국내 항균제 내성 실험실 진단 기준 확립 및 정도관리 = Establishment of guideline and quality control system of laboratory for antimicrobial resistance

국문 초록 (Abstract)

본 연구는 국내의 항균제 내성 현황을 파악하기 위한 Kor-GLASS 사업의 신뢰도 높은 자료 수집 및 도출을 위해, 미국의 CLSI와 유럽의 EUCAST 지침을 분석하고 국내 전문가 들의 의견을 설문 조사하여 진단 기준을 도출하였고, 검사에 적합한 표준 진단법을 확립하였다. 또한 GLASS 사업에 필요한 정도관리 체계를 제안하고 구축하였으며, 적절한 정도 관리 균주를 확보하여 예비 정도 관리를 수행하였다.
국내에서 감시가 필요한 항균제를 국내의 내성 현황, 임상미생물 전문가 및 감염내과 전문가의 의견을 종합하여 결정하였고, 2016년 Kor-GLASS 시험 기준에서 일부 항균제를 추가하거나 삭제할 필요가 있을 것으로 평가하였다. 대표적으로 quinuprisitn-dalfopristin 항균제는 사용이 거의 되지 않는 항균제라 감시의 필요성은 낮을 것으로 보이며, 새롭게 개발된 항균제의 국내 현황을 미리 점검한다는 측면에서 ceftarolone 및 ceftazidime-avibactam 같은 항균제의 추가가 필요할 것으로 판단되었다. 또한 breakpoint 결정을 위해 국내 처방 용량을 조사하여, 국내 현실에 적합한 breakpoint를 제안하였으며, 정도 관리를 위해서는 2016년 분리 균주를 대상으로 특성 규명 및 저장을 시행하였다.

In this study, we aimed to establish the korean quideline to produce the reliable data for Kor-GLASS system by review CLSI and EUCAST guideline for antimicrobial susceptibility testing and survey for clinical microbiologist and infectious disease physician. And we also produce quality control strains for preliminary external quality control program.
It was decided that the antimicrobial agents needed to be monitored in Korea were decided based on the present status of resistance in Korea, opinions of clinical microbiologists and infectious diseases experts, and it is necessary to add or remove some antimicrobial agents in the 2016 Kor-GLASS test standard. Antimicrobial agents such as ceftarolone and ceftazidime-avibactam should be added to quinuprisitn-dalfopristin antibiotics in order to monitor the current state of newly developed antimicrobial agents. In order to determine the breakpoints, we investigated the domestic prescription capacity and proposed a breakpoint suitable for the domestic reality. In order to manage the quality, we carried out the characterization and storage of isolates in 2016.
