 있고 사회적으로 고립되기 쉬우며 경제적 능력도 낮아 전반적으로 삶의 질이 낮아질 우려가 있다. 특히 노인여성의 건강문제 중 요실금은 단순한 노화의 과정으로 인식하고 일상생활에서 쉽게 간과할 수 있는 문제이므로 본 연구에서는 노인여성의 요실금 실태와 우울, 삶의 질 정도를 파악하고 그 관계를 확인하여 노인여성의 요실금, 우울, 삶의 질 향상을 위한 간호중재의 기초 자료를 제공하고자 한다. 재가 노인여성의 요실금 실태, 우울 및 삶의 질을 알아보기 위한 본 서술적 상관관계 연구는 일개 도시에 거주하는 181명의 노인여성을 대상으로 요실금 진단 설문지(Questionnaire for Urinary Incontinence Diagnosis; QUID), 한국판 노인우울 검사도구(Korean form of Geriatric Depression Scale; KGDS), 요실금관련 삶의 질 도구(Incontinence Quality of Life Instrument; I-QOL) 등으로 자료수집 하였으며 SPSS 12.0 프로그램을 이용하여 기술통계, t 검정, ANOVA 검정, 부분상관관계 분석을 실시하였다. 대상자의 평균연령은 72.70세이었고, 59.7%가 아파트에서 거주하고 있었으며, 월수입이 50만원 미만인 경우가 32%이었다. 요실금 유병률은 29.8%였으며, 38.1%가 경도 우울 이상으로 나타났다. 요실금이 없는 노인여성의 33.1%가 경도 우울 이상인데 반해 요실금 노인여성의 50.0%가 우울로 나타났으며, 우울과 요실금관련 삶의 질 점수는 유의한 음의 상관관계에 있었다(r= -.300, p= .000). 요약하면, 약 30%의 노인여성이 요실금을 앓고 있고, 요실금이 없는 노인여성에 비해 높은 우울 비율을 보였다. 또한 우울정도가 심할수록 삶의 질 정도가 낮아지는 것으로 보아 이들을 위한 중재가 시급함을 알 수 있다.
