Background: Depressive disorders (DD) affect not only mood and behavior but also various physical functions. Traditional Chinese medicine (TCM) has been shown to have some benefits in treating DD. However, one formula or one single herb might be not show high efficacy when used to treat depression. Thus, this study aimed to examine the core prescription pattern of Chinese herbal medicine (CHM) among patients with DD in Taiwan as a reference for related research and clinical applications. Methods: All patients, who had been diagnosed with major depressive disorder or minor depression or dysthymia without any other baseline diseases and had at least one CHM outpatient clinical visit from 2002 to 2011, were extracted from three randomly sampled cohorts, namely the 2000, 2005 and 2010 cohorts of the National Health Insurance Research Database (NHIRD) of Taiwan. The collected data was analyzed to explore the patterns of herbal products. Results: There were 197,146 patients with a diagnosis of DD and of these 1806 subjects had only a diagnosis of DD and utilized CHM. The most common formula was Gan-Mai-Da-Zao-Tang (12.19%), while Suan-Zao-Ren (3.99%) was the most commonly prescribed single herb. The core pattern of prescriptions consisted of a combination of Gan-Mai-Da-Zao-Tang, Jia-Wei-Xiao-Yao-San, Chai-Hu-Jia-Long-Gu-Mu-Li-Tang, He-Huan-Pi, Yuan-Zhi and Shi-Chang-Pu. Conclusions: This study describes the CHM core prescription pattern used to treat patients in Taiwan with DD and it is a potential candidate for study in future pharmacological or clinical trials targeting DD.
Hybrid Navigation of a Four-Wheeled Tour-Guide Robot

This paper develops methodologies and techniques for motion control and autonomous navigation of a tour-guide robot with a four-wheeled omni-directional mobile platform. A nonlinear unified kinematical control method is presented for point stabilization and trajectory tracking of a mobile robot which equipped with four independent driving omni-directional wheels equally spaced at 90 degrees between adjacent wheels. A hybrid navigation method is proposed to achieve safely autonomous navigation of the tour-guide robot; this approach includes two behaviors: one is the point-to-point trajectory tracking, and the other is the obstacle avoidance function using the traversability distance histogram (TDH) method to avoid barriers in museums. The effectiveness and merit of the proposed techniques are exemplified by conducting several experiments on an experimental four-wheeled omni-directional tour-guide robot.
Triangular No-arbitrage Estimation through Bitcoin: An Application in Venezuelan Bolivars

In this study, we apply the Bitcoin to estimate the price of the Venezuelan bolivar, due to the unreliability of this currency’s official exchange rate. Our approach is based on the triangular no-arbitrage condition and takes the Bitcoin as an intermediate currency. To verify its validity, in addition to bolivars, six currencies are first considered in the empirical study. We find that trading through the Bitcoin market produces higher transaction costs or requires higher risk compensation than trading through the conventional exchange market. Then, we explore the Venezuelan case. Comparing the estimates of the black market bolivars, which are generated from several popular media sources and the Web site Dolartoday.com, using Bitcoin can produce reliable bolivar exchange rates quickly and easily. To sum up, we verify the feasibility of using the triangular no-arbitrage condition in foreign exchange markets to estimate exchange rates through the Bitcoin. This is especially useful when capital controls exist such as they do in Venezuela.

A about 2-month- old, mixed female cat was refered to Yeon Chang Veterinary Clinic in Taiwan. Becausethis patient was wandering cat, precise history was not known. At first admision, miosis, ptosis and protrusion of the nictitating membrane was observed in the right sided eye, and also slight miosis was found in the left sided eye. The patient was diagnosed into feline HS. Oculo-AP and injection-AP with dexamethasone were applied to this patient. minutes. In addition, injection-AP with dexamethasone (0.2 ml/acupoint) at BL01-Jing Ming, BL02-Zan Zhu and ST01-Cheng Qi. After AP treatment, prolapse of the nictitating membrane was amazingly disapeared and pupil was dilatated at sesion 1. Ocular findings at sesion 2 (one day after sesion 1) were maintained with nearly normal state. Accordingly, the present patient was a case with feline HS that showed favorable therapeutic effect by AP treatment.

