전이성 석회화(metastatic calcification)는 전신적인 무기질 불균형이 있는 환자에게서 연부조직에 석회침착이 발생하는 질환으로 이형성 석회화(dystrophic calcification)나 이소성 골화(heterotopic ossification)와는 구별되는 질환이다. 저자들은 만성 신부전 환자에게서 발생한 수지의 전이성 석회화 1예를 경험하였고 경과 관찰 중 자연 소실되는 경과를 보여 이를 문헌고찰과 함께 보고하고자 한다. Metastatic calcification is defined as ectopic calcification of soft tissues in patients with a systemic mineral imbalance, and dystrophic calcification or heterotopic ossification is distinct from that disease. The authors encountered a metastatic calcification of the finger in a patient with chronic renal failure, the lesion disappeared spontaneously during the observation period. We report on a case of metastatic calcification of the finger with a review of the literature.
고관절 고위 탈구 환자에서 전자하 횡형 단축 절골술을 이용한 무시멘트성 인공 고관절 전치환술

목적: 고관절 고위 탈구 환자에서 전자하 횡형 단축 절골술을 이용하여 무시멘트성 인공 고관절 전치환술을 시행하고 그 임상적 및 방사선학적 결과를 알아보고자 하였다. 대상 및 방법: 2001년부터 2012년까지 고관절 고위 탈구로 대퇴 전자하부 횡형 단축 절골술과 함께 금속판 또는 케이블 고정 및 절골부위의 골이식을 시행하여 무시멘트성 인공 고관절 전치환술을 시행하였던 환자 18명(18예)을 대상으로 하였다. 평균 추시 기간은 5.2년(2-8.3년)이었다. 임상적으로 Harris 고관절 점수, 파행, 하지 부동의 변화를 측정하였고, 방사선학적으로 골용해, 삽입물의 해리 및 절골 부위의 유합 유무를 평가하였다. 결과: 최종 추시 시 전 예에서 Harris 고관절 점수, 파행 및 하지 부동의 호전이 관찰되었다. 방사선학적으로 감염이 발생했던 2예를 제외한 전 예에서 삽입물의 해리, 골용해 등을 보인 경우는 없었으며, 전 예에서 절골부의 유합 소견을 보였다. 합병증으로 감염 2예, 탈구 1예가 있었으며 감염으로 재수술을 시행한 2예 중 1예에서 좌골 신경 마비가 발생하였다. 결론: 고관절 고위 탈구 환자에서 전자하부 횡형 단축 절골술을 이용한 무시멘트성 인공 고관절 전치환술은 비교적 만족할 만한 결과를 보였다. 그러나 감염 등의 합병증 발생이 상대적으로 높게 나타 나이에 대한 각별한 주의가 필요할 것으로 생각된다. Purpose: The purpose of this study was to evaluate the clinical and radiographic results of cement less total hip arthroplasty (THA) with subtrochanteric transverse shortening osteotomy in patients with high hip dislocation. Materials and Methods: Eighteen patients with high hip dislocation who underwent cement less THA combined with a subtrochanteric transverse shortening osteotomy, plate or cable fixation and bone graft on the osteotomy site between 2001 and 2012 were evaluated in this study. The mean duration of follow-up was 5.2 (2- 8.3) years. We evaluated Harris hip score, limping, limb length discrepancy as a clinical parameter and osteolysis, loosening and union of the osteotomy site as a radiographic parameter. Results: Harris hip scores at the final follow-up showed improvement of limping and limb length discrepancy in all cases. And, with the exception of two cases of infection, there were no loosening and osteolysis. All cases showed union of the osteotomy site. There were two cases of infection and one case of dislocation as a complication. Infection occurred in two patients who underwent reoperation and one patient developed sciatic nerve palsy. Conclusion: Cement less THA with subtrochanteric transverse shortening osteotomy showed relatively satisfactory clinical and radiologic results. However, the incidence of complications, such as infection, is relatively high, therefore, careful attention is needed.

