드림스타트 아동통합사례관리사의 사례관리역량이 직무만족에 미치는 영향

본 연구의 목적은 드림스타트 아동통합사례관리사가 인식한 사례관리역량의 중요도와 실행도를 비교분석하여 이들 차이를 극복할 조직전략을 마련하고 직무만족을 높일 수 있는 방안을 모색하는데 있다. 전국 드림스타트 아동통합사례관리사를 대상으로 e-설문조사를 수행하였고 분석방법으로는 IPA(중요도-실행도 비교분석)과 회귀분석을 실시하였다. 연구결과, 첫째, 드림스타트 아동통합사례관리사의 사례관리역량 중요도는 5점 기준으로 평균 4.17점, 실행도는 3.50점으로 중요도 인식에 비해 상대적으로 실행도가 낮았지만 실행도 자체도 보통이상의 수준이었다. 둘째, 사례관리역량의 IPA(중요도-실행도 간 차이분석)결과, 중요도는 높으나 실행도가 낮은 역량은 지역사회 자원연계개발과 관리, 대상자의 자원정보 분석, 사례관리 단기목표 및 장기목표 수립, 사례관리 실행계획 전략, 자원활용계획, 대상자의 문제해결 역량강화, 지역사회연계 및 개발 등이었는데, 이들 역량들에 대한 향후 중점관리전략이 요청되었다. 셋째, 사례관리역량과 직무만족의 관계를 다중회귀분석한 결과, 사례관리역량의 중요도와 실행도 모두 직무만족에 통계적으로 유의미한 긍정적인 영향을 미치는 것으로 나타났다. 이와 같은 결과를 기반으로 향후 전문적이며 효과적인 사례관리역량을 강화하고 아동통합사례관리사의 직무만족을 높일 수 있는 조직전략 방안을 제시하였다. The purpose of the study was to conduct a comparison analysis of the importance and performance of case management competency as perceived by integrated case managers of Dream Start, and to develop an organizational strategy to overcome the gap between the two and increase the degree of job satisfaction. To achieve the goal, email questionnaires were conducted targeting 229 Dream Start’s integrated case managers. For means of analysis, IPA and regression analysis were conducted. The results of the study were, first, degree of importance regarding case management of integrated case managers in Dream Start averaged 4.17 of 5, and its performance averaged 3.50 of 5. Second, as a result of IPA for case management competency, competencies high in importance but lower in performance, were community network development and management. Thus, strengthening management strategies related to network development is recommended. Third, through multiple regression analysis of relationship between case management competency and job satisfaction, importance as well as performance of job competency posed statistically meaningful, positive influence on job satisfaction. With such results, promotion of professional and effective case management competency as well as increasing job satisfaction are required in the future.

지방정부 복지거버넌스와 통합사례관리 체계

사회복지전달체계의 변화는 지역 중심의 통합서비스 실현을 위한 민관협력과 지역 중심의 복지거버넌스(welfare governance) 구도로 발전하여 왔다. 본 연구에서는 복지전달체계 개편과 통합서비스 체계 구축으로나타나는 지방 정부 복지거버넌스를 고찰하고, 일본 지역포괄지원센터의 시사점을 파악하였으며, 한국적인사례관리 모델에 따라 통합사례관리의 특성을 파악하였다. 지방자치단체를 중심으로 추진중인 통합사례관리는 사회복지법령에 근거한 활 동으로, 사례관리 거점 지정에 따른 핵심적 과정은 ‘연계’ 기능이며, 이에 따른 지방자치단체의 과제는 추진 체계 및 대상과 서비스 수준에 따른 통합사례관리 유형화 방안 등이다. 이에 지방자치단체가 추진하는 통합사례관리는 지방정부 복지거버넌스를 특성화 하므로 지역 중심 통합사례관리 유형화와 방향성을 제언하였다. The purpose of this study is to consider the process of revising local governments’ welfaregovernance and social welfare delivery system and suggest the implementation system anddirectivity of integrated case management. To attain the goal, the study examined the process ofpolitical changes and Korean-style case management models and analyzed Japan’s welfaregovernance and cases of local inclusive support centers. According to the research results, in Korea’s local welfare governance, Integrated casemanagement should perform the function of social care centering around the local government, theservice provider, and it is the function that strengthens the function of ‘linkage’ through apublic-private partnership network. With regard to this, what Japan’s care management suggests to us is the teamwork among socialworkers, care managers, and nursing work force to provide support with preventive services for theaged with mild symptoms. Therefore, this paper will suggest the issues and directivity regardinghow to build a public-private cooperative integrated case management service delivery system atthe level of local welfare governance as written below:Integrated case management conducted by a local government is the activity of law enforcement,and it is needed to categorize the methods of case management according to the classification ofsubjects of welfare and strengthen its preventive function. Moreover, regarding the characteristics of integrated case management seen through Korean-style case management models, each individualmodel should strengthen the function to support the ‘linkage’.

