        응급환자에서의 쇼크계수의 의의

        For the emergency patients in need of immediate resuscitation, cardiac catherization along with intensive cardiac monitoring is necessary to evaluate the cardiovascular stability. But this method requires machinery, technique and time which limits its use in emergency situations. In 1961, Allgower and Buri first discribed shock index, heart rate divide by systolic blood pressure, as easy to calculate and easy to use in acute settings given the simple measurements of the heart rate and the blood pressure. It has been said in circulatory failure patients shock index is inversely related to left ventricular stroke work. Because shock index is an indicator for resuscitation and also a prognostic indicator it could be applied in triaging an emerency patients. Therefore we studied the significance and the possible triage application of shock index in emergency patients. In the period of 6 months from May 1 to October 31, 1994. Among total 15,782 patients, who visited Yonsei University-affliated Yong Dong Emergency Department, 2425 patients were included in the study. There were 201 admissions to the intensive care unit and 2105 patients to general beds and 199 selected patient from discharged. Retrospective chart review for pulse rate, systolic blood pressure, diastolic pressure and shock index was done. Statistical analysis was performed using t'-test, p value of less than 0.05 was designated significant. Average shock index for ICU patients were 0.96 ±0.50, general ward admittees were 0.67 ±0.18, and discharged were 0.65 ±0.16. There were significant statistical difference in shock indices between ICU and general ward patients, and ICU and discharged patients. Shock index is helpful in identifing patients with potential for admission to the ICU in emergency situations. Therefore we conclude that the shock index should be recognized as a triage tool.

        An unconscious 20 year old male carried into the emergency department after an electric shock injury 20 minutes earlier. Cardiopulmonary resuscitation was undertaken when asystole appeared on the electrocardiograph monitor. A 12-lead electrocardiograph was taken when in that myocardial infarction was found after performing cardiopulmonary resuscitation on an electrical injury patient. We present out case's electrocardiographs with echocardio-graphic findings and radionuclide imaging studies.

        Centrifuge Model Tests and Numerical Simulations of the Landslide Evolution Process

        Centrifuge model tests and numerical simulations were performed to study the landslide evolution process and failure mechanism. A TLJ-500 geotechnical centrifuge was used for the experiments and landslide deformation and stress was monitored using high-precision differential displacement sensors and earth pressure micro-sensors. Discrete element numerical simulations were performed using PFC2D based on the experimental results. The findings show that the landslide evolution process can be divided into three stages: 1) compaction and consolidation; 2) uniform deformation; and 3) accelerated deformation and failure. The numerical simulation results verify the distinct stage characteristics of the landslide evolution process. According to the migration of microscopic soil mass particles within the landslide, stage 3) can be further divided into a deformation development stage and instability and failure stage. The simulation displacement monitoring curves and displacement map show distinct deformation characteristics and displacement indicators from stages 2) to 3) and from the deformation development stage to the instability and failure stage. The experimental and numerical results reveal the landslide failure mechanism: the upper part of the landslide thrusts and slides; the middle part squeezes; the lower part collapses; and shear plane penetration leads to landslide failure.

      • ADPRT Val762Ala and XRCC1 Arg194Trp Polymorphisms and Risk of Gastric Cancer in Sichuan of China

        Objective: Gastric cancer remains a major health problem in China. We hypothesized that XRCC1 Arg194Trp and ADPRT Val762Ala may be associated with risk. Methods: We designed a multicenter 1:1 matched case-control study of 307 pairs of gastric cancers and controls between October 2010 and August 2011. XRCC1 Arg194Trp and ADPRT Val762Ala were sequenced, and demographic data as well as lifestyle factors were collected using a self-designed questionnaire. Results: Individuals carrying XRCC1 Trp/Trp or Arg/Trp variant genotype had a significantly increased risk of gastric cancer (OR, 1.718; 95% CI, 1.190-2.479), while the OR for ADPRT Val762Ala variant genotype (Ala/Ala or Val/Ala) was 1.175 (95% CI, 0.796-1.737). No gene-gene or gene-environment interactions were found. In addition, family history of cancer and drinkers proportion were higher among cases than among controls (P<0.05). Conclusions: XRCC1 194 Arg/Trp or Trp/Trp genotype, family history of cancer, and drinking are suspected risk factors of gastric cancer from our study. Our findings may offer insight into further similar large gene-environment and gene-gene studies in this region.

      • NGO and Precise Poverty Alleviation in China

        This paper analyzes the market logic and operating model of precise poverty alleviation programs carried out by a native Chinese NGO in China’s rural areas based on the case study of the BADICAO activity. Research shows, compared to the government, the NGO demonstrates special advantages at the micro operating level in poverty alleviation programs because of its advanced concepts in sustainable development and its precise targeting to assist poverty-stricken people, as well as its mobilization ability of resources in many ways. While for the special national conditions and the administrative management system of China, the development of NGO is still at an immature stage; independence, legality and the acceptance of citizens are the main factors to restrict the further development of the NGO in acting as the main body in poverty alleviation. Policy support and promotion from the government from a macroscopic aspect are very necessary.

