지방정부 의료서비스 만족도와 주민들의 삶의 질에 관한 연구 = 연령의 조절효과를 중심으로

국문 초록 (Abstract)

자치분권이 강조되고 인구의 고령화 문제가 대두되는 상황에서 건강관련 삶의 질을 향상시키는 것은 지방정부의 정책과제로서 그 중요성이 갈수록 높아지고 있다. 이러한 상황에서 본 연구는 지방 주민들의 의료서비스 만족도가 이들의 삶의 질에 어떠한 영향을 미치는지를 확인하고, 이러한 양자의 관계가 주민들의 연령대에 따라서 달라질 수 있음을 확인하고자 하였다. 이를 위하면 전국의 235개 지방자치단체를 대상으로 한 서베이 자료를 활용하여 의료서비스 만족도가 주민들의 삶의 질(전반적 삶의 질, 건강관련 삶의 질)에 어떠한 영향을 미치는지를 연령을 중심으로 조절효과 모형을 분석하였다. 모형의 분석결과, 주민들의 의료서비스 만족도는 건강관련 삶의 질과 전체적 삶의 질 모두와 긍정적인 관계를 보이는 것으로 나타났으며, 연령대는 양자의 관계를 조절하는 것으로 나타났다. 조절효과를 구체적으로 살펴보면, 노년층의 경우 양자의 관계가 더 강화되고, 중년층은 양자의 관계가 약화되는 것을 확인할 수 있었다. 이러한 결과는 지방정부의 의료서비스가 대상집단이 어떠한 생애주기에 걸쳐있는가에 따라서 삶의 질에 미치는 영향의 정도가 달라질 수 있다는 점을 제시하며, 기존의 의료서비스 행동모형을 수정하는 근거로 제시될 수 있다. 또한 정부의 개입에 대한 논의에 사회적 구성 개념의 추가적 함의를 제시할 수 있다.

다국어 초록 (Multilingual Abstract)

The purpose of this study is to examine the effect of satisfaction with health service on the QoL(quality of life) in local community, and to confirm that this relationship can be affected by the age of local residents. We analyzed the model with the survey data of 235 local governments across the country to examine the effect of satisfaction with health service on the QoL (overall quality of life) and the HRQoL(helath-related quality of life). As a result, there was a positive relationship between residents’ satisfaction with local health service and health-related quality of life, as well as overall quality of life. In addition, it is found that the age of local residents moderates the relationship between the satisfaction and the QoL(HRQoL), which indicates the elderly have a lower QoL(HRQoL) than other age group. These results suggest that the degree to which local governments influence the quality of life may vary depending on the life cycle of the target group. Moreover, it may provide additional implications of the concept of ‘social construction framework’ for the discussion of government policy intervention.