의료서비스대상자의 통합사례관리 방안 연구 : 지방자치단체 총괄 방식의 ‘행정관리 모델’ 중심으로

본 연구는 최근 시·군·구, 복지현장에서 부각되는 통합사례관리 논의에서 장기적 사례관리 대상인 의료급여 사례관리 집단이 연계되지 못하거나 간과되고 있음을 파악하였으며, 의료서비스 전달체계 및 의료급여 사례관리 제도의 장기적인 효과성 보장 방안을 모색하고자 하였다. 의료재정의 급증현상은 재원의 효율성 및 사후관리 방식에 대한 효과성 평가의 과제를 부여하고 있다. 이와 관련하여 보건복지부는 2006년부터 전국적으로 저소득층의 의료이용 능력의 향상을 기하기 위한 ‘의료급여 사례관리 제도’를 실시하였다. 한편 의료급여 사례관리와 본인부담제의 시행 등은 의료재정 증가율을 안정시키는 성과에도 불구하고 수급자의 건강권·생명권 침해, 총 진료비의 계속 증가 비판, 의료급여 사례관리 인력 관리, 통합서비스 연계 검토 등의 과제를 안고 있다. 이에 지자체 내 사례관리영역에서 의료급여대상을 포함한 민·관 통합사례관리의 확대 수행 방식으로 지방자치단체가 총괄하고 사전 예방적, 대상 포괄적 방식으로 운용되는 통합사례관리 모델로서 ‘행정관리 모델’을 제시하였다. The Ministry of Health and Welfare carries out ‘medical care case management system’ to upgrade medical care of the low-income bracket to the proper level and improve health care capability of medical care beneficiaries. The increase in medical care funding gives efficiency of finances and assignment of post-management efficiency evaluation. Medical care case management and cost-sharing system are found to have an effect of stabilizing the medical care funding growth rate. However, such visible problems occurred as invasion of right to health and life, increasing total medical expenditures, wrong personnel operation of medical care case management, and lack of integrated local service management system. This study suggests the integrated case management system, a method to secure long-term efficiency of the medical care case management system aiming at improving health care management capability of the low-income bracket. This study is meaningful in that it points out that the medical care case management for the old · high cost · long-term medical care users is insufficient and it is neglected as the separate post-service, in terms of the integrated case management that has been noticed in the discussion of recent local governments and actual fields. In addition, the purpose of this study is to suggest policy foundation to operate the whole low-income residents’ medical services as the local, preventive, and inclusive integrated case management under the local government’s control.

