        Original Article : Red Cell Distribution Width: A Novel Marker of Activity in Infl ammatory Bowel Disease

        Background/Aims: Studies concerning red cell distribution width (RDW) for use in the assessment of infl ammatory bowel disease (IBD) activity are limited. We investigated whether RDW is a marker of active disease in patients with IBD. Methods: In total, 61 patients with ulcerative colitis (UC) and 56 patients with Crohn`s disease (CD) were enrolled in the study group, and 44 age- and-sex-matched healthy volunteers were included as the control group. A CD activity index >150 in patients with CD indicated active disease. Patients with moderate and severe disease based on the Truelove-Witts criteria were considered to have active UC. In addition to RDW, serum C-reactive protein levels, erythrocyte sedimentation rates, and platelet counts were measured. Results: Twenty-nine (51.7%) patients with CD and 35 (57.4%) patients with UC had active disease. The RDW was significantly higher in patients with CD and UC than in controls (p<0.001 and p<0.001, respectively). A subgroup analysis indicated that for a RDW cut-off of 14%, the sensitivity for detecting active CD was 79%, and the specicity was 93% (area under curve [AUC], 0.935; p<0.001). RDW was the most sensitive and specifi c marker for active CD. However, it was not valid for UC, as the ESR at a cutoff of 15.5 mm/hr showed a sensitivity of 83% and a specicity of 76% (AUC, 0.817; p<0.001), whereas the RDW at a cutoff of 14% showed 17% sensitivity and 84% specicity for detecting active UC. Conclusions: RDW was elevated in IBD in comparison with healthy controls and increased markedly in active disease. RDW may be a sensitive and specifi c marker for determining active CD, whereas ESR is an important marker of active UC. (Gut Liver 2011;5:460-467)

        Removal of aqueous rare earth elements (REEs) using nano-iron based materials

        The uptake of REEs was investigated using nano-zero valent iron (nZVI) and alumina-supported nZVI (AlnZVI). The results indicated fast uptake, with saturation approached within 10 min of contact between solutions and solids. Upon using nZVI, the uptake of REEs seemed to be quantitative over the studied range of concentration; 1.0–100.0 mg Lˉ1. When Al-nZVI was used, complete removal was achieved within the range of 1.0–10.0 mg Lˉ1. The solids demonstrated stable performance beyond pH of 2.0, up to neutral pH conditions. Fractionation of REEs on Al-nZVI was observed. The REE ions seem to be fixed to oxide and oxyhydroxide surface groups.

        This paper studies Young diagrams of symmetric and pseudo-symmetric numerical semigroups and describes new operations on Young diagrams as well as numerical semigroups. These provide new decompositions of symmetric and pseudo-symmetric semigroups into a numerical semigroup and its dual. It is also given exactly for what kind of numerical semigroup S, the semigroup ring 𝕜⟦S⟧ has at least one Gorenstein subring and has at least one Kunz subring.

      • Social Support and Hopelessness in Patients with Breast Cancer

        Background: Patients with breast cancer can experience a feeling of hopelessness very deeply in the adjustment process, and the social support provided during this period can be effective in increasing the level of hope. The present study aimed to identify breast cancer patients' social support and hopelessness level. Materials and Methods: The target population of this analytical study was all breast cancer patients (total of 85) who had treatment in the oncology department of a university hospital located in Adana/Turkey and who met the inclusion criteria. Data were collected through "Personal Information Form", "Beck Hopelessness Scale (BHS)" and "Multidimensional Scale of Perceived Social Support" (MSPSS). Analysis was performed using Shapiro Wilk, One Way ANOVA Welch, Student t-test, Mann Whitney U, and Kruskall Wallis tests. Homogeneity of variance was tested with the Levene, Bonferroni and Games Howell tests. Mean scores and standard deviation values are given as descriptive statistics. Results: Average age of the participants with breast cancer is $48.6{\pm}10.6$. Of all the participants, 84.7% are married, 49.4% graduated from primary school, 81.2% are housewives, and 82.4% had children. The participants' multidimensional perceived social support total scores were found to be high ($57.41{\pm}13.97$) and hopelessness scale scores low ($5.49{\pm}3.80$). There was a reverse, linear relationship between hopelessness scale scores and social support total scores (r=-0.259, p=0.017). A statistically significant relationship was found between hopelessness scores and education level and having children, occupation, income status, and education level of spouses (p<0.05). Conclusions: The present study indicates that hopelessness of the patients with breast cancer decreased with the increase in their social support. Therefore, activating patient social support systems is of importance in increasing their level of hope.

