환자 만족도 조사를 통한 QI활동의 효과 평가 : 일개 병원을 중심으로

Background : Evaluation of patient's satisfaction is one of the most important aspects of quality improvement. If the patient highly satisfies with the medical service provided in the hospital, he/she will be likely to visit the same hospital again. Patient's satisfaction of a particular hospital is directly correlated with hospital profits as well as reputation in the community. For this reason, various kinds of survey measuring satisfaction level have been performed and many kind of QI activities for enhancing the profits as well as reputation in the community. For this reason, various kinds of survey measuring satisfaction level have been performed and many kind of QI activities for enhancing the patient's satisfaction. This study is to find the effect of hospital QI activities on the patient's satisfaction level. Methods : After questionnares were developed, survey of measuring satisfaction level was performed in August. 1998. On the basis of survey results. Ql activities were carried out to attain the target point of 4.0 and subsequent survey was done in November. 1998. Results : With three main principles of "problem solving approach with kindness". "helping patient to participate in medical procedure with sufficient information", and "putting employees into practice of attitude with human respect", the average level of satisfaction was enhanced from 3.45 to 3.55 level. Also kindness level of employees was increased from 3.71 to 3.82. Level of dissatisfaction about insufficient explanation and unkind attitude was dropped from 69% to 48% and from 82% to 46% respectively. Conclusion : With the result of this study overall satisfaction level was enhanced. In order to keep these advantages a operation of Quality Improvement Task Force Team in each subject will be required.

본 연구의 목적은 급변하는 사이버 환경 속에 단순 범죄 행위를 넘어 국가적인 위협으로 부 각되고 있는 사이버테러가 날로 발전적 양상을 보이고 있는 점을 바탕으로 사이버테러 대응체 계의 현황과 문제점을 국내에서 발생했던 주요 사이버테러 사례들의 분석을 통해 연구해 보고, 이를 통해 향후 발생할 수 있는 국가적 위기에 효율적으로 대응할 수 있는 사이버테러 대응체 계 구축방안을 모색해 보고자 하는데 있다. 현재 국내 사이버테러 대응체계는 사이버 위기 발생 시 국가차원 대응정책을 강구하는 국가 사이버안전 전략회의를 중심으로 국가·공공분야의 국가정보원 ‘국가사이버안전센터’, 국방 분야의 국방부 ‘국방사이버지휘통제센터’, 민간분야의 방송통신위원회 ‘인터넷침해대응센 터’ 등이 유기적으로 결합하는 구조를 가지고 있다. 하지만 ‘1·25 인터넷 대란’, ‘7·7 DDoS 대란’, ‘농협 전산망 마비사태’ 등을 겪으면서 각 분야의 기관을 조정·통제할 수 있 는 총괄전담기관과 관련 법체계의 부재로 인해 부처간 협조와 정보 공유가 미흡하여 사이버테 러에 적극적인 대응을 하지 못하는 등의 문제점이 제기되었다. 이에 본 논문은 사이버 위기 발생 시 적극적인 대응을 하기위한 법과 제도적인 뒷받침을 마 련하고자 다음과 같은 방안을 제시하였다. 책임과 권한이 여러 부처에 분산됨으로 인해 발생하는 정책상의 혼란을 방지하고 모든 역량 을 한 곳에 집중하기위해 정부 각 부처 및 기관에 복잡하게 분산되어 있는 사이버테러 대응과 관리 등 각종 기능을 한 곳에 모아 국가 차원의 사이버위기 극복을 위한 총괄전담기관을 설치 하고 네트워크를 형성하여 이를 통해 위기 발생 시 부처간 실시간의 정보 공유와 대처로 신속 한 대응체계를 구축하도록 한다. 총괄전담기관의 설치와 함께 공공부문뿐만 아니라 민간의 전 분야까지 적용할 수 있는 통합 법제의 제정으로 평시 사이버테러 예방을 위한 각급기관의 역할 정립과 위기발생시 범국가 차 원의 대책기구 구성 등 효율적이고 체계적인 위기관리체계 구축할 수 있도록 하고 기존 법제의 재정비를 통해 이를 뒷받침할 수 있는 방안을 마련해야 한다. This study aims to research on the status of cyber terror response system and problems having been occurred in Korea through analyzing its cases because cyber terrors having been highlighted as national threats exceeding simple crime behaviors in midst of rapidly changing cyber environments have displayed developing features day by day. Also, the study is to search for establishments on cyber terror response systems that are able to effectively respond to national crises which could be occurred afterwards. Current cyber terror response systems in Korea have structures of organically combining several organizations together such as ‘National Cyber Safety Center’ in National Intelligence Service, ‘National Defense Cyber Command & Control Center’ in the Ministry of National Defense from public and national fields, and ‘Internet Infringement Response Center’ in Korea Communication Commission from civilian fields. However, the necessity of the general responsible organization that can coordinate and control organizations at each field together with relevant legal systems were risen to the surface while experiencing ‘1·25 Internet great disturbances,’ ‘7·7 DDoS upheaval’, and ‘NH computer paralysis’ etc. That is, problems of not aggressively responding to cyber terrors were suggested. Therefore, this study proposed the following methods so as to prepare for legal and institutional assistances for making aggressive responses in case of cyber crisis’s occurrences. In order to prevent from policy’s troubles happened by responsibility and authority’s departments to a lot of relevant governmental organizations, and to concentrate its abilities, the general responsible organization for overcoming cyber terrors in national dimensions is necessary so as to collect all kinds of functions such as cyber terror responses and managements having been dispersed to each minister and offices complicatedly into one place. By doing so, real-time information shares between ministries and offices all together with establishing prompt response systems shall be made accordingly by forming networks each other. Also, this study suggests that effective and systematic management systems on crisis including a measure organization’s establishment from pan-national dimensions have to be made in case of crises together with setting up each level organization’s roles for preventing from cyber terrors in ordinary times by enacting consolidated legislations that can be applied to all civilian fields, not to mention of public sectors together too. In addition, methods for backing these systems shall be made through rearranging existing legislative systems.
유아의 기질과 부모의 성격특성이 유아와 부모간의 상호작용에 미치는 영향 : Child Interaction

