부위 정맥 마취시에 사용한 근 이완제의 효과와 사용의 필요성 = The Effects of Muscular Blocking Agents in Intravenous Regional Anesthesia

국문 초록 (Abstract)

연구목적: 부위 정맥 마취시에 소량의 근이완제를 국소 마취제에 혼합하여 환자에게 사용하였을 때 어떤 임상적인 이득이 있는지를 보기 위해 본 연구를 수행하였다.
대상 및 방법: 부위 정맥 마취로 손 부위의 수술을 받는 30명의 환자를 무작위로 추출하여 무작위로 15명씩 두 군으로 분류하였다. 베큐로니움 군은 0.5% 리도카인 40ml에 베큐로니움 2mg을 혼합하여 주입하였고, 대조군은 0.5% 리도카인 40ml를 투여하였다.
결과: 베큐로니움 군은 대조군에 비해 좋은 근 이완과 수술 조건을 제공했고(p<0.05), 통증의 경감도 있었다(p<0.01). 임상적으로 두 군간의 차단 발현의 기간에는 차이가 없었다.
결론: 부위 정맥 마취시에 베큐로니움을 혼합하는 것에 의해 부작용의 증가 없이 좋은 수술조건을 만들 수 있고 통증의 완화도 가능하다고 보여진다.

다국어 초록 (Multilingual Abstract)

Purpose: This study was undertaken to examine the possible clinical advantages of using muscle relaxant with intravenous regional anesthesia.
Materials and Method: Thirty unpremedicated adult patients undergoing hand surgery were randomly selected to receive either 40ml 0.5% lidocaine or 40ml 0.5% lidocaine with 2mg of vecuronium.
Results: The vecuronium group of patients had a significantly greater degree of muscle relaxation, easier reduction of fractures, and better operative conditions(P<0.05). Less pain was also reported during surgery(P<0.05). Clincally, there was no difference in the speed of onset of block between the two groups.
conclusion: It is concluded that the addition of vecuronium to lidocaine improves the operationg condition during intravenous regional anesthesia with less pain during surgery.