      • Diagnostic and Prognostic Importance of the Neutrophil Lymphocyte Ratio in Breast Cancer

        Background: The aim of this study was to determine diagnostic and prognostic roles of the neutrophil to lymphocyte ratio (NLR) in breast cancer patients. To date, data are limited on associations of primary breast carcinoma (PBC) and benign proliferative breast disease (BPBD) with preoperative NLR values. Materials and Methods: Our study covered of 120 female patients with PBC and 50 with BPBD. Diagnostic values of NLR were estimated using sensitivity, specificity and areas under receiver operating characteristic curves (AUC). Results: NLR values were significantly higher in the PBC patients than in those with BPBD, with an AUC of 0.668 in the PBC case. The optimal cut-off for NLR was 2.96 and this was validated in the testing set, giving a sensitivity and a specificity of 79.7% and 76.2%, respectively, in PBC patients. Conclusions: Preoperative high NLR is a significant diagnostic predictor of distinction of breast cancer from BPBD and elevated NLR is also an important prognostic marker for primary invasive breast cancer.

        Evaluation of maxillary sinusitis from panoramic radiographs and cone-beam computed tomographic images using a convolutional neural network

        Purpose: This study developed a convolutional neural network (CNN) model to diagnose maxillary sinusitis on panoramic radiographs(PRs) and cone-beam computed tomographic (CBCT) images and evaluated its performance. Materials and Methods: A CNN model, which is an artificial intelligence method, was utilized. The model was trained and tested by applying 5-fold cross-validation to a dataset of 148 healthy and 148 inflamed sinus images. The CNN model was implemented using the PyTorch library of the Python programming language. A receiver operating characteristic curve was plotted, and the area under the curve, accuracy, sensitivity, specificity, positive predictive value, and negative predictive values for both imaging techniques were calculated to evaluate the model. Results: The average accuracy, sensitivity, and specificity of the model in diagnosing sinusitis from PRs were 75.7%, 75.7%, and 75.7%, respectively. The accuracy, sensitivity, and specificity of the deep-learning system in diagnosing sinusitis from CBCT images were 99.7%, 100%, and 99.3%, respectively. Conclusion: The diagnostic performance of the CNN for maxillary sinusitis from PRs was moderately high, whereas it was clearly higher with CBCT images. Three-dimensional images are accepted as the "gold standard" for diagnosis; therefore, this was not an unexpected result. Based on these results, deep-learning systems could be used as an effective guide in assisting with diagnoses, especially for less experienced practitioners.

        Investigation of the effects of storage time on the dimensional accuracy of impression materials using cone beam computed tomography

