Control and synchronization of Lorenz systems via robust backstepping technique

To control and synchronize the Lorenz systems, a robust backstepping technique using time delay estimation is proposed. Since backstepping technique is a systematic controller design procedure with a recursive method and time delay estimation enables an effective and efficient cancellation of unknown parts including dynamics of the Lorenz system and matched, mismatched disturbances, the proposed control scheme does not require a specific system model and is robust to all types of uncertainties. Numerical simulation results show fast, accurate and robust performance of the proposed backstepping technique under matched, mismatched disturbances and parameter’s variations.

각종 재난 및 다수사상자 사고 발생 시 현장의료대응을 위하여 2016년부터 각 보건소에서는 「재난응 급의료비상대응매뉴얼」에 근거하여 보건소 신속대응반을 조직·운영하고 있다. 본 연구는 국가재난 대응체계 내 보건소 신속대응반의 운영 및 활동근거를 파악하고, 실제 활동사례 및 재난의료실무자 면담을 통해 이들의 역할을 분석하고 고찰하고자 시도되었다. 보건소 신속대응반은 국가재난안전관 리체계 내 긴급구조통제단의 응급의료반으로서, 2018년 8월 기준 전국 249개(468팀, 총 2,557명)이 다. 의료인 및 행정지원 인력으로 구성되며, 재난 현장에서의 역할은 초기 환자 분류, 의료자원조달 및 분배, 환자 이송, 현장응급의료지원, 언론 대응 및 의사소통 등이다. 보건소 신속대응반이 보다 체계적으로 국가안전관리체계 내에서 활동하기 위해서는 재난에 있어 공공보건의 역할에 대한 재정 립과 보건소 신속대응반 활동에 대한 법·제도적 뒷받침, 역량 강화를 위한 개인적, 조직적 차원에서 의 노력이 요구되며 이들을 대상으로 한 실증적 연구가 추후에 수행되어야 함을 제언한다. In South Korea, Public Health Disaster Response Team (PHDRT) has been organized by every Community Health Center since 2016. When a disaster or accident with heavy casualties is expected to occur, PHDRT is dispatched. This study shed light on the role of PHDRT within the Korean National Disaster Response System by analyzing the related law and regulations, their activities, and interviews with disaster medical professionals. PHDRT is the part of the national disaster management system in Korea, organized under the Framework Act on the Management of Disasters and Safety. As of 2018, there was a total of 468 teams and 2,557 members, mostly consisting of physicians, nurses and administrators. The roles of PHDRT in disaster medical response include early triage, medical resource planning, patient transportation, support of on-site emergency care, and media relations and communications. To facilitate the activities of PHDRT, their roles need to be well defined and supported by legal and institutional frameworks, along with individual and organizational efforts to strengthen capacity.
[P265] A case of cutaneous protothecosis masquerading as herpetic lesion

Protothecosis is a rare infection caused byachlorophyllic algae of the genus Prototheca. They aremainly found in the environment, in the soil, freshand salted water, mud of trees, sewage, animal wasteand in some types of food.There are 3 clinical forms: cutaneous,olecranon bursitis and systemic. The cutaneous formis the most common, and most infections are causedby P. wickerhamii.An 82-year-old female patient presented with prickling multiple grouped vesicles and crustson face, neck, chest and back for 1month. There was not a history of contact with soil,contaminated water and any injury of animal bite. Based on prickling pain and multiplegrouped vesicles and crusts, we initially diagnosed as herpetic diseases such as herpes zoster, eczema herpeticum. Histopathological examination revealed an intraepithelial abscess formation, fungal spherules with morula-like appearance and chronic granulomatous inflammation in upper dermis. For species identification, a suspension of theculture was submitted to the carbohydrate assimilationtest, indicating P.wickerhamii as the causative agent.After the diagnosis of cutaneous protothecosis,treatment was started with itraconazole at a dose of 200mg daily,resulting inimproving the lesion. Herein, we report a case of cutaneous protothecosis masquerading as herpetic lesion.
A case of subcutaneous and intranasal phaeohyphomycosis caused by Microsphaeropsis arundinis

Microsphaeropsis arundinis is a dematiaceous fungus and capable of causing soft tissue infections known as phaeohyphomycosis, mostly in immunocompromised patients. An 76-year-old man presented with a 5-month history of diffuse erythematous plaques and swelling on both forearms and dorsal hands, and complained of rhinalgia. He was being treated with hypertension, angina pectoris, and diabetes.On the histopathologic examinations of skin, dense infiltrates of inflammatory cells such as neutrophils, monocytes, histiocytes, and giant cells were seen. On methenamine silver stain and Periodic-acid-Schiff stain, scattered fungal spores and hyphae were observed. In the ENT examinations, necrotic changes in the nasal septum were observed and a biopsy was performed at that site. And the histopathologic findings of nasal septum lesions were similar to the results of skin biopsy.The cultures of the biopsy specimen and nasal septum tissue grew a dematiaceous fungus showing irregularly shaped septate hyphae. So, the molecular identification of the organism was performed by gene sequencing analysis. Based on these findings, the authors diagnosed as subcutaneous and intranasal haeohyphomycosis caused by M. arundinis. He was treated with oral itraconazole for over 5 months, and there has been no recurrence to date. To our knowledge, this is the rare case of subcutaneous and intranasal phaeohyphomycosis caused by M. arundinis.
[P323] A case of vulvar intraepithelial neoplasia in a patient with kidney transplantation

Vulvar intraepithelial neoplasia(VIN) is a high-grade intraepithelialsquamous lesion and precursor of invasive squamouscell carcinoma. Theclassification distinguished twotypes of VIN: usual type (human papillomavirus(HPV)-related)and differentiated type (not HPV-related). There is a close relationship with other genital and extragenital premalignant and malignant condition, immunosuppression, and lichen sclerosus.A 44-year-old woman presented with a 1-month history of painful multiple various sized eyrthematous hyperkeratotic maculopapules and plaques with oozing on vulva.On past history, there was a kidney transplantation10 years ago and she had taken antihypertensive and immunosuppressive medications. Histopathological examination revealedbasal layer involvement, enlarged nuclei, hyperchromatic, pleomorphic, andincreased numbers of mitotic figures. The pap smear and ultrasonography were within normal limit. HPV typing was positive with 6 and 11. Topical 5% imiquimod cream was initiated. After 1 week, she refused to use the treatment due to burning sensation.Later on, treatment with cryotherapy was appliedevery 2 week, resulting in the resolution of most lesions. Herein, we report a case of vulvar intraepithelial neoplasia in a patient with kidney transplantation successfully treated with cryotherapy.