The purpose of this study was to investigate the effect of children's temperament and parents' personality on affective and controlling parent-child interaction. The results of this study were as follows: 1. There were no significant differences in children's temperament and parent-child interaction according to children' sex. It implies that the child's sex is not important factor to perceive children's temperament and parent-child interaction. 2. The level of children's activity and response sensitivity, and father's compulsiveness were significant factors for predicting affective father-child relationship. Also, controlling father-child relationship was significantly influenced by father's personality of compulsiveness. 3. Mother's vulnerability, mother's introversion, and children's response sensitivity were significant factors for predicting affective mother-child relationship. Also, the controlling mother-child relationship was significantly effected by mother's vulnerability and compulsiveness, and children's adaptability. This research suggests that father-child interaction was more strongly associated with child's temperament than father's personality whereas mother-child interaction was predicted by mother's personality more than child's personality. In addition, this study was discussed in terms of children's temperament and parents' personality as antecedent factors of parent-child interaction.

The purpose of the current study was to compare the pregnancy outcomes in relation to weight gain pattern and to obtain information about effective prenatal care. The subjects for the study were 241 women who delivered the single infant. The data were collected by chart review. In the instrument, weight related factor(age, gestational period, prenatal weight and height, hemoglobin, hematocrit, fundal height, abdominal circumference), pregnancy outcomes(length of first and second stage labor, birth weight, apgar score), obstetric characteristics were included. The data was analyzed by descriptive statistics, ANOVA, chi-square test, multiple repression. The result of study were as follows, There was statistically significant difference among the three weight groups on the fundal height and abdominal circumference and birth weight, prenatal complication. By means of multiple regression analysis, fundal height and gestational period, abdominal circumference explained 33% of the variance of the predicted birth weight. To facilitate optimal outcome for the client experiencing over weight gain during pregnancy, health care providers should be knowledgeable about the impact that excessive weight gain has on birth weight and labor. Also, they are in a position to advise the client regarding nutrition, weight gain, exercise during prenatal care.

