국문 초록 (Abstract)

한국복지국가의 기원과 발전에 대한 연구는 확증되지 않은 가설들의 영역이다. 본 연구에서는 의료보험제도의 기원과 발전에 대한 연구를 통해 한국 복지국가 발전에 관한 가설들을 검토하고자 했다. 의료보험은 연금과 함께 현대 복지국가를 구성하는 핵심적인 지주 중에 하나이기 때문이다.
구체적으로 의료보험법제정(1963년), 의료보험법 2차 개정(1976년), 의료보험의 통합(1999년)이라는 세 가지‘결정적 국면’(critical juncture)들 사이에 놓여진 연속성과 단절성을 분석했다. 그 결과 각 결정적 국면은 과거 유산이나 균열에 의해서 촉발되거나 영향을 받았으며, 새롭게 도달한 결정적 국면은 새로운 유산과 균열을 남김으로써 다음의 국면에 영향을 미쳤다는 것을 알 수 있었다.
이 과정에서 복지국가 발전을 설명하는 산업화의 논리나 사민주의적 논리는 설명력이 제한되어 있다는 점을 확인할 수 있었다. 오히려 제도주의의 주장처럼, 다양한 사회ㆍ정치적 맥락 속에서 제도 자체가 주요 행위자들의 행위와 활동 방식을 변화시키고 조건 지우는 주요한 요인 중에 하나로 작용했다.

다국어 초록 (Multilingual Abstract)

There are some hypotheses about the origin and development of the welfare state in Korea. The purpose of this study is to examine these hypotheses by exploring national health insurance.
This study analyse three critical junctures which national health insurance has experienced: the enactment of National Health Insurance Law(1963), the second revision of National Health Insurance Law(1976), the total revision of National Health Insurance Law(1999). Research reveals that there are limits for the “logic of industrialism” or social democratic accounts to explain the development of national health insurance in Korea. Instead, institutional context and effects and historical sequence have played important roles in the development of national health insurance, as the institutionalist perspective would suggest.
