Arterial steal syndrome is rare but a serious complication which causes retrograde fi ow disruption distal to arteriovenous fi stula (AVF) because of excessive blood fi ow through the AVF. A 65-year-old woman with diabetic end stage renal disease on hemodialysis was admitted with complaint of coldness and pain in the right hand. AVF stenosis had been developed 6 months after the upper arm AVF operation. These manifestations developed 2 days after successful radiologic intervention and became worsened to ulceration, and aggravated especially during dialysis. fistulogram revealed arteriovenous anastomosis site was patent but blood fi ow toward forearm was severely decreased. Dialysis access steal syndrome developing after percutaneous angioplasty of arteriovenous fi stula stenosis was suspected, and ligation of the arteriovenous fi stula was performed successfully. After ligation of the arteriovenous fi stula, hand pain and ulceration were resolved. To the best of our knowledge, it is fi rst case of arterial steal syndrome after angioplasty.