조지훈은 이 시대의 마지막 선비라 일컬어지는 시인이다. 그의 작품 세계는 전통 탐구에서 시작하여 자아에 대한 탐구, 역사의식에 이르기까지 다양하다. 이같이 다양한 그의 작품에 일관하는 시정신을 선비의식의 측면에서 고찰해 보는 작업 중 한 부분으로 그의 작품 중 초기의 작품 세계를 전통 탐구, 한시적 절제미, 동양화의 여백미의 측면에서 조명해 본다. 지훈은 지극히 어려운 식민지의 삶을 살아 내며 전통과 민족에 대한 믿음에서 출발하여 우리가 우리다울 수 있음은 전통에의 회귀에서 찾아야 한다고 생각하였는데 이는 그의 역사의식의 천착에서 얻어진 믿음이라 할 수 있다. 그는 시대적 암울과 극한 상황에서 매우 힘겹게 살았음에도 불구하고 그의 초기 시세계에서 전통과 절제와 여백의 아름다움을 보여 주었다. 한시의 전통을 계승하여 한시 창작의 역량을 현대시로 옮겼으며, 한시와 현대시를 병행하며 상보적인 발전을 꾀하고 전통적인 동양의 정서를 현대시의 정서로 승화 정착시키는 작업, 그리하여 한국의 민족 정서의 현대시적 서정을 정립시키는 성과를 남겼다.

The children's gravity in a home is really great, so the education of children is an important problem. When they are in the infant days, the children are entirely influenced by their parents, but as soon as they are at paberty, they want to stand on their own legs with desire to find out the self. As a man who has character, they learn the responsibility to their activity through the communication. Also, they become defiant because they have mental troubles and can't adapt to the ideal and reality. We all hope to stabilize our home and grow the teenagers splendid, so the communication between parents and children is very important. The purpose of this study is to provide the basic imformation for the happy family life with the smooth communication between mother and the adolescent children work by analyzing the elements regarding the communication between mother and the adolescent children. The detailed contents of this study are as follows: 1) How general level of the communication satisfaction between mothers and their adolescent children is ? 2) How general level of the communication satisfaction in every low-ranking division is ? And how influence by socio-demographic factors(sex distinction, numbers of brothers and sisters, mother's education, profession and age, the level of home income). 3) How background variables influence independently mothers ans their adolescent children ? The research was inspected by 432 students of Girls' and Boys' High School in Seoul. As the means of the measurement, the questionaire was used and α=.61 as the result of Cronbach's Confidence Test. The questionnaires are composed for 30 days from the 25th of June. The data were analyzed by frequency, percentage, arithmetic mean, t-test, F-test, Duncan's Multiple Regression Test and Stepwise Multiple Regression Analysis. The results are summarized as follows: 1) The very satisfactory group is 26.2%, ordinary satisfactory group is 35.8% and undersatisfactory group is 38.0% in the communication satisfaction test between mothers ans their adolescent children. 2) The mean of the satisfaction is 3.41. Especially, the low-ranking divisions of the scholarly attainment, future aspiration, school life, making friends, social problems are more higher than total mean but the low-ranking divisions of the sex education, sex role, physical growth, valuation, family life are more lower than total mean. The children's communication satisfaction according to independent variables is significant. It showed high satisfaction in a home which has small number of children(p<.001) ans the high level of mother's education(p<.001). But mother's profession and age is partially significant. 3) The background variables influence the communication satisfaction between mothers and their adolescent children: mother's education is 35.7%, sex distinction is 8.5%, home income is 5.6%, number of brothers and sisters is 1.26%, mother's age is 1.28%. And they show totally 52.4%. As the above results show, in our country mother's concern about their children's scholary attainment and school life is great but they don't understand children's mental and emotional anxiety. So it is necessary to educate children through the communication in order to inspire them with the right valuation and firm subjectivity, because they are very important for the future of this country.

The purpose of this syudy was to investigate the perception of the parent's value education for children. The subjects of this survey study were 89 parents with 6-13 aged children in Daejeon. The results are as follows : 1. The value of being as honesty and responsibility were perceived the most important value. While the value of giving as unselfishness and friendliness, cooperation were perceived the least important value. 2. Patience, responsibility, self-reliance were perceived very difficult values in the teaching values for children.

