삼중음성유방암은 매우 악성의 종양 하위 유형으로 치료법이 매우 제한적이다. 우리는 이번 연구에서 ELK3가 악성 유방암 하위 유형에서, 특히 삼중음성유방암에서 높게 발현하는 것을 보여주었다. MDA-MB-231 세포주에서 ELK3를 억제하였을 때, 침습성 및 간엽성 특징에서 비침윤성 및 상피조직의 표현형으로 전환분화하는 결과가 시험관 수준과 동물 모델에서 모두 나타났다. 삼중음성유방암 세포주에서 ELK3의 억제는 광범위한 유전자 발현의 변화를 가져온다. 특히 암 전이를 억제하는 중요 유전자인 GATA3의 발현이 후성학적 변화를 동반하여 활성화 되었다. GATA3를 함께 억제하였을 때 다시 세포 이동 및 전이 능력이 복구 되는 것을 확인 하였으며, ELK3-GATA3 축은 삼중음성유방암의 전이 기작에 중요한 경로로 작용할 것이다.
종양 연관 림프관은 종양 세포의 전파에 작용하는 중요한 경로인데, 특히 림프관 내피 세포와 암세포의 상호작용이 암 전이에 기여한다. 종양 미세 환경에서 혈관 내피 세포에서 분비하는 혈관 분비 물질들과 비교해서 림프관 내피 세포 분비 물질들은 종양의 전이에 관하여 충분한 연구가 진행 되지 않았다. 우리는 림프관 내피 세포에서 ELK3의 발현이 종양 세포의 전파에 기여한다는 것을 확인 하였고 이는 림프관 내피 세포가 분비하는 엑소좀의 miRNA에 의한 것임을 발견하였다. ELK3가 억제된 림프관 내피 세포에서 추출한 배양액은 종양의 발달과 전이를 억제하는 것을 알 수 있었으며 그 배양액에서 분리한 엑소좀만을 처리하여도 종양 세포의 성장과 전이를 억제 하였다. ELK3 억제에 따른 림프관 내피 세포의 miRNA 발현 패턴을 분석 하였을 때, 우리는 miR-503-3p, miR-4269, miR-30e-3p를 ELK3의 하위 표적 유전자로 선정 할 수 있었다. 이 miRNA들은 위에서 언급한 암의 표현형을 발현시키며 림프관 내피 세포에서 전달되어 암세포에 영향을 미친다는 것을 증명하였다. 이 발견은 림프관 내피 세포에서 ELK3가 주요 조절자로 종양 세포와 주변 미세 환경을 통제하며 암의 전이를 지원한다는 것을 보여준다.
자연 살해 세포는 초기의 종양 형성에 관여하는 선천 면역 반응의 직접적인 작용 세포이다. 이 자연 살해 세포에 대한 반응은 종양 세포의 생존에 큰 영향을 미치며, 형성된 종양에서는 자연 살해 세포가 제대로 작동하지 못하게 된다. 우리는 ELK3가 억제된 삼중음성유방암이 자연 살해 세포에 대하여 민감하게 반응한다는 것을 확인 하였다. 우리는 삼중음성유방암의 두 가지 세포주에서 ELK3가 자연 살해 세포 수용기에 결합하는 리간드의 발현을 조절함으로써, 위의 결과를 나타낸다는 것을 증명하였다. 또한, ELK3는 암 세포의 미토콘드리아 관여 세포 사멸 과정에 관여하는 유전자들의 발현을 조절 하며 이에 따라 자연 살해 세포에 대한 반응성에 차이를 가져온다는 것을 증명하였다. 이와 같은 결과는 종양 세포의 면역 회피 기작에 있어서 ELK3가 중요한 역할을 한다는 것을 시사한다.
우리는 이번 연구를 통해 유방 조직의 악성 종양에서 높게 발현하고 있는 ELK3가 종양 미세 환경을 조절하여 암의 전이에 영향을 주며, 자연 살해 세포에 대한 면역 반응에도 관여한다는 것을 증명하였다. 이러한 이유로 우리는 ELK3를 잠재적인 항암 치료제 개발 타겟으로 제안한다.

Part I. The ELK3-GATA3 axis orchestrates invasion and metastasis of breast cancer cells in vitro and in vivo
Triple-negative breast cancer is a highly aggressive tumor subtype that lacks effective therapeutic targets. Here, I show that ELK3 is overexpressed in a subset of breast cancers, in particular basal-like and normal-like/claudin-low cell lines. Suppression of ELK3 in MDA-MB-231 cells led to transdifferentiation from an invasive mesenchymal phenotype to a non-invasive epithelial phenotype both in vitro and in vivo. Suppression of ELK3 resulted in extensive changes in genome expression profiles. Among these, GATA3, a master suppressor of metastasis, was epigenetically activated. Also, suppression of GATA3 led to the restoration of migration and invasion. These results suggest that the ELK3-GATA3 axis is a major pathway that promotes metastasis of MDA-MB-231 cells.

Part II. ELK3 expressed in lymphatic endothelial cells promotes breast cancer progression and metastasis through exosomal miRNAs
Tumor-associated lymphatic vessels (LV) serve as a route of cancer dissemination through the prometastatic crosstalk between lymphatic endothelial cells (LECs) lining the LVs and cancer cells. Compared to blood endothelial cell-derived angiocrine factors, however, LEC-secreted factors in the tumor microenvironment and their roles in tumor metastasis are poorly understood. Here, I report that ELK3 expressed in LECs contributes to the dissemination of cancer cells during tumor growth by providing oncogenic miRNAs to tumor cells through exosomes. I found that conditioned medium from ELK3-suppressed LECs lost its ability to promote the migration and invasion of breast cancer cells such as MDA-MB-231, Hs578T and BT20 in vitro. Suppression of ELK3 in LECs diminished the ability of LECs to promote tumor growth and metastasis of MDA-MB-231 in vivo. Exosomes derived from LECs significantly increased the migration and invasion of MDA-MB-231 in vitro, but ELK3 suppression significantly diminished the pro-oncogenic activity of exosomes from LECs. Based on the miRNA expression profiles of LECs and functional analysis, I identified miR-503-3p, miR-4269 and miR-30e-3p as downstream targets of ELK3 in LECs, which cause the above phenotype of cancer cells. These findings strongly suggest that ELK3 expressed in LECs is a major regulator that controls the communication between the tumor microenvironment and tumors to support cancer metastasis.