This study investigates the resonated diurnal impact of the surface wind forcing from several atmospheric wind products, including CCMP (Cross-Calibrated Multi-Platform), ERA Interim, NCEP-2 (NCEP-DOE AMIP-II Reanalysis), MERRA (Modern-Era Retrospective analysis for Research and Application), NARR (Northern American Regional Reanalysis) and NAM (North American Mesoscale Forecast System), on the wind-driven near-inertial ocean motions in the northern Gulf of Mexico by numerical simulation, and in comparison, with NDBC buoy surface observed winds. Our analyses show that the near-inertial wind power input, which can affect vertical mixing in the water column, is closely associated with the variability of land-sea breeze by various wind products. In comparison with buoy observations in the northern Gulf of Mexico, the MERRA has the worst land-sea breeze representation, while the NCEP2 and CCMP, which are chosen to represent the coarsest and medium–high horizontal resolution, have the best performance. We further show that by comparison of the buoy data, that the near-inertial wind power input derived from all the wind products is underestimated by 30–50% approximately.

Background/Aims: Direct-acting antivirals (DAAs) have been approved for hepatitis C virus (HCV) treatment in patients with end-stage renal disease (ESRD) on hemodialysis. Nevertheless, the complicated comedications and their potential drug-drug interactions (DDIs) with DAAs might limit clinical practice in this special population. Methods: The number, class, and characteristics of comedications and their potential DDIs with five DAA regimens were analyzed among HCV-viremic patients from 23 hemodialysis centers in Taiwan. Results: Of 2,015 hemodialysis patients screened in 2019, 169 patients seropositive for HCV RNA were enrolled (mean age, 65.6 years; median duration of hemodialysis, 5.8 years). All patients received at least one comedication (median number, 6; mean class number, 3.4). The most common comedication classes were ESRD-associated medications (94.1%), cardiovascular drugs (69.8%) and antidiabetic drugs (43.2%). ESRD-associated medications were excluded from DDI analysis. Sofosbuvir/velpatasvir/voxilaprevir had the highest frequency of potential contraindicated DDIs (red, 5.6%), followed by glecaprevir/pibrentasvir (4.0%), sofosbuvir/ledipasvir (1.3%), sofosbuvir/velpatasvir (1.3%), and elbasvir/grazoprevir (0.3%). For potentially significant DDIs (orange, requiring close monitoring or dose adjustments), sofosbuvir/velpatasvir/voxilaprevir had the highest frequency (19.9%), followed by sofosbuvir/ledipasvir (18.2%), glecaprevir/pibrentasvir (12.6%), sofosbuvir/velpatasvir (12.6%), and elbasvir/grazoprevir (7.3%). Overall, lipid-lowering agents were the most common comedication class with red-category DDIs to all DAA regimens (n=62), followed by cardiovascular agents (n=15), and central nervous system agents (n=10). Conclusions: HCV-viremic patients on hemodialysis had a very high prevalence of comedications with a broad spectrum, which had varied DDIs with currently available DAA regimens. Elbasvir/grazoprevir had the fewest potential DDIs, and sofosbuvir/velpatasvir/voxilaprevir had the most potential DDIs. (Clin Mol Hepatol 2021;27:186-196)