Stroke is a crisis for the individual and family. The sudden change in health status and functional ability requires long-term care. Disability of stroke patients require early systemic rehabilitation because the efficiency of rehabilitation exercise in the acute stage after and attack of a stroke or at home after their discharge has a great influence on returns to their social life. Rehabilitation programs must include the acute ad passive exercises for the prevention of complications of muscles, skeletal or respiratory systems which can be caused by immobilization. These exercises have to be intensified to be taken continuously during the hospitalization or after the discharge. The study was designed to test the effects of rehabilitation exercise on functional outcome after stroke. The quasi-experimental study employed control group design with admission, 4weeks and 8weeks as measured on the Functional Independence Measure. Participants were assigned to the control or the experimental group, with each group having 23 participants. The experimental group participated in individual intervention that featured the active and passive exercise, whereas the control group received the routine schedule from the hospital. Data were analyzed using Fisher's exact test, ANCOVA, Repeated measure ANOVA. The result of this study are 1) No significant difference were found between the two groups on characteristics related disease and demografic data. 2) There was a statistically significant differences in the total FIM during the rehabilitation process over time(F=115.35, p<0.001) and an interaction between groups and time(F=6.17, p=0.003). However, there were no significant differences between the groups. There was no significant differences between groups in the motor FIM(F=0.04, p=0.837), but there was changes over time and an interaction between group and time(F=3.98, p=0.025 ; p=101.83, p=0.0001). In cognitive FIM, only changes over time in functional outcome were found significant difference(F=6.03, p<0.004). 3) The effect of rehabilitation exercise in functional outcome were as follows: After the 4weeks, there were no significant differences between groups in functional outcomes, but significant differences in total FIM and motor FIM occured between the two groups after the 8weeks(Z=-2.79, p<0.05 ; Z=-2.62, p<0.05). Especially, self-care item was significantly differences between groups after the 4 or 8weeks(Z=-2.71, p<0.01 ; Z=-3.67, p<0.01). This study provides the basis of nursing research for stroke rehabilitation and is useful in the application of home care. Ultimately, more rapid and cost-effective rehabilitation as well as greater functional independence can be expected for stroke patients on discharge home.

The change of design situation according to the changes of recent communication means requires a change of design education and using computer in design education is very natural phenomenon, and most of design works are performed on computer screen which is cyber space. Considering such a change and requirement of the times and the direction of design education, the introduction of computer in art education of elementary school is compulsory and the application of computer should be started from the concept of CAI (Computer assisted instruction). That is, as 'crayon' and 'mud' are the object of art education, it is not to teach 'computer' but to us it as the means in expressing children's self and its technique and it is desirable that C.G. is applied to the various areas as the instrument to help the children's expression rather than its introduction as independent expression area at elementary art education, but this study limits the range of research to computer application to graphic design of design area (decorating and making) which is assumed to be effective in its uses based on the figurative features of C.G.. Therefore this study suggests teaching guide and field application cases of expression areas as the learning method using 'picture board' of C.G. editor of Hangul window which children can access and use easily as an alternative for elementary graphic design education using computer media corresponding to the changes of the times and then it expects that elementary school teachers use the cases presented in this research and it plays the role of guide for teachers assisting textbook research. To summarize the results of this research, the use of computer induces children to participate and concentrate on class by enhancing their interest in class in the process of motivation and the class applying design process achieves the effective art class escaping from function centered teaching method of computer and children who don't like art class use the various functions easily and have joy and confidence by making a work in cooperative relations with classmates. Most of children learn the basic functions and elements, and basic knowledge of design through design process and makes the works of high perfection by using colors and shapes suitable to topic. In conclusion, children can approach the use of C.G. in graphic design education of elementary design area and in its learning effects, it has reverse function compared to conventional media, but since it can interact with learners according to program contents and offer learning environment according to learner's development level and cognitive ability, it confirms the potential to achieve the learning effects by inducing children's interest and concern in learning.
