Peripartum cardiomyopathy (PPCM) and eclampsia with HELLP syndrome are serious complications of pregnancy, but the coincidence of both in one pregnancy is extremely rare. We report a case of 20-year-old primipara women with HELLP syndrome who presented in symptoms of severe heart failure (HF). After the pregnancy was terminated with urgent cesarean section, the patient developed respiratory insufficiency and echocardiographic examination, which demonstrated a dilatation of heart chambers and impaired left ventricular systolic function with decreased ejection fraction (EF) 28%. Both of diseases are needed to be performed in early diagnosis, and proper management of the disorder to improve the maternal, perinatal outcomes. Therefore, in order to decrease maternal and fetal morbidities, cooperation between internists and obstetric doctors is very important.
