생명공학과 말기환자 연명치료 방법의 발달로 최근 생명에 관한 논의는 생명의 시작과 끝에 관한 주제로 넘쳐난다. 우리가 숨 쉬는 공기처럼 눈에 보이지도 않고 너무나 당연한 돌봄은 관심 밖으로 밀려나 있다. 타인의 보살핌 없이는 생명을 유지할 수 없는 영유아나 치매노인들의 어이없는 죽음 또는 긴 병수발에 지친 가족들의 간병살인 또는 간병자살이란 극단적인 사건이 터짐으로써 돌봄이 존재 의미를 드러내고 있다. 본능적이고 모성애적인 돌봄은 인류의 발생과 더불어 존재해 왔다. 의도적인 돌봄의 역사는 초기기독교시대 이타주의적 실천봉사의 형태로 시작되었다. 나이팅게일의 영향으로 자원봉사형태였던 돌봄이 전문화하기 시작했다. 돌봄이란 단순히 신체적, 물질적 차원만이 아니라 사회심리적인 측면까지 포함한다. 돌봄은 도덕적 가치로 특히 보건의료전문인들의 덕목으로까지 논의되고 있다. 한국에서 간병은 주로 가족, 특히 여성들의 몫이었다. 그러나 최근 가족구조와 기능의 변화로 가족 이외의 돌봄 필요성이 증대되고 있다. 1인 가구의 증대로 그 심각성은 더 커지고 있다. 그럼에도 불구하고 한국에서는 아직 병원에 입원한 환자들조차도 가족이 침상 옆에서 간병서비스를 해야 한다. 어려울 경우 간병인을 개별적으로 채용해서라도 돌봄 책임을 다해야 한다. 영유아의 경우 돌봄 교육을 받지 못한 유아교사들이 담당하다 보니 돌연사까지 발생하고 있다. 어린이집이나 노인요양시설이 ‘값싼 노동, 싸구려 복지’로 전락하고 있다. 이제 국가가 국민의 생명 돌봄에 관심을 기울여 치료중심의 국민건강보험제도를 개선하고, 제공자중심의 의료서비스 평가는 환자 관점에서 그들의 경험 차원의 질관리도 포함해야 한다. 돌봄에 책임뿐만 아니라 돌봄 노동자들의 노동권과 돌봄에 대한 권리도 제대로 인정하고 보장해야 한다. 돌봄은 다른 서비스와는 달리, 개개인의 존엄성을 존중하는 돌봄 윤리에 기초해야 한다. 적합한 돌봄이야말로 인간이 인간답게 살 수 있게 해준다. Ethical discussions of life are overflowing with issues relating to the beginning or ending of life due to the development of biotechnology and life-sustaining care. However, how caring is important has been forgotten, because caring is as normal and common as air. Caring as an issue comes to the surface only through extreme cases of the sudden deaths of babies without due care, or through the ‘murder or suicide’ due to care givers’ burdens. The origin of instinctive maternal care can be assumed to lie in primitive human history. Intentional care originated from the altruistic service of early Christians. Florence Nightingale influenced the development of volunteer care into professional care. Caring includes psychosocial aspects, as well as physical. Care today is regarded as a kind of moral value, and especially as a virtue of healthcare professionals. In Korea, family members (especially female family members, such as mothers or daughters) must take care of patients. However, changes in the function and structure of the family havecaused the needs of non-family members’ care to increase. Patients whose family members cannot care for them should hire independent care givers. Babies are cared for only by teachers without care professionals. Nurseries and nursing homes have become a kind of ’cheap labor and cheap welfare’ without due care. Now, the author suggests that government should pay attention to caring for people’s lives and should reform the national healthcare insurance system; changing the focusfrom medical treatment to carebased treatment and having the healthcare accreditation system include patient’s experiences. The right to care of professionals should be recognized, as well as the professional responsibility of care. Caring is different from other kinds of services. Caring must be based on the ethics of care and should include respect for personal dignity. Due care gives human life meaning and value.