COVID-19 시대, 통합사례관리 실천에 관한 질적사례연구

통합사례관리는 지역사회 중심의 방문형 복지서비스 역할을 중요하게 감당하고 있다. 본 연구의 목적은 COVID-19 시기에 통합사례관리사를 대상으로 서비스 제공 경험을 탐색하며 위기상황 이해 및 대응상황을 파악하여 위기상황에서 통합사례관리의 효율적 운영과 전달방안을 모색하고자 하였다. 본 연구수행을 위해 통합사례관리사 4인을 대상으로 일대일 심층면담을 실시하였고 면담자료를 질적사례연구 방법으로 분석하였다. 연구결과 4개의 대주제가 도출되었다. 대주제는‘컨트롤 타워 기능 부족’,‘비대면에서 클라이언트의 상황적 공감 파악의 한계’,‘자원 불균형 현상과 의료서비스 지원의 어려움’,‘위기발생에 대한 부담감과 지지체계 부족’이었다. 본 연구결과를 토대로 코로나 코로나19와 같은 위기상황에서 취약가구, 복지 사각지대 발굴에 대한 원활한 통합사례관리 서비스 지원을 위한 컨트롤타워 기능 재점검 및 강화, 단계별 위기대응 매뉴얼 구축, 자원공유를 위한 가상 네트워크 구성의 필요성을 제언하였다. Integrated case management plays an important role as a community-centered visit-type welfare service. The purpose of this study was to explore the experience of providing services to integrated case managers during the COVID-19 pandemic, and to understand and respond to crisis situations in seeking efficient operation and delivery plan of integrated case management. In this study, one-on-one in-depth interviews were conducted with four integrated case managers, and the interview data were analyzed using a qualitative case study method. As a result of the study, four major themes were derived. The main themes were lack of control tower function , limitation of client s non-verbal communication and situational empathy by non-face-to-face contact , resource imbalance and difficulties in medical support , and responding to the COVID-19 crisis alone . Based on the results of this study, this paper suggests the need for reexamination of control function of integrated case management and its capacity building, response manual by crisis level and virtual network for resource sharing to discover vulnerable households and welfare blind spots, and support smooth integrated case management services in a crisis situation such as the COVID-19 pandemic.

다문화가족지원센터 사례관리사들의 현장 실천경험에 관한 사례연구

본 연구는 다문화가족 사례관리사들의 다문화현장 사례관리 실천경험을 질적 사례연구(Yin, 2004)로 접근하여 다문화가족 사례관리사업 활성화를 위한 기초자료를 제공하는데 그 목적이 있다. 10명의 다문화현장 사례관리사와의 질적 사례연구를 실시한 결과 첫째, 다문화가족 사례관리서비스 플랫폼 구축하기의 핵심범주는 다문화사례관리업무에 대한 당사자 인식, 체계가 없어 좌충우돌하며 매뉴얼을 정립함, 대상자 발굴과 자원연계 시스템을 구축함 등으로 나타났다. 둘째, 다문화가족 사례관리실천의 다중적 딜레마는 다중적 클라이언트체계의 어려움, 공적/비공식적 경계가 불분명함, 심각한 복지의존증, 기관 내 인식부족으로 인한 역할혼동, 서비스 기간제한의 의미가 없음, 노하우를 상실시키는 센터 내 열악한 처우와 고용불안정, 지역기관의 업무 떠넘기기 등으로 나타났으며 셋째, 다문화가족 사례서비스에 대한 비판적 성찰은 일석이조의 효과성이 두드러짐, 해를 거듭할수록 쌓이는 노하우, 사례관리자로서의 자기성찰로 나타났다. 넷째, 다문화가족 사례관리 활성화를 위한 소고(小考)는 중복서비스 방지를 위한 지역사회통합사례관리회의 활성화, 정규사업 편입으로 지속가능한 사례관리서비스 확보, 다문화가족 사례관리 종사자들의 복지향상, 체계적인 사례관리 역량강화교육 등으로 나타났다. 이러한 연구결과를 토대로 다문화현장에서의 사례관리의 실천상의 이슈 해결과 다문화가족 사례관리 전문화 방안을 제안하였다.

