The Relationship of Leptin (+19) AG, Leptin (2548) GA, and Leptin Receptor Gln223Arg Gene Polymorphisms with Obesity and Metabolic Syndrome in Obese Children and Adolescents

Purpose: Obesity is defined as the abnormal or excessive accumulation of fat over acceptable limits. Leptin is a metabolic hormone present in the circulation in amounts proportional to fat mass. Leptin reduces food intake and increases energy expenditure, thus regulating body weight and homeostasis. Various polymorphisms are present in the leptin gene and its receptor. These polymorphisms may be associated with obesity. This study aimed to show the association of leptin (+19) AG, leptin (2548) GA, and Gln223Arg leptin receptor polymorphisms with obesity and metabolic syndrome in Turkish children aged 6-17 years, and to conduct further investigations regarding the genetic etiology of obesity. Methods: A total of 174 patients diagnosed with obesity and 150 healthy children who were treated at Tokat Gaziosmanpasa Medical School Hospital between September 2014 and March 2015 were included in this study. The ages of the children were between 6 and 17 years, and anthropometric and laboratory results were recorded. Genotyping of leptin (+19) AG, leptin (2548) GA, and leptin receptor Gln223Arg polymorphisms was performed by polymerase chain reaction. Results: An association between leptin receptor Gln223Arg gene polymorphism and obesity was detected. Conclusion: Further studies are needed to determine the role of genetic etiologies and to indicate the role of leptin signal transmission impairment in the pathogenesis of obesity. We hope that gene therapy can soon provide a solution for obesity.

Soil-pile interaction effects in wharf structures under lateral loads

Wharfs are essential to shipping and support very large gravity loads on both a short-term and long-term basis which cause quite large seismic internal forces. Therefore, these structures are vulnerable to seismic activities. As they are supported on vertical and/or batter piles, soil-pile interaction effects under earthquake events have a great importance in seismic resistance which is not yet fully understood. Seismic design codes have become more stringent and suggest the use of new design methods, such as Performance Based Design principles. According to Turkish Code for Coastal and Port Structures (TCCS 2008), theinteraction between soil and pile should somehow be considered in the nonlinear analysis in an accurate manner. This study aims to explore the lateral load carrying capacity of recently designed wharf structures considering soil-pile interaction effects for different soil conditions. For this purpose, nonlinear structure analysis according to TCCS (2008) has been performed comparing simplified and detailed modeling results.

The Selective Problem Solving Model (SPS) and Its Social Validity in Solving Mathematical Problems

The aim of this study was to investigate student acceptability as a form of social validation of the Selective Problem Solving (SPS) model that is used for developing creative problem solving skills, as well as for creatively solving problems. The model consists of six steps: definition of the target problem, identification of the source problem, solution of the target problem, construction of an original problem, solution of the original problem, and reflection. The study included 210 sixth and seventh-grade students in a city in the mid part of Turkey. A four-hour instruction consisting of two SPS lessons was designed and used for teaching in mathematics courses. A 20-item questionnaire was administered to the participants to measure their perceptions about the SPS after the instruction. One Sample t-test analysis showed that mean scores of all the items were higher than the criterion, meaning that students had high acceptability about the use of the SPS in teaching problem solving skills and creativity in mathematics. The results did not differ by gender, but by grade.

One surgeon experiences in childhood inguinal hernias

Purpose: In this manuscript we report one pediatric surgeon’s experience in childhood inguinal hernia repair. Methods: From 2005 to 2008, 402 children with inguinal hernias were operated on by one surgeon. A retrospective survey of their charts was carried out to evaluate the demographics and clinical aspects of these patients. Results: The ages ranged from 20 days to 16 years with a male-to-female ratio of 2.5:1. 64.9% right, 27.1% left, and 7.9% bilateral hernias. Hydroceles were present in 6.2% assosiated hernias. Incarceration occurred in 8.7% of children. An opposite-side hernia developed in 5.7%. 5.3 percent of patients with a hernia repair on the right side later developed a hernia on the left side, and 8.2% of patients with a hernia repair on the left side later developed one on the right side. 4.5 percent of all male patients in this series and 8.6% of female patients developed an opposite-side hernia. While overall recurrence rate was 1.2%, our recurrence rate was 0.25%. There was a 0.24% wound infection rate, and 1 (0.24%) testicle was atrophic at follow-up. Conclusion: In this study, in the recurrence of childhood hernia, the general surgeon’s intervention was the prominent cause. It is suggested by the study that inguinal hernias on the contralateral side becomes symptomatic within the first six months following initial operation.Therefor, close observation is needed in that time.

