신체적 건강상태가 노인의 자살생각에 미치는 영향 = The Influence of Physical Health on Suicidal Ideation in Older Adults

국문 초록 (Abstract)

본 연구는 신체적 건강상태가 노인의 자살생각에 독립적인 영향을 주는 변수인지를 파악하고 노인의 자살생각에 대해 신체적 건강상태와 우울증상 간의 상호작용이 미치는 영향에 대해 검증하고자 하였다. 본 연구의 대상자는 대구지역에 거주하는 만 60세 이상의 노인 302명이었으며, 설문지 조사를 통해 자료를 수집하였다. 연구결과 노인들의 자살생각 정도는 평균 17.88(SD=13.82)(가능범위 0-38점)로 높게 나타났다. 건강상태는 인구사회학적 변수, 우울증상을 통제한 상태에서 자살생각에 유의한 영향을 주는 변수인 것으로 확인되었으며, 특히 우울증과의 상호작용을 통해 자살생각에 영향을 미치는 영향이 더 큰 것으로 확인되었다. 또한 노인의 자살생각에 가장 큰 영향을 미치는 변수는 우울증이었고, 남자 노인일수록, 연령이 낮을수록, 배우자가 없을수록 자살생각이 더 높은 것으로 나타났다. 건강서비스 제공자들은 본 연구에서 확인된 노인자살의 고위험군을 대상으로 자살예방을 위한 스크리닝을 시행하여야 하겠으며, 특히 신체적 질환을 앓고 있는 노인들이 질병의 과정에서 우울증을 경험하고 있는지 모니터할 필요가 있겠다. 노인자살의 위험정도가 매우 높은 것으로 확인된 대상자의 경우 적절한 의뢰와 입원 및 예방 프로그램을 시행할 필요가 있겠다.

다국어 초록 (Multilingual Abstract)

The purposes of the study were to explore the influence of physical health on suicidal ideation in older adults above and beyond sociodemographic and depressive variables and to test the interaction effect between physical health and depression on suicidal ideation. Subjects in the study were 302 community residents in Daegu who were over 61 years old. The findings showed that suicidal ideation in older adults was high that the average score was 17.88±13.82 (possible range 0-38). Physical health was a significant predictor of suicidal ideation, controlling for demographic variables (gender, age, having spouses, and economic status) and depression, but the interaction effect between physical health and depression had much higher impacts on suicide ideation. The most powerful predictor of suicide ideation was depression. The other significant predictors of higher suicide ideation were male, younger individuals, and having no spouse. Results suggest that health care providers need to assess whether older adults with physical illnesses have been placed with the subsequent development of depression, and if needed, make referral for mental health centers and provide some programs for suicide prevention.
