        Activity, Participation, and Goal Awareness After Acquired Brain Injury: A Prospective Observational Study of Inpatient Rehabilitation

        Objective To examine the frequency and timing of inpatient engagement in meaningful activities within rehabilitation (within and outside of structured therapy times) and determine the associations between activity type, goal awareness, and patient affect.Methods This prospective observational study performed behavioral mapping in a 42-bed inpatient brain injury rehabilitation unit by recording patient activity every 15 minutes (total 42 hours). The participants were randomly selected rehabilitation inpatients with acquired brain injury; all completed the study. The main outcome measures included patient demographics, observation of activity, participation, goal awareness, and affect.Results The inpatients spent 61% of the therapeutic day (8:30 to 16:30) in their single room and were alone 49% of the time. They were physically socially inactive for 76% and 74% of their awake time, respectively, with neutral affect observed for about half of this time. Goal-related activities were recorded for only 25% of the inpatients’ awake time. The odds of physical activity were 10.3-fold higher among in patients receiving support to address their goals within their rehabilitation program (odds ratio=10.3; 95% confidence interval, 5.02–21.16).Conclusion Inpatients in a mixed brain injury rehabilitation unit spent a large amount of their awake hours inactive and only participated in goal-related activities for a quarter of their awake time. Rehabilitation models that increase opportunities for physical, cognitive, and social activities outside of allied health sessions are recommended to increase overall activity levels during inpatient rehabilitation.

        Evaluation of the Cost Effectiveness of Routine Histopathologic Femoral Head Analysis in Hip Arthroplasty

        Purpose: Histopathologic analysis of femoral head specimens following total hip arthroplasty (THA) is a routine practice that represents a significant use of health care resources. However, it occasionally results in discovery of undiagnosed hematopoietic malignancy and other discrepant diagnoses such as avascular necrosis. The purpose of this study was to determine the rate of discordant and discrepant diagnoses discovered from routine histopathological evaluation of femoral heads following THA and perform a cost analysis of this practice. Materials and Methods: A review of patients undergoing primary THA between 2004-2017 was conducted. A comparison of the surgeon’s preoperative and postoperative diagnosis, and the histopathologic diagnosis was performed. In cases where the clinical and histopathology differed, a review determined whether this resulted in a change in clinical management. Medicare reimbursement and previously published cost data corrected for inflation were utilized for cost calculations. Results: A review of 2,134 procedures was performed. The pathologic diagnosis matched the postoperative diagnosis in 96.0% of cases. Eighty-three cases (4.0%) had a discrepant diagnosis where treatment was not substantially altered. There was one case of discordant diagnosis where lymphoma was diagnosed and subsequently treated. The cost per discrepant diagnosis was $141,880 and per discordant diagnosis was $1,669 when using 100% Medicare reimbursement and Current Procedural Terminology (CPT) code combination 88304+88311. Conclusion: Histopathologic analysis of femoral head specimens in THAs showed an association with high costs given the rarity of discordant diagnoses. Routine use of the practice should be at the discretion of individual hospitals with consideration for cost and utility thresholds.

        Can Faith and Spirituality Promote Healing and Post-Traumatic Growth in the Aftermath of Trauma? What We Can Learn from Cambodian Trauma Survivors and Those Who Support Them

        Although precise numbers are difficult to estimate, close to 1 billion children under the age of 18 experience trauma every year. Many of these children will go on to thrive despite their lived experiences of trauma, whereas some will not. Thriving in the aftermath of trauma has been defined by some researchers as Post-Traumatic Growth. This article will explore Post-Traumatic Growth (PTG) in a Cambodian context, with trauma survivors who have been supported by Non-Government Organisations (NGOs) and looking specifically at faith and spirituality, which are recognised within a PTG framework. This article will present some of the findings stemming from a wider piece of research conducted in Cambodia, seeking to understand resilience in the aftermath of trauma (Wyatt, 2021). The data captured in this research contains the perspective of 26 young Cambodian trauma survivors. These adult participants (aged 18-30) not only survived significant trauma in their early childhood but appeared to do remarkably well in life. This research set out to understand what role, if any, faith did play in building the resilience for these individuals. Furthermore, this article presents the perspectives of NGOs and the people who support trauma survivors in Cambodia and what they can add to the PTG narrative. Specifically, this article will explore how individual coping methods such as social resources, faith and spiritual practices, culture and community can facilitate positive adaptation and PTG for these young trauma survivors.

