대량의 상부위장관 출혈을 유발한 중부식도의 견인게실 1예 = Massive Upper Gastrointestinal Bleeding from a Traction Type of Diverticulum in the Midesophagus

The common sites of esophageal diverticula are the pharyngoesophageal junction, midesophagus and epiphrenic. The pathophysiological mechanisms of acquired esophageal diverticula are traction and pulsion forces. Traction diverticula of the midesophagus are usually asymptomatic, and found incidentally on an esophagogastroduodenoscopy or barium contrast esophagogram. Midesophageal traction diverticula are caused by inflammatory processes between the external wall of the esophagus and the adjacent structure. Pneumonia, bronchoesophageal fistula and gastrointestinal bleeding can occur due to an extension of inflammatory process into the lung or blood vessels. There are a few reports of midesophageal diverticular bleeding. We present a case of massive upper gastrointestinal bleeding from a traction diverticulum of the midesophagus that was successfully managed by endoscopic treatment. 국문 초록 (Abstract)

식도게실의 호발부위는 인두식도경계부, 중부식도부, 횡격막상부의 3곳이며 발생기전에 따라 압출형(pulsion type)과 견인형(traction type)으로 분류한다. 중부식도의 견인게실은 식도외벽과 주위 장기의 염증성 유착으로 발생하며, 증상이 없는 경우가 많아 상부소화관 내시경 검사나 식도조영술에서 우연히 발견되는 경우가 대부분이다. 식도게실의 합병증으로 게실의 미란이나 염증반응이 폐나 기관지 동맥에 파급되어 폐렴, 상부위장관 출혈을 일으킬 수 있다. 식도게실의 출혈에 대한 보고는 소수인데, 특히 중부식도게실에서 발생한 출혈은 매우 드물며 대부분 횡격막상부 식도게실의 출혈로 인해 수술을 하였고 출혈의 원인은 게실점막의 궤양이었다. 저자들은 폐결핵의 후유증으로 인해 발생한 중부식도의 견인게실에서 대량의 출혈이 발생하여 내시경적으로 치료한 1예를 경험하였기에 문헌고찰과 함께 보고하는 바이다.