치매안심센터 사례관리자의 맞춤형사례관리 실천 경험에 관한 현상학적 연구

본 연구의 목적은 치매안심센터에서 사례관리를 실천해 온 간호사와 사회복지사의 사례관리 실천 경험에 대한 질적 분석을 통해 치매환자 및 가족을 위한 표준화된 사례관리 실천 방향을 모색함에 있다. 연구대상은 6개치매안심센터 근무자 중 연구 참여를 수락한 1년 이상 맞춤형사례관리를 수행한 간호사 및 사회복지사 11명으로 하였다. 연구자는 대면 및 유선전화 심층면접을 통해 사례관리 경험을 표현하도록 하였고, 인터뷰 자료는 개인적 속성보다는 전체 연구 참여자의 공통적인 속성을 도출해 내는데 초점을 맞추는 Colazzi 분석방법을 적용하였다. 자료의 분석결과 연구 참여자들의 경험의 구조는 44개의 구성된 의미에서 17개의 주제모음을 확인하여4개로 범주화하였다. 4개의 범주화는 조직 환경, 스트레스, 대처, 성장으로 나타났다. 이러한 연구결과로 치매안심센터와 사례관리자의 맞춤형사례관리 수행에 긍정적인 변화를 도모하기 위하여 평가체계 재설정 및 표준업무매뉴얼 등 조직의 재정비가 필요함을 확인하였다. 여기에 치매 공공부문 사례관리 연계 및 협력 모형개발을제안하였으며, 또한 사례관리 실천과정에서 업무지도, 심리 및 교육적 지지와 치매안심센터 사례관리의 실천과정을 지속적으로 모니터링하여 개선할 수 있도록 슈퍼비전 체계 수립마련 등을 위한 후속 연구를 제언한다.

조직환경이 사례관리자의 전문성에 미치는 영향

본 연구는 서울시 지역 내 노인복지시설, 장애인복지시설, 사회복지관 3곳에서 근무하고 있는 사회복지사 중 사례관리자 300명을 대상으로 조직환경이 사례관리자의 전문성에 미치는 영향과 조직몰입의 매개효과를 파악하기 위해 실시되었다. 이 관계에서 사례관리자의 일반적 특성으로 경력, 집중사례 수 등을 살펴보았다. 분석방법으로는 SPSS 25.0을 이용하여 빈도분석, 상관관계분석, Baron & Kenny의 관계분석, Sobel test를 활용하였다. 연구결과 사례관리자의 전문성에 조직몰입, 조직환경이 정(+)적인 상관관계가 있음을 통계적으로 확인하였다. Sobel test 분석 결과, 조직환경이 사례관리자의 전문성에 미치는 영향에 있어 조직몰입의 완전 매개효과를 확인하였다. 이외 인구사회학적 특성에서는 사례관리 경력 3년 이상 5년 미만 비율이 가장 높았으며, 담당사례 수는 연 16 case 이상, 집중 사례관리 사례 수는 2∼3 case 또는 6 case 이상이 비슷한 분포로 가장 많았다. 이러한 결과값을 토대로 사례관리자 전문성의 중요성을 인식하고, 사례관리의 효과적 개입을 위한 조직환경의 대응체계 구축 및 효과적인 실천모델을 제시하기 위한 기초자료를 제공하였다. This purpose of this study was to investigate the effect of organizational environment on the case manager expertise and the mediating effect of organizational commitment. The subjects were 300 case managers among social workers working at three welfare centers for the elderly, disabled, and social welfare centers in Seoul. As general characteristics of case managers, gender, age, case management experience, job title, number of concentrated cases, and case management training were examined. Data analysis was conducted using frequency analysis, correlation analysis, Baron & Kenny's relation analysis, and Sobel test by applying SPSS 25.0. As a result of the study, it was statistically confirmed that there is a positive (+) correlation between the case manager's professionalism, organizational commitment, and organizational environment. As a result of the Sobel test analysis, the complete mediating effect of organizational commitment was confirmed, on the effect of the organizational environment on the professionalism of case managers. In terms of demographic characteristics, 30's, three and five years in case management experience, and five and ten years in institutional experience accounted for the highest proportions. The ratio of front-line social workers in position was the highest, and six or more cases yearly in the number of case management was the highest. Two to three cases or six cases or more in the number of intensive case management was similarly distributed. Based on these results, the importance of case manager expertise was recognized, and basic data were provided to establish an organizational environment response system for effective case management intervention, as well as to present an effective practice model.

