The Goldilocks technique for breast reconstruction utilizes redundant mastectomy flap tissue to construct a breast mound. This technique is suitable for women who decline, or are poor candidates for, traditional postmastectomy reconstruction. Moreover, this technique can be applied in secondary operations after the failure of initial reconstruction efforts. A 74-year-old patient underwent the Goldilocks procedure after reconstruction failure with an implant and acellular dermal matrix. At her 6-month follow-up, the cosmetic outcome of the procedure was satisfactory, and no complications were noted. Therefore, the Goldilocks procedure is a safe alternative to reconstruct breast mounds following reconstruction failure, especially in obese patients.

An experimental study was performed to investigate the dissipated energy to failure of sand samples subjected to uniform and non-uniform cyclic simple shear loading. The hypothesis evaluated was that for a given initial sample state the specific dissipated energy required to reach failure should be reasonably constant independent of the type of stress-time history used in the testing. Test samples consisted of dry Ottawa sand prepared at nine different initial states that were subjected to different stress controlled cyclic horizontal shear loading waveforms that included 15 uniform sinusoidal waves and up to 33 non-uniform loading wave forms. The experimental program presented showed that the measured cumulative dissipated specific energy to failure, defined when the double amplitude shear strain reaches 7.5%, for the different sample initial states was reasonably constant but with coefficients of variation ranging between 13 to 44%. As expected, the cumulative dissipated energy increased with increasing initial stress level and relative density. The findings support the notion that specific dissipated energy can be used as a reasonable failure predictor for uniform dry sands based on their initial state and are independent of the type of cyclic simple shear loading waveform using in the testing.

Background/Aims Different non-invasive diagnostics strategies have been used to assess patients with gastroesophageal reflux. Gastroesophageal reflux disease (GERD) questionnaire (GerdQ) is a 6-item, easy to use questionnaire that was developed primarily as a diagnostic tool for GERD in primary care. Our aim was to validate and assess diagnostic utility of GerdQ questionnaire in Mexican patients in the primary care setting. Methods The study was performed in 3 phases: (1) a questionnaire translation and comprehension study (n = 20), (2) are a reproducibility and validation study (50 patients and 50 controls) and (3) a study to assess the clinical utility in 252 subjects with GERD symptoms. Diagnostic accuracy was calculated using endoscopy and/or pH-metry as the gold standard. Results Internal consistency measured by the Cronbach``s a coefficient was 0.81 for patients and 0.90 for healthy controls, with a mixed coefficient of 0.93. Reproducibility for GerdQ was very good and its discriminating validity was 88%. Most of the patients with erosive reflux and non-erosive reflux with abnormal pH-metry had scores > 8, meanwhile most of the patients with functional heartburn and hypersensitive esophagus had < 8. Sensitivity, specificity and positive predictive value of GerdQ compared to the gold standard were 72%, 72% and 87%, respectively.Conclusions In Mexico, the GerdQ questionnaire Spanish validated version is useful for GERD diagnosis in the primary care setting. Antiamoebic Activity of Petiveria alliacea Leaves and Their Main Component, Isoarborinol

Petiveria alliacea L. (Phytolacaceae) is a medicinal plant with a broad range of traditional therapeutic properties, including the treatment of dysentery and intestinal infections caused by protozoan parasites. However, its effects against Entamoeba histolytica have not been reported yet. We investigated the antiamoebic activity present in the leaves of P. alliacea Antiamoebic activity was evaluated in methanolic and aqueous extracts, as well as in the hexanic, methanolic, and EtOAc fractions. The P. alliacea methanolic extract showed a better antiamoebic activity than the aqueous extract with an IC<sub>50</sub> = 0.51 mg/ml. Likewise, the hexanic fraction was the most effective fraction, showing a dose-dependent activity against E. histolytica, with an IC<sub>50</sub> = 0.68 mg/ml. Hexanic subfraction 12-19 showed the highest antiamoebic activity at 0.8 mg/ml, producing 74.3% growth inhibition without any toxicity in mammal cells. A major component in subfraction 12-19 was identified as isoarborinol, which produced 51.4% E. histolytica growth inhibition at 0.05 mg/ml without affecting mammal cells. The P. alliacea leaf extract has antiamoebic activity that can be attributed to a major metabolite known as isoarborinol.

Abstract. This paper presents the evolution the public policy on the health attention of hearing impaired people in Mexico has had. A socio-historical reconstruction has been made from those policies in which the backlog generated from its society integration within the field of the laws, plans and programs of social development is stated. Likewise, there is a mention of the findings detected from the execution of a social action project focused on that group; those findings set the basis for further proposals or lines of investigation. As a conclusion, there are given guidelines to link the public policy proposals with the challenges for the social action with that specific population. Resumen. En este trabajo se presenta la evolución que ha tenido la política pública en materia de atención a la salud de las personas con discapacidad auditiva en México. Se hace una reconstrucción socio-histórica de dichas políticas, en las cuales se pone de manifiesto el rezago generado con respecto a su integración a la sociedad dentro del marco de las leyes, artículos, planes y programas de desarrollo social. Asimismo, se hace mención de los hallazgos detectados a partir de la implementación de un proyecto de intervención social enfocado a este grupo poblacional, lo cual sienta las bases para el planteamiento de propuestas y líneas de investigación a futuro. Finalmente, se presenta a manera de conclusión, algunas directrices que articulan las propuestas de política pública con los retos para la intervención social con grupos de población con discapacidad auditiva.
Optical Observations with Milliarcsecond Resolution of Stars, Their Environments and Companions

Observations with milliarsecond resolution using the Navy Optical Interferometer have been obtained for a number of stellar systems which include high-mass binaries, eclipsing binaries, and radio stars. These observations also reveal the previously unseen companions in single-lined spectroscopic binaries via directly measured flux ratios. We will present examples of published and ongoing research efforts of these systems to illustrate how an optical interferometer contrib\-utes to our knowledge of stars, their environment, and companions. These studies include a conclusive revealing of the previously unseen companion in the single-lined binary Φ Herculis, the direct determination of orbital parameters in the wide and close orbits of Algol, and revealing the orbit of β Lyrae with spatially resolved images of the Hα emission.

Currently, there are no antiviral drugs that effectively reduce the risks and treat the symptoms associated with dengue virus (DENV). Consequently, efforts remain primarily focused on transmission reduction. One such effort concerns DENV receptors in mosquito vectors Aedes aegypti and Aedes albopictus. Despite a lack of direct evidence demonstrating the binding of DENV to cells in mosquito vectors, one putative DENV binding protein has been α-enolase. To develop a deeper understanding, this study tested whether DENV proteins bind to enolase localized in the cytoplasmic membrane of C6/36 cells using both anti-enolase-specific antibodies, and by colocalization analysis, using confocal microscopy. Additionally, to probe the interaction of enolase with the DENV E protein, we performed a docking analysis using PatchDock and FireDock software packages. Study results demonstrate that the DENV E protein interacts with enolase in the plasma membrane of C6/36 cells of Ae. albopictus. Specific anti-enolase antibodies were found to inhibit DENV infection of these cells. Moreover, enolase was found to be localized to the cytoplasmic mem brane, cytoplasm, and nucleus. These combined findings suggest that enolase participates in the entrance mechanism of DENV into vector cells.
