불임전문병원의 선택요인과 이용 만족도에 따른 병원 재이용 및 추천의향에 관한 연구 : 서울지역과 경기지역의 일 병원을 대상으로

The purposes of this study are to find out the overall satisfaction of hospital service and the correlation factors between hospital selection and service satisfaction based on survey of outpatients of infertility specialized hospital, and to figure out factors affecting the intention of hospital revisit & recommendation to others. The data was collected through a survey subjected to 235 female outpatients for infertility treatment at Seoul A hospital and GyeongGi-Do B hospital. The survey was consisted of 59 questions, classified under four categorized factors - hospital selection, service satisfaction, overall satisfaction, and individual characteristics of respondents. The collected data were processed through ANOVA, t-test and correlation analysis to find factors affecting revisit & recommendation to others, logistic regression are performed. SAS program(version 9.1)are used to analyzed data. The results are as follows. 1. The respondents were informed about the hospital firstly by good word-of-mouth(40.6%) and had made more than 8 visits for last 6 months (60.7%). They made the hospital selection by themselves (88.5%) without other hospital treatment experience (60.7%), and their decision for primary doctors at the initial consultation(39.3%). 1. Hospital selection factors and satisfaction were scaled from 1 to 5, and it was found that trust(4.43), kindness(3.85) and reputation(3.81) were the main factors of the hospital selection but not hospital location. Also kindness of staff(4.25), specialty(4.17), patient care service(4.02) and hospital facility(3.73) were variables relating to patients' satisfaction. 1. No particular satisfaction differences were found among respondents. 1. There were powerful correlation factors between hospital selection and service satisfaction for kindness of hospital and trust in staff. 1. The 3rd trial or more for the infertility treatment, geographical location, inconvenient treatment process were the factors lowering the intention of hospital reuse and recommendation to others. On the other hand, hospital facility and patient care service were positively related to the intention hospital reuse and hospital recommendation to others, indicating high priority for improvement. On the basis of findings above, it is recommended that special program reducing emotional as well as physical distress of infertility patients should be introduced. Satisfaction of waiting time regarding treatment, laboratory and payment, facility management and patient care service should be improved, In addition, the improvement of services should be carried out, because it may directly affect the reuse and recommendation to others of hospital. key words : infertility specialized hospital, hospital selection, satisfaction factor, hospital reuse, recommendation to others. 이 연구는 불임 전문병원 외래환자를 대상으로 전반적인 이용 만족도와 선택요인 및 만족요인의 상관관계를 알아보고, 연구대상자의 특성별, 선택 요인 및 만족요인별로 병원 재이용 의사 및 추천의향에 영향을 주는 요인이 무엇인지 파악하고자 하는 것이다. 이 연구는 서울지역 A병원과 경기지역 B병원의 불임센터를 이용하는 외래 재진환자 여성234명을 대상으로 구조화된 설문지를 이용하여 자기 기입식 조사방법으로 수집하여 분석하였다. 설문지의 구성은 병원선택요인, 이용만족요인, 전체만족도, 인구통계학적 특성 등 4개의 분류로 나누어 총59개 문항으로 구성하였다. 회수된 자료는 통계분석 SAS 프로그램 Version 9.1을 이용하여 분산분석과 t검정, 상관관계 분석을 실시하였고 연구대상자의 선택요인과 만족요인별 재이용 의사와 추천의향에 미치는 요인을 알아보기 위해 로지스틱 회귀분석을 실시하여 비차비(Odds ratio)를 산출하였다. 이 연구의 구체적인 결과는 다음과 같다. 1. 인구사회학적 특성 주요결과를 보면, 병원을 처음 알게 된 계기는 좋은 입소문(40.6%), 최근 6개월간 방문횟수는 8회 이상(60.7%), 병원선택 결정은 본인 직접선택(88.5%), 타 병원 시술경험은 없다(60.7%), 주치의 결정요인은 병원 초진상담 때(39.3%)로 각각 높게 나타났다. 1. 병원선택요인과 만족요인을 5점 만점 평균으로 산출해본 결과 선택요인 중에서는 신뢰성(4.43점), 친절성(3.85점), 명성(3.81점)으로 나타났으며, 접근성은 병원선택 요인의 중요한 변이가 아닌 낮은 점수로 나타났다. 또한 만족요인은 직원의 친절도(4.25점), 전문성(4.17점), 환자에 대한 관심과 서비스(4.02점), 시설환경(3.73점)순으로 나타났다. 1. 연구대상자 특성별 만족도에는 유의한 차이가 없는 것으로 분석되었다. 1. 병원선택 요인과 이용 만족도의 상관관계에서 병원의 친절성과 신뢰성은 강한 양의 상관관계가 있는 것으로 나타났다. 1. 병원의 재이용 및 추천의향에 미치는 요인 분석결과, 양쪽모두 의향을 낮추는 항목은 본 연구병원에서 불임시술 단계가 세 번째 이상인 경우와 지리적 접근성 및 이용절차의 편의성으로 나타났으며, 반대로 시설환경 및 관심과 서비스는 한 단위 증가할 때 마다 재이용 및 추천의향이 높게 나타나 해당부문 개선욕구가 그만큼 크다는 반증이라 하겠다. 이상의 결과를 통하여 알 수 있듯이 불임시술 단계별로 고객의 심적, 육체적 고통을 감소시킬 수 있는 프로그램 도입검토가 필요하며, 대기시간 (진료, 검사, 수납)만족도와 시설환경 그리고 환자에 대한 관심과 서비스만족도를 증가시켜야 할 부분이다. 즉, 재이용과 추천의향에 미치는 요인이 병원경영에도 유의한 영향을 초래하므로 추후 지속적인 연구와 꾸준한 개선의 노력이 필요할 것이다.
