To investigate the accuracy of ultrasonic axial lengths measurement in opaque intraocular lens after hydrophilic lens(ACRL-160^(??),Ophthalmed,USA) implantation. We measured axial length by ultrasonic scan at pre-intraocular lens exchange. Twelve eyes of 14 patients were conducted a clinical study who had intraocular lens opacity after hydrophilic acryl lens implantation. We compared pre-cataract operative axial lengths to pre-IOL exchange axial lengths with A-scan(Ocuscan^(??), Alcon, USA). The pre-IOL exchange axial lengths were measured in aphakic, and pseudophakic mode. In pseudophakic mode, the velocity in IOL was set at the rate of 1500, 1550, 1600, 1650, 1700, 1750 1800, 1850, 1900, 2000 and 2200 m/sec. The pre-IOL exchange axial lengths in pseudophakic mode at the rate of 1600m/sec in lens velocity were the closest to the pre-cataract operative values(p=0.859). However, other pseudophakic mode(1500, 1700, 1750, 1800, 1850, 1900, 2000, 2200 m/sec) and aphakic mode had statistically difference from pre-cataract operative axial lengths(p<0.05). When pre-cataract operative axial length is known in hydrophilic acrylic intraocular lens,previous values are proper to use for IOL exchange in opacity patient.but, if not, we can get the closest values to pre-cataract operative axial lengths by setting the velocity in pseudophakic mode at the rate of 1600m/sec in lens velocity.
