        Trans-sphenoidal approach에 의한 pituitary microadenoma 수술 후의 임신 1례

        We treated a patient with pituitary microadenoma having hyperprolactinemia and irregular menstruation infertility by transsphenoidal microsurgery. Her menstrual cycle become irregular after marriage but previous menstrual cycle was regular. When she visited our hospital due to infertility, initial serum prolactine level was slightly elevated 35.68ng/ml, During bromecriptine therapy, abrupt increase of serum prolactine level up to 126.52 ng/ml and headache and facial flush and galactorrhea developed. Pituitary microadenoma was confirmed by CT and MRI. The patient underwent transsphenoidal microsurgery. Patient was pregnant after surgery, and it was confirmed by USG and she is following up, OPD now.

        The advent of laminate composite materials opened the way for efficient shell structures. Cylindrical shells are already widely used in indstrial applications. And their buckling behaviour is a vital safety consideration. The objective of this paper is to analyse buckling behavior of laminated composite cylindrical shells subjected to combination of axial compression and torsion by finite element analysis. The influence of load type, fiber orientation angle, and stacking sequence on the buckling behaviour is discussed.

        This paper describes the formulation for the analysis of the flexural free vibration of rectangular plate structure with elastic supports and stiffeners by the transfer influence coefficient method. This method was developed on the base of the concept of the successive transmission of dynamic influence coefficients. Rectangular plate are simply supported at two opposite sides, and it has the stiffeners and distributing springs over node line normal to two simply supported edge. The validity of the present method is demonstrated through simple numerical example, and is also compared with results of the transfer matrix method on a personal computer. And all boundary conditions and the intermediate stiff can be treated only by adequately controlling the values of spring constants.

        멀티캐스트 ATM망에서 대체가상경로의 설정을 위한 유전 알고리듬

        Multicasting is the simultaneous transmission of data to multiple destinations. In multicast ATM networks, the effect of failures on transmission links or nodes can be catastrophic, so that the issue of survivability is of great importance. However, little attention has been paid to the problem of multicast restoration. This paper presents an efficient heuristic technique for routing backup virtual paths in multicast networks with link failure. Genetic algorithm is employed here as a heuristic. In the application of genetic algorithm to the problem, a new genetic encoding and decoding method, and genetic operators are proposed in this paper. The other several heuristics are also presented in order to assess the performance of the proposed algorithm. Experimental results demonstrate that our algorithm is a promising approach to solving the problem.

        수요함수 접근법을 이용한 생활용수 공급편익 산정

        기존에는 댐사업의 생활용수 공급편익 산정시 실무적으로 대체댐비용법을 적용하였다. 그러나 해당 댐의 대체시설로서 2순위의 댐을 적용하는 것은 논리적으로 적절하지 않다. 본 연구의 목적은 생활용수 공급사업의 효과에 대한 소비자의 효용을 지불의사액으로 측정하고, 수요함수를 도출하여 편익산정 방법을 제시하는 것이다. 이를 위해 수도권지역에서 1,000기구를 대상으로 생활용수의 사용량 변화에 대한 한계지불의사액을 측정하였다. 그리고 한계지불의사액을을 이용하여 가구별 수요함수를 추정하고, 최종적으로 용수공급 사업의 경제성분석에 적용할 수 있는 월평균 편익산정식을 도출하였다. 제시된 방법론은 적용방법 설명과 토론을 위해 예제로서 송리원댐(현 영주댐)에 적용하였다. 예제 적용 결과, 내용연수인 50년간 총 편익은 약 900억원으로 산정되었다. 본 연구에서 제시된 방법론은 향후 관련 연구 뿐 아니라, 댐 건설사업을 비롯한 생활용수 공급사업에서 경제성분석시 실무적 활용이 가능할 것으로 기대된다. In the past, the domestic water supply benefit of dam has been estimated by replacement dam cost approach. But it is logically inappropriate that we use the second priority dam as a replaced facility. Therefore, this study aims to suggest the estimation method of the domestic water supply benefit by using demand function, which is deduced from Willingness-To-Pay (WTP) of consumers. For this purpose, a survey concerning the marginal WTP is carried out according to the change of water use amount used, targeted 1,000 households in metropolitan area. And by using the marginal WPT, we estimated the demand function of a family. Finally, the monthly benefit equation is derived. The approach is demonstrated and discussed for an example, the Song-Li-Won dam project which is now renamed Young-Ju dam. From the example study, the total benefit for the durable years (50 years), was about 90 billion won. The method proposed herein is expected to be practical and useful in the economic analysis of the domestic water supply project including dam construction, as well as in further studies.