Alkaloids from Amaryllidaceae III. Alkaloids from the Bulbs of Pancratium maritimum

The extract from the bulbs of Pancratium maritimum L. afforded 12 alkaloids belonging to the skeletally six different groups of the Amaryllidaceae allcaloids. In this paper, the isolation and identification of (-)-N-dernethyl-galanthamine (1), (+)-tazettine (2) and (-)-2-O-demethylmontanine (3) are described. Their structures have been determined by using extensive spectroscopic techniques. This is the first report describing the occurrence of 1 and 3 in this plant.

Determination of Biogenic Amines in Fish Products

Concentrations of 5 biogenic amines were simultaneously determined in 63 samples of fish products using HPLC. Fish products were purchased from different cities in Turkey. Fish samples included canned sardines, canned mackerel, canned tuna, and marinated anchovies. The total level of biogenic amines ranged from 26.58 to 406.55 mg/kg. Histamine (HIS) and cadaverine (CAD) were the dominant biogenic amines. Concentrations of tyramine (TYR) and HIS were less than 50 and 100 mg/ kg, respectively. The relationship between formation of biogenic amines in fish products and thiobarbituric acid (TBA), total volatile basic nitrogen (TVB-N), and pH values was also investigated. There were significant negative correlation (p<0.01) between TBA, and TVB-N, pH respectively. Significant positive correlation was found between TVB-N, and pH, protein, and CAD, TYR, and HIS. There were significant (p<0.01) positive correlations between TBA, TYR, tryptamine, pH, and TVB-N values in fish samples.

METALLIC INTERFACES IN HARSH CHEMO-MECHANICAL ENVIRONMENTS

The use of multiscale modeling concepts and simulation techniques to study the destabilization of an ultrathin layer of oxide interface between a metal substrate and the surrounding environment is considered. Of particular interest are chemomechanical behavior of this interface in the context of a molecular-level description of stress corrosion cracking. Motivated by our previous molecular dynamics simulations of unit processes in materials strength and toughness, we examine the challenges of dealing with chemical reactivity on an equal footing with mechanical deformation, (a) understanding electron transfer processes using first-principles methods, (b) modeling cation transport and associated charged defect migration kinetics, and (c) simulation of pit nucleation and intergranular deformation to initiate the breakdown of the oxide interlayer. These problems illustrate a level of multi-scale complexity that would be practically impossible to attack by other means; they also point to a perspective framework that could guide future research in the broad computational science community.

Warfarin is an anticoagulant used in a variety of clinical indications and may rarely cause severe bleeding that can be life-threatening.Although intramuscular bleeding frequently occure secondary to trauma,it can be induced in cases that cause bleeding tendency.Hematoma is often treated with conventional methods.However, surgical decompression can also be applied.Case Report;A 71 year old male patient admitted to our clinic with increased skin temperature, swelling and redness on the right leg.Warfarin was the only drug in the present treatment that cause bleeding tendency.During the physical examination, swelling, skin redness, echymosis, decreased motility, pain with palpation and tenderness were deteceted on the gastrocnemius location of the right leg.There was no other fi nding in the rest systemic examination.Magnetic resonance imaging (MRI) of the right lower limb of the patient showed a mass lesion on posteromedial of right cruris, displacing the medial head of the gastrocnemius muscle laterally(fig 1).Considering the datas of the performed imaging methods and using an ultrasonography-assisted GF-multipurpose drainage pigtale catheter,intracavitary administration of 5 mg tPA and 25 cc serum sale mixture was performed.Subsequently the catheter was clamped for a 25-30 minutes time and spontaneous drainage occured. After the intervention,a compression bandage was done with elastic bandage in order to avoid reloading of the cavity.Ultrasonographic control was performed after each step of the intervention to check a possible presence of an organised hematoma and intervention went on till there was no liquid rest in the location of hematoma. The total recovery was also proved by magnetic resonance imaging after total clinical recovery was deteceted (fig2).This case report represents that the addition of fi brinolytics to the standard drainage techniques is a safe and effective therapy method in organised hematoma as well as abscess complicated with surgery.
