We report a rare case of radioactive iodine (RAI)-induced Graves` disease in patient with toxic adenoma. Forty two-year-old woman was presented with neck masses. Hot nodule on thyroid scan suggested toxic adenoma and she got RAI treatment. Three months later, she complained thyrotoxic symptoms such as weight loss, palpitation, diarrhea, and menstrual irregularity. Thyroid scan showed diffuse increased uptake while the previous toxic adenoma became cold nodule. Thyroid-stimulating hormone receptor antibody increased. It suggested Graves` disease and she took anti-thyroid drug. Physicians keep RAI-induced Graves` disease in mind when thyrotoxicosis occurred after RAI treatment.
