Recently, It is well established that OKT3, a murine IgG2a monoclonal antibody directed against the CD3 molecule on human T lymphocytes, is a potent immunosuppressive agent for prophylaxis and treatment of acute allograft rejection. A 34 year-old male patient had a chronic renal failure due to chronic glomerulonephritis of undetermined etiology. He experienced renal transplantation failure due to surgical complication and got a second transplantation After the second transplantation, he showed acute accelerated rejection and received OKT3 in high dose (5nw``day) for 16 days. During the treatment, peripheral blood cell population changes of absolute lymphocytes and absolute lymphocyte subsets (CD3, CD4, CD8, CD19 and CDI6+56) were evaluated. We discussed the, immune mechanism by which OKT3 induce rapid disappearance of virtually all circulating T lymphocytes, OKT3 may react with all circulating T lymphocytes and form imnune complexes, This imrn.me complex formation consists of three stages, the first is OKT3 binding to antigen of target cells, the second is cawing, erasing-linking of antigen into patches, and the third is imnune complex internalization. The cells are now surface-negative for the modulation antigens and make apparent disappearance of Tlymphocytes. Also upregulation of adhesion molecules on the endothelial cell memrane causes adhesion of lymphocytes to the endothelial cells may contribute to persistent lymphocytopenia in peripheral blood.
