보건 관리 체계의 지정학적 분할과 신자유주의적 변형

국문 초록 (Abstract)

중심부 국가에서는 사회의 의료화, 일상생활의 의료화라고 부르는 현상들, 즉 의료의 과잉 현상이 나타나고 있다. 반면 주변부 국가는 의료의 빈곤에 시달리고 있다. 이 글의 기본적인 목표는 세계-체계 내에서 발생하는 의료 불평등의 지리적 편중을 야기하고 가속화하는 정치경제적 힘을 추적하는 것이다. 이 글에서는 그 정치경제적 힘의 원천을 1970년대 이후 급격히 성장하기 시작한 제약산업의 이윤창출 전략과 동일한 시기에 위세를 떨치기 시작한 신자유주의 체계의 구축에서 찾을 것이다. 의료 불평등과 보건 관리 체계의 지정학적 분할은 의학과 건강을 매개로 이윤을 축적해 가는 거대 기업들의 전략 속에서 배치되고 변형된 것들이며, 또한 그 기업들의 수익성을 보장하는 국가 간 체계의 재편에 따라 형성된 것이다. 이 글에서는 세 가지 영역을 통해 이 문제를 살펴볼 것이다. 우선 우리는 중심부 국가의 과잉 의료화 및 주변부 국가의 과소 의료화와 그러한 차이들을 만들어내는 논리에 주목할 것이다. 그 후 임상시험이라는 기이한 수출품에 대해 살펴보면서 주변부 국가의 빈곤 계층이 어떻게 의료 산업의 새로운 착취의 대상으로 전환되는지 살펴볼 것이다. 그리고 마지막으로 주변부 국가들에서 나타나는 보건 관리 체계의 파괴 과정과 그 함의를 분석할 것이다.

다국어 초록 (Multilingual Abstract)

In core states, there are eccentric phenoma, called medicalization of society ―based oversupply for medical services. On the other hand, peripheral states suffer from shortage of medical services. The phenomena lacking in medical servises do not come from uncivilized or under-developmental situations in the peripheral states. They originate from the neoliberal restructuring programs that destruct existing health care systems. The primary purpose of this essay is to explore the politico-economic power that causes and accelerates geographical divergence of medical inequality in modern world-system. This essay argues that the very source of the power resides in two crucial factors: 1) strategies for creating enormous profit in pharmaceutical industries, drastically developed since 1970s, and 2) the construction of neoliberalism in the same period. Medical inequality and geo-political division of health care system is deployed and transformed by strategies of TNCs which focus on amassing their profit. And, the divisions are formed along with rearranging interstate system that secures profitability of TNCs(especially, big pharmas). Thus I examine these problematic issues in three ways. First, I will pay attention to differences between ‘neglected disease’ and disease attracted by big pharma. Second, odd exports of clinical trial will be discussed. Finally, this essay analyzes the destructing process of health care system in the peripheral states.