커뮤니티 케어와 통합사례관리 연계 네트워크 실천 방안 연구

이 연구의 목적은 정부가 도입한 읍⋅면⋅동 중심 커뮤니티 케어의 효율화를 위한 통합사례관리 연계 네트워크 실천 방안 제시를 목적으로 한다. 이의 근거는 국민의 평생사회안전망 보장을 위한 사회보장기본법, 사회보장급여법 시행과 찾아가는 보건복지서비스 전달체계에서 통합서비스를 관장하는 읍⋅면⋅동 중심 통합사례관리 제도의 확대이다. 따라서 본 연구의 목적을 달성하고자 OECD 복지국가들의 커뮤니티 케어 제도를 분석하였고, 한국에서 도입한 커뮤니티 케어의 개념과 통합사례관리의 통합적 개념인 공공재적 사회서비스 성격을 고찰하였다. 또한 커뮤니티 케어와 통합사례관리의 연계 및 네트워크 실천을 위한 읍⋅면⋅동 복지환경의 대내외적 여건 분석을 위하여 최초로 SWOT 분석을 시도하였다. 커뮤니티 케어 체계에서 지역사회 통합돌봄 서비스 이용자(Service User)의 초기 접수로부터 노후에 이르기까지 서비스를 관장하는 읍⋅면⋅동 중심 통합사례관리의 핵심은 공공과 민간의 협력을 전제로 다양한 기관, 전문가 연계, 네트워크, 조정을 거쳐 서비스 이용자의 욕구를 충족시키는 과정이다. 이에 읍⋅면⋅동, 지역사회보장협의체, 돌봄서비스 기관, 종교⋅봉사단체 등의 네트워크화와 공⋅사 혼합복지체제로 효율화를 기해야 하는 지방자치단체의 복지거버넌스 전략과 새로운 거버넌스(New Governance) 기능으로 돌봄공동체 구축을 위한 커뮤니티 케어와 통합사례관리 연계 및 네트워크, 조정 기능을 제언하였다. The purpose of this study is to propose a linking integrated care management liaison network to promote the efficiency of the municipal-centered community care introduced by the government. The legal and administrative bases for this include the Framework Act on Social Security to ensure the life-long social security network and the expansion the municipal-centered integrated care management system that governs the integrated services developed from the visiting health and welfare service delivery system. Therefore, to achieve the purpose of the study, community care cases among welfare states have been examined, followed by the understanding of concept and theory of Korean community care services and the analysis of the characteristics of social service system. In addition, the study has conducted the first-of-its-kind SWOT analysis on the welfare environment for community care and integrated care management liaison network. The key to the success of the municipal-based integrated care management system that takes care of a comprehensive range of services from the initial-stage receipt of care service users to the senior care is to meet the needs of care service users through various institutions, linkage with experts, networks, and coordination based on cooperation in both public and private sectors. The study has, thus, found that the system of care services requires a welfare governance strategy to build a network of various relevant players, including local self-governing bodies, municipal assurance consultative groups, care service providers and religious and voluntary service groups and to promote the efficiency with the mixed welfare system of both public and private sectors. All in all, the study has suggested the strategy to build up the roles and responsibilities of local welfare governance that promotes linkage and networking to be able to strengthen the new governance function, consolidate the community and integrated care management, and to facilitate the access to the integrated services.

