변잠혈 검사에서 Guaiac법과 Latex법의 비교 = Comparsion of Guaiac Method with Latex Method for the Fecal Occult Blood Test

There was concern about the false positivity of the popular guaiac-based test in fecal occult blood screening for the early detection of colorectal carcinoma. Recently the new immunological fecal occult blood test which was specific for humal hemoglobin was introduced.
We evaluated guaiac, latex agglutination, and latex agglutination inhibition method.
In senstivity study, guaiac and latex agglutination inhibition methods were positive above 1 mg/dl, latex agglutination method was positive above 0.1 w/dl. In specificity study, guaiac method was pos-itive to all seven animal hemoglobin, but two immunologic methods were opposit results. In the effect for temperature in stool collec-tion, converted to negative at 3-days in guaiac method, at 7-days in latex agglutination method, and at 14-days in latex agglutination inhibition method. In comparision study, the guaiac and immunolog ic positive rates were 35.0% and 9.0%. Most guaiac positive results were observed in unrelated gastrointestinal disorders.
Guaiac method had been often false positive results, but immuno-logic fecal occult blood tests were excellent specific method for human hemoglobin methods showed similar positive rates.