        PURPOSE. The storage conditions of impressions affect the dimensional accuracy of the impression materials. The aim of the study was to assess the effects of storage time on dimensional accuracy of five different impression materials by cone beam computed tomography (CBCT). MATERIALS AND METHODS. Polyether (Impregum), hydrocolloid (Hydrogum and Alginoplast), and silicone (Zetaflow and Honigum) impression materials were used for impressions taken from an acrylic master model. The impressions were poured and subjected to four different storage times: immediate use, and 1, 3, and 5 days of storage. Line 1 (between right and left first molar mesiobuccal cusp tips) and Line 2 (between right and left canine tips) were measured on a CBCT scanned model, and time dependent mean differences were analyzed by two-way univariate and Duncan's test (${\alpha}=.05$). RESULTS. For Line 1, the total mean difference of Impregum and Hydrogum were statistically different from Alginoplast (P<.05), while Zetaflow and Honigum had smaller discrepancies. Alginoplast resulted in more difference than the other impressions (P<.05). For Line 2, the total mean difference of Impregum was statistically different from the other impressions. Significant differences were observed in Line 1 and Line 2 for the different storage periods (P<.05). CONCLUSION. The dimensional accuracy of impression material is clinically acceptable if the impression material is stored in suitable conditions.

        The effect of various polishing systems on surface roughness and phase transformation of monolithic zirconia

        PURPOSE. The purpose of this study was to evaluate and compare three polishing systems on the surface roughness and phase transformation of monolithic zirconia. MATERIALS AND METHODS. 100 disk shaped specimens (10 mm diameter, 3 mm thickness) were fabricated from monolithic zirconia blocks. 20 specimens were left as a control group and remaining specimens were grinded by diamond bur to simulate the occlusal adjustments. Grinded specimens were randomly divided into 4 groups: group G (no polishing), group M (Meisinger, zirconia polishing kit), group E (EVE Diacera, zirconia polishing kit), and group P (EVE Diapol, porcelain polishing kit). Surface roughness was measured with profilometer and surface topography was observed with SEM. XRD analysis was performed to investigate the phase transformation. Statistical analysis was performed with one-way ANOVA and Tukey’s post hoc tests at a significance level of P=.05. RESULTS. All polishing groups showed a smoother surface than group G. Among 3 polishing systems, group M and group E exhibited a smoother surface than the group P. However, no significant differences were observed between group M and group E (P˃.05). Grinding and polishing did not cause phase transformations in zirconia specimens. CONCLUSION. Zirconia polishing systems created a smoother surface on zirconia than the porcelain polishing system. Phase transformation did not occur during the polishing procedure.

        Effi cacy of the Combination of Tetracycline, Amoxicillin, and Lansoprazole in the Eradication of Helicobacter pylori in Treatment-Naive Patients and in Patients Who Are Not Responsive to Clarithromycin-Based Regimens: A Pilot Study

        Background/Aims: The aim of this study was to evaluate the eradication rate of a triple therapy regimen that included a proton pump inhibitor, amoxicillin, and tetracycline instead of clarithromycin in treatment-naive patients and in patients who did not respond to standard triple therapy. Methods: This study included 110 patients infected with Helicobacter pylori. Patients in groups A and B were treatment-naive, and those in group C were not responsive to previous standard triple therapy. Patients in group A (n=40) received lansoprazole 30 mg b.i.d., amoxicillin 1,000 mg b.i.d., and clarithromycin 500 mg b.i.d. for 14 days. Patients in groups B (n=40) and C (n=30) received lansoprazole 30 mg b.i.d., amoxicillin 1,000 mg b.i.d., and tetracycline 500 mg q.i.d. for 14 days. Results: In group A, eradication was achieved in 18 (45%) of the 40 patients included in the intention-to-treat (ITT) analysis and in 18 (47.4%) of the 38 patients included in the per-protocol (PP) analysis. In group B, eradication was achieved in 15 (37.5%) of the 40 patients included in the ITT analysis and in 15 (39.3%) of the 38 patients included in the PP analysis. In group C, eradication was achieved in 14 (46.6%) of the 30 patients included in the ITT analysis and in 14 (43.8%) of the 29 patients included in the PP analysis. There was no statistically signifi cant difference among the 3 groups with regard to eradication rates (p>0.05). Conclusions: Despite the low rate of resistance to tetracycline, the combination of lansoprazole, amoxicillin, and tetracycline instead of clarithromycin is not a good option for the eradication of H. pylori.