의료급여 사례관리 성과와 발전방향

우리나라 의료급여사례관리는 2003년 5월부터 의료급여 1종 수급권자를 대상으로 시작되었으며, 그들의 삶의질 향상과 합리적 의료이용을 유도하기 위함이었고, 궁극적으로 의료급여 재정 안정화를 도모하기 위함이었다. 2010년 현재 전국 244개 보장기관에 약 480여명의 의료급여관리사가 배치되어 의료급여수급권자 중에서 신규로 의료급여수급권자가 된 자, 병의원의 외래를 과다 또는 과소 이용하는 자, (요양)병원에 장기적으로 입원해 있는 자들을 대상으로 사례관리를 수행하고 있다. 의료급여관리사들은 전화, 방문, 서신 방법 등을 활용하여 정보제공, 심리적 지지, 보건교육 및 상담, 연계 및 의뢰 서비스를 제공한다. 본 연구는 문헌고찰을 통하여 의료 급여사례관리가 의료급여수급권자의 건강 삶의 질, 자가 관리 능력, 합리적 의료이용, 지지체계 영역에서의 향상과 외래 진료비 감소에 효과가 있는 반면에 입원 진료비는 증가하였음을 알 수 있었다. 이를 기반으로 효율적인 사례관리를 위한 발전방향을 첫째 통합순환 사례관리 체계구축, 둘째 표준화된 임상지침 및 사정도구개발, 셋째 자원 네트워크체계 구축, 넷째 통합 정보체계 개발로 제시하였다. The Medicaid case management system has been implanted to improve Medicaid beneficiary health-related quality of life and efficiency of the Medicaid finance since 2003 in Korea. Emphasis is given to the recipient of reasonable utilization of medical care. In order to provide health and welfare care through the Medicaid case management program, case managers, whose total number is approximately 480 nurses as of December 2010, have been working at 244 local government agencies across the country. Their role in the program includes not only providing information guide, psychological support, health education and counseling, but also networking the related health and welfare resources through personal home-visits, telephone, and mailing. The author reviewed articles and reports related effects of the Medicaid case management system. The case management approach was shown to be effective to improve the beneficiaries’ health-related quality of life and to decrease the cost of the medical treatment on outpatient clinics. The author proposes four policy directions as follows: building an integrated circulation system for the case management; development of standardized instruments and clinical guideline by the type of recipients; development of resources network system; development of integrated information system.

공공영역 사례관리자의 실천경험에 관한 질적 사례연구

본 연구의 목적은 공공영역 사례관리자의 실천 경험을 심층적으로 탐색하고, 이를 통해 공공영역 사례관리 실천의 현황과 과제 및 관련한 개선방안을 제시하는데 있다. 이를 위해 희망복지지원단 소속 통합사례관리사를 중심으로 공공영역에서의 사례관리 실천 경험과 의미를 탐색하였으며, 연구방법은 질적 사례연구방법을 활용하였다. 연구참여자는 대전과 충남지역에서 3년 이상 통합사례관리사로 활동하고 있는 10명을 선정하였다. 수집된 자료의 분석결과 연구참여자들의 경험은 총 72개의 의미 단위와 9개의 하위범주, 3개 영역의 주제로 도출되었다. 구체적으로 각각의 주제는 ‘축적된 경험을 통한 전문직 정체성의 확립’, ‘공공영역 사례관리의 특성과 장점’, ‘지속 가능한 실천을 위한 과제’로 나타났다. 이러한 연구결과를 토대로 공공영역 사례관리 사업과 관련하여 일관된 정책 방향의 제시 및 통합사례관리사를 위한 법적 제도적 지위 개선, 사례관리 실천역량을 강화하기 위한 제도화된 교육과 수퍼비전 체계의 현실화 등을 제언하였다. This study aims to explore the practice experience of case management in the public sector in-depth, and to analyze the current status, tasks and suggest ways to improve case management practice in the public sector. For this purpose, The experiences and meaning of case management practices in the public sector were explored within the integrated case managers of the Heemang Welfare Support Team using a qualitative case study method. The study selected 10 participants who were working as an integrated case managers for more than 3 years in the Daejeon and Chungnam area. As a result of analysis of collected data, 72 semantic units, 9 subcategories, and 3 topics were derived from participants experiences. The topics are establishment of professional identity through accumulated experience , characteristics and merits of case management in the public sector , and tasks to uphold sustainable practice . Based on these findings, the study suggests consistent policy directions related to case management program in the public sector, legal and institutional status improvement of integrated case managers, institutionalized education to strengthen case management practices, and realization of a supervision system.
