This paper aims to examine the status and efficacy of MBSR's healing mechanism, mindfulness, created by Dr. John Kabat-Zinn, within the program, and to consider the status and efficacy of its original Pāḷi word, sati in the early Buddhist scriptures (Nikāya).
MBSR's “mindfulness” is ‘paying attention’ to present meditation objects, and “awareness” is 'knowing' the meditation objects in a state of attention. These two work as a complementary relationship to be mindful and aware of what is happening in the world and me. When these two mechanisms work in harmony, habitual and unconscious reaction to stress is avoided, stress reduction is resulted in, and the way to healing is opened. The meaning of MBSR's mindfulness is more reduced than that of the early Buddhist scriptures, and is mainly used for a healing purpose.
In the early Buddhist scriptures, sati was developed from its basic sense of “memory” to “not forgetting,” so that one does not forget present meditation objects and is mindful of them. Sati interacts in a complementary way with sampajañña (right knowledge) not to forget and to know the present meditation objects.
Buddha uses the “mindfulness of breathing (ānāpāna-sati)” as a typical example of sati practice. He confirmed that this practice would lead to the achievement of the four foundations of mindfulness (cattāro satipaṭṭhānā) and the seven enlightenment factors (satta bojjhaṅgā) and ultimately to gain liberation (vimutti) or nirvāṇa (nibbāna). While MBSR's mindfulness is reduced and mainly used for a healing purpose in its meaning, sati of the early Buddhist scriptures expands its meaning to reach liberation and nirvāṇa. When sati and sampajañña interact in a complementary way and meditation deepens, the stage of calm (samatha) and insight (vipassanā) becomes manifested gradually, and the stage of the more stable and tranquil concentration (samādhi) and the deeper wisdom (paññā) gets manifested. This can be said to be the expanded meaning and efficacy of sati, which is different from the reduced meaning and efficacy of mindfulness used in MBSR.