Background/Aims: Only moderate to severe Crohn`s Disease (CD) patients without a satisfactory conventional therapy effect are eligible to get reimbursement from the National Health Insurance of Taiwan for using adalimumab. These are more stringent criteria than in many Western countries and Japan and Korea. We aim to explore the efficacy of using adalimumab in CD patients under such stringent criteria. Methods: A retrospective analysis was conducted in nine medical centers in Taiwan and we collected the results of CD patients receiving adalimumab from Sep 2009 to Mar 2014. The clinical characteristics, response measured by CDAI (Crohn`s Disease Activity Index), adverse events and survival status were recorded and analyzed. CR-70, CR-100, and CR-150 were defined as attaining a CDAI decrease of 70, 100 or 150 points compared with baseline. Results: A total of 103 CD patient records were used in this study. Sixty percent of these patients received combination therapy of adalimumab together with immunomodulators. CR-70 was 68.7%, 74.5% and 88.4% after week 4, 8 and 12 of treatment, respectively. The steroid-free rate, complications and survival were 47.6%, 9.7% and 99% of patients, respectively. In considering the mucosal healing, only 25% patients achieve mucosal healing after treatment for 6 to12 months. Surgery was still needed in 16.5% of patients. Combination treatment of adalimumab with immunomodulators further decreased the level of CDAI at week 8 when compared with the monotherapy. Conclusions: Even under the stringent criteria for using adalimumab, the response rate was comparable to those without stringent criteria. (Intest Res 2014;12:287-292)

Objective: Previous single photon emission computed tomography (SPECT) studies have demonstrated that co-injection of radiotracers 99mTc-TRODAT and 123IBZM can assess both presynaptic, dopamine transporters (DATs), and postsynaptic, dopamine D2/D3 receptors, markers of dopaminergic system simultaneously. The aim of this study was to determine both DATs and dopamine D2/D3 receptors in drug naïve schizophrenic patients. Methods: Severn drug naïve schizophrenic patients and eleven age-matched healthy controls were recruited. Each subject received one SPECT measurement after intravenous co-administration of 99mTc-TRODAT and 123IBZM and one brain MRI scan. A ratio equilibrium model was applied to calculate the specific uptake ratio (SUR) which corresponds to the ratio of Bmax (receptor density) and KD (affinity). Results: The results showed that the SUR of DATs binding was not significantly different between healthy controls and drug naïve schizophrenic patients (2.92±0.62 vs. 2.65±0.27, p=0.052) and, however, the SUR of dopamine D2/D3 receptors binding was significantly lower in drug naïve schizophrenic patients than those in healthy controls (2.79±0.54 vs. 3.90±0.58, p=0.001). Additionally, there was a significant correlation between left and right binding for both DATs (r=0.80, p=0.003) and dopamine D2/D3 receptors (r=0.96, p=0.000) in healthy controls but not seen in drug naïve schizophrenic patients (r=0.54, p=0.20 for DATs, r=0.70, p=0.08 for dopamine D2/D3 receptors). Conclusion: This study provides evidences to support the idea that abnormalities of dopaminergic system in schizophrenia and further elucidates the disruption of inter-hemispheric interaction of postsynaptic and presynaptic markers.

Background/Aims: Incidences of inflammatory bowel disease (IBD), ulcerative colitis (UC), and Crohn’s disease (CD), havebeen increasing in Asia. In this study, we report the relevant clinical characteristics and determined the epidemiological trend ofIBD in Taiwan from 2001 to 2015. Methods: A retrospective study was conducted to analyze data recorded from January 2001through December 2015 in the registered database compiled by the National Health Insurance and provided by the Ministry ofHealth and Welfare, Taiwan. Results: A total of 3,806 patients with catastrophic IBD illness were registered from 2001 to 2015 inTaiwan (CD, 919; UC, 2,887). The crude incidence of CD increased from 0.17/100,000 in 2001 to 0.47/100,000 in 2015, whereasthat of UC increased from 0.54/100,000 in 2001 to 0.95/100,000 in 2015. The prevalence of CD increased from 0.6/100,000 in2001 to 3.9/100,000 in 2015, whereas that of UC increased from 2.1/100,000 in 2001 to 12.8/100,000 in 2015. The male-to-femaleratio in the study sample was 2.19 for CD and 1.62 for UC. The median age of those registered with CD was lower than that ofthose registered for UC: 38.86 and 44.86 years, respectively. A significantly greater increase in CD incidence rate was identifiedamong 20 to 39-year-old compared with other age groups. Conclusions: Using Taiwan’s nationwide insurance database, we determinedthat the number of patients with CD increased more rapidly during the study period than the number of patients withUC, especially among age 20 to 39-year-old, resulting in a decreased UC-to-CD ratio.
