금연보조제 Bupropion = Bupropion as a Smoking Cessation Aid

국문 초록 (Abstract)

항우울제인 bupropion은 1997년 금연보조제로 미국 FDA에 의해 승인되었다. 금연보조제로서 bupropion 서방정의 정확한 작용기전은 아직 알려지지 않고 있으나 dopamine과 norepinephrine 재흡수를 억제하여 dopamine 및 norepinephrine 작용을 한다. 금연을 위한 bupropion의 효과는 최근 여러 대규모 무작위 위약 대조군 임상시험에서 연구되었다. Bupropion 약물요법은 위약에 비해 금연율을 약 2배로 증가시킨다. 그러나, 투약 종료 후 장기간에 걸친 효과는 아직 미지수이다. Bupropion 서방정은 금연일 1~2주전에 3일간 1일 1회 150 mg씩 투여 후에 1일 2회 150 mg으로 증량하여 사용한다. 치료기간은 보통 7~12주이며 지속적으로 금연을 시도하고자 하는 환자에게는 6개월까지 투여할 수 있다. 주요 부작용은 불면 및 구갈이며 경련의 위험은 약 0.1%이다. 따라서 경련성 질환이나 신경성 식욕부진증 환자에게는 금기이다. 결론적으로 bupropion 서방정은 금연을 위한 효과적인 약물요법이며 쉽게 복용할 수 있으나 경련의 위험을 높인다.

다국어 초록 (Multilingual Abstract)

Anti-depressant bupropion was approved by FDA as smoking cessation aid since 1997. The exact action of sustained release bupropion as a smoking cessation aid is unknown, but it has a noradrenergic and dopaminergic activities by inhibition of presysnaptic reuptake of norepinephrine and dopamine. The effect of bupropion SR for smoking cessation has studied in the large randomized, double-blinded, placebo controlled clinical trials. pharmacotherapy with bupropion doubled smoking cessation rates as compared with placebo treatment, but long-term effect after completion of bupropion pharmacotherapy for smoking cessation is still questionable. Recommended dosage of bupropion SR is 150 mg every morning for 3 days, then 150 mg twice a day prior to starting 1-2 weeks before quit date. Duration of therapy is usually 7-12 weeks with an extended 6 months for those patients continually attempting smoking cessation. Adverse effects are insomnia and dry mouth. Estimated risk of seizure is 0.1%. Bupropion SR is contraindicated in patients who have a history of a current seizure disorder, anorexia nervosa. In conclusion, bupropion SR is effective pharmacotherapy for smoking cessation and well tolerated, but increase risk of seizure.