        기업의 생산성향상과 이익률에 영향을 줄 수 있는 지식이 경제 전반에 확산되어 나가는 과정은 한 나라의 경제발전속도에 영향을 미치는 중요한 요인이다. 기업 측면에서는 도입하려는 기술이 도입 후에 그 기업의 이익을 높여 줄 수 있다면 도입하지 않을 이유가 없다. 하지만 미래 수요의 불확실성이나 기술발전 방향의 불확실성 등으로 해서 기업으로서는 도입 후의 이익을 정확히 사전적으로 측정하기는 어렵다. 본 논문에서는 학계에서 일반적으로 사용되고 있는 두 가지 지식확산 모델을 설명하고자 한다. 그 하나는 하나의 새로운 기술이나 상품이 시간이 흐름에 따라 어떻게 전체 사용 가능자(population)에게 확산되는 지를 보여주는 1) Epidemic Diffusion Model (흔히 5자형 - Sigmoid - 모델이라고도 한다. )과 어떤 도입자가 어느 시점에서 대상이 된 새로운 기술을 도입할 것인지 아닌지를 결정하는 모델로서 2) Probit Diffusion Model (프로빗 모델)을 중심으로 한다 그리고 이러한 지식확산과정과 속도에 영향을 줄 수 있는 기업 내부적 요인으로서 도입하고자 하는 기업의 누적된 경험이 중요하다는 것과 기업 외부적 요인으로서 네트웍 효과와 같은 요인들을 설명하였다.

        Elevated folic acid results in contrasting cancer cell line growth with implications for mandatory folic acid fortification

        Purpose: The initiation of mandatory folic acid fortification using pteroylmonoglutamic acid (PteGlu) has reduced the rate of congenital malformations. However, it also appears to be responsible for several adverse effects, including increased cancer incidence. This may be related to physicho-chemical characteristics of PteGlu. This study examines the potential effect of high concentrations of PteGlu on a population subjected to mandatory folic acid fortification using an in vitro model. Methods: Caco-2 (colorectal cancer) and MCF7 (breast cancer) cell lines were cultured at 6 different PteGlu concentrations (0, 0.1, 1, 50, 250, and 500μg/ml) for 6 days. Cell growth was determined using thiazolyl blue tetrazolium bromide assay. The genotype of dihydrofolate reductase 19bp deletion/insertion (DHFR 19-del) was also scored in cell lines using a restriction fragment length polymorphism technique to examine whether genetic variations may factor in cell proliferation. Results: PteGlu exhibited differential growth promoting properties between cell lines. Caco-2 cells did not show a significant growth difference at low concentrations compared to control, however, at higher concentrations, the growth showed a contrasting trend in the early experimental period, while MCF7 showed enhanced cell growth at all concentrations. The DHFR 19-del genotype differed in the two cell lines. Conclusions: Altered response to PteGlu by Caco-2 and MCF7 may reflect a tissue specific disease aetiology or genotype specific differential enzyme activity, for example by DHFR, to critical levels of PteGlu. As folic acid fortification is a blanket intervention, and DHFR and other enzyme activities vary between individuals, PteGlu intake may have an as yet undefined effect on health. These findings may be relevant when considering mandatory folic acid fortification for disease prevention.

        Pan-genome Analysis Reveals Comparative Genomic Features of Central Metabolic Pathways in Methylorubrum extorquens

        Methylobacterium is a genus belonging to the pink-pigmented facultative methylotrophs, which can use C1 substrates as a sole carbon source. Methylorubrum extorquens (formerly Methylobacterium extorquens) is a potentially valuable bacteria in methanol-based bioindustry producing high-value-added chemicals. Thus, it is critical to understand the core metabolic pathways of M. extorquens strains for further advancement as a platform bacterium in the bioindustry. However, there are no reported systems biological approaches comparing different M. extorquens strains. This paper focuses on comparing the genomic properties of seven completely sequenced M. extorquens strains, including M. extorquens PA1, which was re-sequenced, and M. extorquens ATCC 55366, which was newly sequenced in this study. Pan-genome analysis indicated that a total of 10,431 ortholog clusters composed the pan-genome of these M. extorquens strains, including 3,507 core genome, 1,860 accessory genome, and 5,064 unique genome clusters. The functional annotation discovered that genes related to signal transduction were the most abundant in the pan-genome. Pan-genome analysis of two different habitat-specific groups revealed that the strains isolated from various soil environments had a higher percentage of genes involved in mobilome in their core genome. Subsequent exploration for secondary metabolite biosynthesis clusters denoted the unique ability of M. extorquens AM1 to produce various toblerols. M. extorquens PA1 could be a strong candidate for industrial strain development because of its minimal genome size with fully conserved central metabolism. This study provides the first inclusive insight into the differences in genomic characteristics of central metabolic networks of M. extorquens.
