The pituitary abscess is one of rare complications after surgeries via transsphenoidal approach (TSA). While overall mortality in those cases has been reported to be 51%, it could be estimated at 100% practically, if not surgically treated. Though antibiotic therapy plus surgical intervention is known as the treatment of choice, we experienced one case of post-TSA pituitary abscess which was cured by systemic antibiotic therapy only. The therapeutic decision was made so because of patient's refusal against operation and his old age. We conclude that antibiotic treatment without surgical drainage could be an alternative way of treatment if surgery is not available.