Aedes albopicuts, known as asian tiger mosquito or forest mosquito, is characterized by its black and white strips on legs and body. Ae. albopictus is an important vector mosquito for the transmission of many viral pathogens such as dengue fever and chikungunya fever. Unlike other mosquito, Ae. albopictus attacks people mainly during the daytime in the forest. To evaluate the diel activity of Ae. albopictus, three times mosquito collecting were conducted using BG Sentinel trap with dry ice in bamboo forest in Damyang-gun, Jeoullanam-do, in South Korea from August to September, 2009. Captured mosquitoes were counted and released every one hour during 25 hours experiment time. The result showed that Ae. albopictus activity began with sun rising at approximately 09:00, with peak in the early evening between 16:00 to 19:00, and ended with sunset at 21:00. Among the several factors affecting on the mosquito activity, light intensity seems to be a main factor. Especially, if the light intensity is over some threshold, it negatively effects on the activity of Ae. albopictus.
Surveillance of Mosquitoes Associated with Vector Born Disease

There are over 3,000 different species of mosquitoes throughout the world; currently 56 species are reported in Korea. Mosquitoes can act as vectors for many disease-causing viruses and parasites during blood sucking. Various species of mosquitoes are estimated to transmit various types of disease to more than 700 million people annually in Africa, South America, Central America, Mexico, Russia, and much of Asia, with millions of resultant deaths. At least two million people annually die of these diseases, and the morbidity rates are many times higher still. To prevent the spread of diseases, KNIH (Korea National Institute of Health) used three categorized methods; identifying or taxonomic analysis of mosquitoes, detecting virus caring mosquitoes, and detecting malaria from Anopheline mosquitoes. We have proved that taxonomic analysis using DNA barcording method (COI gene) is useful to complement identification of mosquito species. In detecting virus, we have reported Cx. orientalis as a new potential Japanese encephalitis virus (JEV) vector. We also reported that the Anopheline mosquito species composition and Plasmodium vivax infection rates in malaria hot spot in Korea.
Phylogenic Analysis of Endosymbiont Wolbachia with Aedes albopictus (Diptera: Culicidae) in Korea

The intracellular endosymbiont bacterium Wolbachia is currently considered the most abundant in arthropods and has also been isolated from nematodes, amphipods, isopods, mites and spiders. Recently, Wolbachia-based research was focused on the control of disease vector-population, such as several mosquitoes such as genus Aedes and Anopheles which cause dengue fever and malaria, respectively. For the analysis of regional difference between vector mosquito Aedes albopictus and Wolbachia, we selected different regions and collected Ae. albopictus which were distinguished with mountain chain and waterway. Whole genomic DNA were extracted from collected specimens with 9 regions. PCR analysis and sequencing were accomplished in each specimen for Wolbachia detection and identification using WSP gene. As a results, almost mosquitoes were infected with two strain of Wolbachia both wAlbA and wAlbB. However, regional separation of vector mosquitoes, wAlbA strain of Wolbachia were showed more than 98% sequence similarity. In this study, we first reported that Wolbachia infection and type of Wolbachia in Korea and endosymbiont Wolbachia was showed highly sequence homologies.

Multiport DC-DC converters based on a dual-active-bridge (DAB) topology have attracted attention due to their high powerdensity and bidirectional power transfer capability in DC microgrid systems. In addition, connectivity is high for various distributed resources (DRs). However, power coupling among ports magnetically connected by single or multiple transformers induces poor power control and voltage regulation performance. To mitigate these technical issues, an enhanced power decoupling method for a four-port DAB converter is proposed, which does not have supplementary power decoupling algorithms under the grid-connected mode and islanding mode operations of a DC microgrid system. In addition, a modular structure can be applied to individually connect a system with various loads. Finally, a 3-kW prototype four-port DAB converter is implemented to verify the validity and performance of the proposed method.
