Multiple fractures are becoming increasing due to traffic and industrial developments. Ipsilateral femur and tibia fractures are caused by high energy trauma and associated with many problems such as hemorrhagic shock, longer morbidity, delayed union of nonunion, knee stiffness etc. Recently the principles of treatment are directed to rigid internal fixation and early mobilization for more satisfactory functional recovery. Since July 1990, we managed 6 patients who had ipsilateral femur and tibia shaft fractures mainly with intramedullary nailing. The results are as follows ; 1. All the 6 cases were accompanied by multiple fractures and/or dislocations. Ipsilateral knee injuries were combined in 2 cases. 2. As for the femur, intramedullary nails were used in all cases. Intramedullary nailing was used for the tibia in 5 cases, and external skeletal fixator in the other one. 3. All the fractures united. Union time of the femoral fractures ranged from 6 to 8 months(mean : 29 weeks ; 6.7 months). Tibia shaft fractures united at from 14 to 32 weeks(mean : 22 weeks ; 5.2 months) postoperatively. 4. 3 Knees recovered full range of motion. But in the other 3 knees, limitations of further flexion to average 100 degrees were noted.

Normal feet have the longitudinal arch for the distribution of the weight. The longitudinal arch is known lateral and medial longitudinal arch. Collapsed arch of the foot is flat foot and it is the cause of the foot pain. Of course the low arch does not always make a foot problem in sports and daily activity. Longitudial foot arch is visible in the standing foot lateral radiograph, Harris mat and visual observation. In Radiographic study the lateral Talo-lst Metatarsal angle is known in standing foot lateral film. But the longitadinal medial arch is not correctly obtained in Talo-lst Metatarsal angle. So this study for Calcaneo-lst metatarsal angle in standing lateral foot radiograph is more valuable angle in medial longitadinl arch. And we do the study on difference in the longitadinal arch in normal weight versus overweight person.

The purpose of operative treatment in thoracolumbar spine fractures is anatomical reduction and stabilization of the fractures and thus promote neurological recovery and early rehabilitation. To achieve these goals, complete preoperative assessment and recognition of the fracture pattern is necessary in addition to good surgical technique. The authors analyzed 27 cases of thoracolumbar spine fractures operated from Jun. 1994 to Dec.1995 at the Department of orthopaedic surgery, Dan-kook University Hospital. All cases were analysed according to McAfee classification. In deciding the appropriate surgical approach, fracture pat-tern, stability and neurologic status were considered. The result were as follows; 1. Unstable burst fracture, 11 cases, was the most frequent type. 2. 1st lumbar fracture was the most common level in this series. 3. Fall down, 15 cases out of 27, was the most common cause on thoracolumbar spine fracture. 4. Anterior approach was performed in 8 cases and posterior approach was performed in 19cases. 5. Good results were obtained through firm fixation and early rehabilitabion.

Reye syndrome is a disorder of unknown etiology in which a child recovering form a viral prodrome suddenly develops pernicious vomiting & eventually develops neurologic changes. The syndrome is defined to acute noninflammatory encephalopathy with fatty degeneration of liver and other organs. The coagulopathy or Reye syndrome is associated with proloned PT caused by the mitochondrial injury compromising synthesis of proteins and clotting factors in liver. In this case, we observed ICH on right parietal and occipital area in brain CT. Actually, bleeding in the major organ is extremely rare in Reye syndrome, this may be the first case report of CNS bleeding in Korea associated with Reye syndrome. In addition to case report, we present a brief review of the related literatures.

Angular deformities of the distal femur occur in congenital diseases or due to acquired causes, such as malunion after a fracture of the distal femur. Angular deformities of the lower extremities affect the mechanical axis, causing changes in the weight pressure on the articular surface. As a result, angular deformities quicken the progression of osteoarthritis. Therefore, correction of deformities should be performed to prevent the progression of osteoarthritis. Distal femoral osteotomy is one of the methods to correct angular deformities in unicompartmental osteoarthritis. However, femoral supracondylar dome osteotomy with retrograde intramedullary nailing in the distal femur with a varus deformity has been rarely reported. Herein, we describe a technique for femoral supracondylar dome osteotomy with retrograde intramedullary nailing in a varus deformity after a pathologic fracture of giant cell tumor in the distal femur with a review of the relevant literature.

Purpose: To evaluate the radiologic and functional outcomes of medial open wedge high tibial osteotomy (HTO) combined with arthroscopic procedure in patients with medial osteoarthritis. Materials and Methods: From June 1996 to March 2010, 26 patients (32 knees) who underwent medial open wedge osteotomy and arthroscopic operation for medial osteoarthritis were retrospectively reviewed. Measurements included hip-knee-ankle (HKA) angle, femorotibial angle, medial proximal tibial angle, posterior tibial slope angle, and Kellgren-Lawrence grade. Clinical evaluation was performed using Lysholm knee scoring scale and knee and function score of the American Knee Society. Results: Differences between the mean preoperative and postoperative measurements were significant in all angles including the HKA angle (-5.7° and +5.5°), femorotibial angle (-1.9° and +9.8°), and medial proximal tibial angle (82.9° and 90.5°) (p<0.05). Mean Lysholm knee scoring scale was 63.6 preoperatively and 88.7 at the last follow-up, mean Knee Society knee score was 61.2 and 86.6, and mean function score was 59.3 and 87.2, respectively. All differences were significant (p<0.05). Conclusions: Medial open wedge HTO in combination with arthroscopic procedure is an effective treatment method for medial osteoarthritis to treat varus deformity and an intra-articular lesion.
Case Report : Baker`s Cyst Filled with Hematoma at the Lower Calf

Baker’s cyst is a distension of the gastrocnemius-semimembranosus bursa of the knee, which communicates with the posterior portion of the joint capsule. Baker’s cyst is commonly located in the inferomedial or superficial layers of the knee joint and rarely extends laterally or proximally. Complications of Baker’s cysts are dissection, rupture, pseudothrombophlebitis, leg ischemia, nerve entrapment, and compartment syndrome. However, hematomas in the Baker’s cyst have not been reported in Korea. We report a rare case of hematoma in the Baker’s cyst with subfascial extension into the calf. The hematoma was demonstrated by magnetic resonance imaging and removed by mass excision.