        상아질 결합제가 컴포머의 불소유리에 미치는 영향에 관한 연구

        본 연구의 목적은 와동 충전시 충전재와 지질 사이에 개재되는 상아질 결합제가, 수복물에서 유리되는 불소가 와동 벽으로 침투하는 과정에 어떠한 영향을 주는지를 조사함이었고, 부가적으로 레진 강화형-글라스 아이오노머 시멘트의 접착에도 상아질 결합제를 도포하는 것이 체질과의 결합력을 강화시킬 수 있는지에 대해 평가하고자 하였다. Fuji II LCR와 Dyract APR를 선정하여 상아질 결합제의 도포 여부에 따른 불소 유리량 측정과 전단 결합 강도도를 비교분석하였으며, 치질 내로의 불소 침투 양상은 교환 시기에 있는 재2유구지에 Fuji II LCR와 Dyract APR를 충전하고 3주내에 발거하여 EPMA필 분석하였다. 상아질 결합제는 불소 유리량을 현저하게 감소시키는 것으로 나타났으며(p<0.05) Fuji II LCR의 경우 상아질 결합제의 도포가 결합강도를 증가시키지 못하였다. EPMA분석결과 상아질 결합제는 충전재로부터 유리되어 나오는 불소가 치질 내로 확산되는 것을 방해하는 것으로 확인되었다. It is well known that glass-ionomer cement and compomer can release fluoride which can inhibit the progresion of dental caries. The purposes of thins study were to evaluate whether the fluoride from fluoride-containing ailing materials can penetrate the dentin bonding adhesive and the dentin bonding adhesive can increase the bond strength of resin-modified glass lonomer. The amount of fluoride release from resin-modified glass lonomer and compomer was measured during 28 days of period find statistically analyzed by t-test. The bond strength of each material with/without dentin bonding adhesive was measured and also statistically analyzed by t-test. The distribution of fluoride from each material into the tooth was explored by electron probe microanalysis system (EPMA). The expenmental teeth used were second primary molars about to exfoliate. The amount of fluoride from each material was diminished by dentin bonding adhesive during 28 days(p<0.05) and the bond strength was not increased by dentin adhesive in resin-modified glass ionomer. The bond strength of group VI was not detectable. The distribution of fluoride from earth material into teeth was according to dentino-enamel junction and dispersed into pulp from pulpal floor. The widest distribution was shown in resin-modified glass ionomer cement filled without the application of dentin bonding adhesive.

      • 제2형 당뇨병 환자에서 체내 총항산화능 측정의 의의

        Background: The diabetic patients are at significantly increased risk of developing vascular disease. It's etiology may involve oxidative damage by free radiacals and protection againse such damage can be offered by antioxidants. We investigated that oxidative stress as assessed by measurement of total antioxidant status may play a role in development of diabetes mellitus. Method: We measured total antioxiant status using merchandised kit, glycated hemeglobin(HbA1c) in 46 type 2 diabetes mellitus patients and 50 healthy matched control subjects. Result: The total antioxidant status(TAS) was 2.10(±0.04) mmol/L in uncontrolled type 2 DM patients, 2.60(±0.03) in controlled type 2 DM patients and 2.70(±0.16) in healthy control subjects. TAS was significantly lower(P<0.05) in uncontrolled type 2 DM patients, but no significant association between in controlled type 2 DM patients and healthy control subjects. The TAS was 2.00(±0.17) mmol/L in complicated type 2 DM patients and 2.10(±0.29) uncomplicated type 2 DM patients. There was no significant associations between complicated type 2 DM patients and uncomplicated type 2 DM patients. Conclusion: Poor glycemic control is associated with reduced TAS in type 2 DM patients. TAS was thought indirect index that predict glycemic control of type 2 DM patients.