        Performance-oriented State-feedback Controller Parameter Tuning with Guardian Maps

        A novel method of controller parameter tuning is proposed, based on the guardian maps stability theory. The tuning sequence could be divided into three steps: first, make sure the appropriate region in the complex planein order to achieve the desired performance. Then, calculate the guardian function with guardian maps techniques. Finally, let the guardian function equal to zero, and find the parameters tuning space by solving the equation. Besides, if the controller parameters are selected, we can make sure the allowable perturbation range of the plantparameters is determined. It can be shown that our new performance-oriented scheme is convenient and practical,and also it is suitable for solving the controller parameter tuning problems of various plants with most of the classicalcontrol methods.

        Analysis of the Indexes on Early Childhood Education Environment Construction in Chinese Communities - Taking the Integration of Natural Attributes as an Example -

        This research is aimed at improving community environment where Chinese children engage themselves in daily play, communication, learning, fitness and other activities. At present, the natural environment that is truly suitable for children’s activities is lacking in Chinese communities, and there is no clear construction standard. In view of this, this research adopts the literature research method to study the relevant literature in South Korea, China and other countries, and integrates the child-friendly city action plan, the nature education theory for young children and the construction standard of Korean forest kindergarten. Through comparative analysis, this research attempts to unfold the current situation and existing problems of the natural environment for children"s activities in Chinese communities and reveal the analytical indexes suitable for community construction, so as to provide a basis for the construction of the Chinese community environment. The research findings and content summary are as follows: First, in terms of recreation, the environment shall satisfy young children with fun activities, provide rich construction areas, and set up facilities for young children to enjoy adventurous climbing or water. Second, in terms of naturalness, a quiet and diverse ecological environment shall be created, and natural materials as well as natural elements can be used as much as possible, besides, local materials shall be adopted in order to save energy. Third, in terms of education, an artistic and participatory environment shall be created to help children improve their aesthetic ability, and meanwhile help them experience the magic of life and establish a correct outlook on life. Fourth, in terms of sociality, space for children to interact with others shall be provided to help them quickly adapt to the environment and get a sense of belonging. Fifth, in terms of functionality, the characteristics of children"s physical and mental development shall be fully considered and the environment must be added with fun and diversity. Sixth, in terms of safety, maintaining hygienic environment, using environment-friendly materials, regularly inspecting and maintaining facilities, setting up barrier-free passages and facilities to cope with emergencies can add up to give children a sense of physical and psychological safety. It is hoped that the 6 elements, namely, recreation, naturalness, education, sociality, functionality, and safety, together with the 36 analysis indexes, will provide guidance and suggestions for the construction of the natural environment for children in Chinese communities.

        Head-on Collision Between Two Envelope Solitary Waves in a Granular Medium

        We investigated the head-on collision between two envelope solitary waves. Head-on collisions between two envelope solitary waves are first discussed in one-dimension (1D) granular chains. The interesting result is that no phase shift or phase delay detected after the head-on collision between two envelope solitary waves. The maximum amplitude during the head-on collision between two envelope solitary waves is also found to be less than the sum of the amplitudes of the two envelope solitary waves, but is larger than the amplitude of the either of the envelope solitary waves.

        <P><B>Abstract</B></P> <P>As a new magneto-responsive smart material, core–shell structured mesoporous Fe<SUB>3</SUB>O<SUB>4</SUB>@mSiO<SUB>2</SUB> nanoparticles were fabricated by a surfactant-based sol–gel approach using Fe<SUB>3</SUB>O<SUB>4</SUB> nanoparticles, and then adopted as magnetorheological (MR) materials. The Fe<SUB>3</SUB>O<SUB>4</SUB> particles were initially synthesized by a solvo-thermal process. Scanning electron microscopy and transmission electron microscopy were employed to observe the morphology of the as-synthesized mesoporous silica-coated Fe<SUB>3</SUB>O<SUB>4</SUB> particles. Fourier transform infrared spectroscopy and X-ray diffraction results showed that the mesoporous silica shell is successfully coated on the Fe<SUB>3</SUB>O<SUB>4</SUB> nanoparticles. BET adsorption measurement also confirmed their mesopore formation. Rheological properties of the silica-coated Fe<SUB>3</SUB>O<SUB>4</SUB>-based MR fluid dispersed in silicone oil were investigated, and a change in the slope of the yield stress as a function of magnetic field strength from 2.0 to 1.5 was observed, with improved dispersion stability.</P> <P><B>Highlights</B></P> <P> <UL> <LI> Core–shell structured mesoporous Fe<SUB>3</SUB>O<SUB>4</SUB>@mSiO<SUB>2</SUB> nanoparticles were fabricated. </LI> <LI> Both dynamic and elastic yield stresses of MR fluids were well correlated with universal yield stress equation. </LI> <LI> Dispersion stability of MR fluid was improved compared to the sedimentation rate of Fe<SUB>3</SUB>O<SUB>4</SUB>-based MR fluid. </LI> </UL> </P> <P><B>Graphical abstract</B></P> <P>[DISPLAY OMISSION]</P>
