Factors of the nurse's turnover in general hospitals

최근 병원행정가들의 가장 관심을 끄는 분야 중 하나가 간호사의 이직이다. 본 연구는 실제 이직을 한 간호사를 대상으로 이직요인을 파악하고 이에 대한 이직대책을 제시함으로써 조기에 간호사의 이직을 예방하여 효율적으로 간호인력을 관리하고 간호의 질적 향상을 하는데 도움을 주고자 46개 종합병원에서 1998년 1월${\sim}$1999년 1월 사이에 이직을 한 간호사 369명을 대상으로 조사, 분석한 후향연구이다. 연구결과를 통해 이직결정에는 다양한 요인들이 영향을 미침을 알 수 있었다. 특히 결혼상태가 가장 큰 요인으로 나타났으며, 기혼인 경우는 자식의 양육문제가 가장 원인이었으며, 미혼인 경우는 불규칙한 근무시간이 큰 영향을 미치고 있었다. 그에 대한 해결책으로 자식 양육문제의 경우, 근무병원 내에 탁아시설을 운영하거나 육아양육휴가제도 및 시간제 근무 등의 도입을 심각하게 고려해 볼 필요가 있다. 불규칙한 근무시간은 근무외 수당을 지불하거나 간호사가 스스로 자신의 근무시간을 선택할 수 있는 융통성 있는 스케줄링, 또는 승진평가 제도를 적절하게 할 수 있는 제도의 도입을 제안한다.

Trends in US Nursing Research: Links to Global Healthcare Issues

Nursing research in the United States (US) spans several decades. Many of the priorities/trends have stayed through the years. Today, the goal of producing evidence to support nursing care interventions coupled with the drive for Magnet Recognition has encouraged academic nurses (faculty) to work with nurse clinicians to form research teams. Interdisciplinary research teams have also formed to address growing concerns over patient safety and quality care. These issues are not just US issues but global ones. This article addresses US trends with the link to global research trends. The role that organizations such as the International Council of Nurses (ICN), the World Health Organization (WHO), and the Council of International Neonatal Nurses, Inc. (COINN) pay in shaping research agendas and promoting nursing research is highlighted. It emphasizes the key role that nurses, especially nurse leaders/administrators play in changing health outcomes through support of nursing research.

Purpose: In order to provide a systematic and standardized treatment course for MVD patients, a critical pathway (CP) program was developed and the results of its long term application were analyzed. Methods: This was a methodological study. The CP was established and applied to 75 (step I) and 1,216 (step II). Another group of 56 with similar features was used as a control group. Results: The application of CP turned out to be useful in many regards: the rate of hearing loss was reduced from 1.8% to 0% (step I) and 0.5% (step II), and low cranial nerve palsy was reduced from 1.8% to 1.3% and 0.7%, respectively. The length of hospitalization decreased by 2.56 days (25.2%) for step I and 3.05 days (30.0%) for step II. Days of ICU stay were reduced by 7.9% and 1.8%. The total cost per patient was reduced by 14.8% (step I). The cost per day was increased by 13.7% and 52.4%. An increase in the patient satisfaction index was noted, as shown in the ICU information guide (p=.002). Conclusion: The development and application of CP was found to improve the quality of medical treatment and the efficacy of hospital management in MVD patients. Well organized and efficient system and multidisciplinary teamwork are the key component of the successful application of CP.

Perceptions of nurses as a profession in Korea

연구목적 : 전문직으로서 간호사에 대한 인식을 집단별로 파악하고 이들에 대한 인식을 비교한다. 연구방법 : 설문지를 이용한 비교조사연구로서 조사대상은 간호사, 간호학교수, 의사, 약사, 간호조무사, 병원행정직원, 행정관료, 정당활동가, 일반시민 등을 포함한 총 584명이었으며, 설문지는 12개 하부항목으로 범주화된 36개 문항으로 구성되었다. 연구결과 : 간호사에 대한 인식은 의사가 가장 긍적적이었으며, 병원직원이 가장 부정적이었다. 모든 집단에서 가장 긍정적인 인식을 갖고 있는 항목은 '사회적 요구'였으며, 그 다음은 '직무기술', '사회적 인식' 등의 순으로 나타났다. 가장 부정적인 것은 '간호교육'이었고, 그 다음은 '간호조직체 활동', '직무환경' 순으로 나타났다. 결론 : 본 연구를 기초로 하여 간호사들은 다른 집단들을 대상으로 전문직으로서의 간호사에 대한 인식을 높이기 위한 전략을 마련하여야 한다.

Informed Consent: A Complex Process in Iran's Nursing Practice

Informed consent should not be regarded as a rigid process. Instead, it should be regarded as a flexible process that provides patient with the right amount of information required to facilitate meaningful decision-making. Informed consent is an important ethical and practical part of patient care. Understanding the moral foundation of informed consent can help to guide therapists in their communication with all patients. Nurses, as members of the multidisciplinary team, are currently key contributors in providing information to patients regarding the treatment process. But in this process, there are some challenges that make it difficult. This paper describes the underlying ethical and regulatory requirements as well as related challenges regarding informed consent in the context of a nursing practice in Iran.

향후 더욱 경쟁적인 상황에 놓이게 될 보천의료 분야에 능동적으로 대처하기 위한 방안의 하나로서 보건의료 조직은 간호부서의 성과 향상을 위한 노력을 지속적으로 기울이고 있다. 이러한 성과 향상을 위한 노력은 정확한 성과의 측정이 함께 이루어질 때 가능하게 된다. 간호서비스의 성과 측정은 여러 각도에서 조명되고 측정이 이루어져야하며 간호관리자는 총괄적인 성과측정시스템에 대한 새로운 인식과 사전 준비작업으로 무장을 하여야 한다. 이러한 요구에 맞추어, 본 논문은 간호 서비스의 성과측정방법에 대한 고찰, 즉 생산성, 효율성, 간호의 질, 수익성. quality of work life에 대한 간호분야에서의 접근방법을 재정리하고, 새로운 총괄적인 성과측정방법의 개념을 제시한다.
