        Background: Anemia is an accelerating problem among patients with heart failure. In this study, we investigated whether anemia in heart failure was related to hepcidin level. Methods: Totally 70 patients; 50 patients with heart failure and 20 otherwise healthy subjects with no history of a chronic illness including heart failure as control group, were included in the study. Heart failure was verifi ed by echocardiography in each subject and patients were defi ned as ones with reduced ejection fraction (HFrEF) if EF = 40% and with preserved ejection fraction (HFpEF) if EF 40-50%. Exclusion criteria included presence of kidney failure (serum creatinine level>1.4), history of hemorrhage (gastrointestinal or severe menstrual bleeding), chronic liver disease (ALT>2 folds), hematologic disease, iron replacement therapy or blood transfusion in the past six months, hypothyroidism, autoimmune disease and manifest infection. Results: There was no correlation between hepcidin concentration and age, weight, creatinine, iron, vitamin B12, folate, white blood cell (WBC), platetelet, mean corpuscular volume, ESR, ejection fraction (EF) (p > 0,05). There was a positive correlation between hepcidin level and urea, ferritin, hemoglobin, hematocrite, C-reactive protein (p < 0,05). Hepcidin levels of anemic heart failure patients were signifi cantly lower than the non-anemic heart failure patients (p < 0,05). Conclusions: We found that serum hepcidin level in anemic patients with heart failure was lower than in heart failure patients without anemia. Previously, progression of heart failure was found to be associated with decline in circulating hepcidin and the development of iron defi ciency and low hepcidin level was related to unfavorable out come. We believe that iron defi ency occurs as a result of infl ammatory process in heart failure and therefore hepcidin concentrations decrease as a response. However, long-term follow up studies are needed.

        Mass transfer enhancement factor for chemical absorption of carbon dioxide into sodium metaborate solution

        Hydrogen is getting increasing attention as a medium for energy storage, and sodium borohydride is accepted as a suitable carrier for hydrogen. The main product of the process by means of which hydrogen is produced from sodium borohydride is sodium metaborate. Our aim was to find an alternative use for sodium metaborate and specifically investigating the feasibility to use it for carbon dioxide capture from flue gases. The products of this chemical absorption are sodium carbonate, sodium bicarbonate and boric acid, all of which are industrially important chemicals. A bubble column was used in the experiments. Oxygen desorption technique was employed to determine the liquid side physical mass transfer coefficient. Chemical mass transfer coefficient was determined by absorption of carbon dioxide from its mixture with nitrogen into sodium metaborate solution. Enhancement factor was then calculated and a correlation was developed for it.

      • Awareness of Cancer Screening During Treatment of Patients with Renal Failure: A Physician Survey in Turkey

        Background: Today, survival rate of patients with chronic renal failure/hemodialysis has increased so that chronic illnesses are more likely to occur. Cancer is the main cause of morbidity and mortality in such patients. Aim: In this study, physician attitudes were examined about cancer screening in patients with renal failure. Materials and Methods: This study was done by face to face questionnaire in the $27^{th}$ National Nephrology Congress to determine if the physicians dealing with chronic renal failure, hemodialysis or renal transplanted patients, recommend cancer screening or not and the methods of screening for cervix, prostate, breast and colon cancer. Results: One hundred and fifty six physicians were included in the survey. A total of 105 (67%) participants were male and the age of responders was $48{\pm}9$ years. About 29% were specialists in nephrology, 28% internal medicine, and 5% were other areas of expertise. Some 48% of participants were hemodialysis certified general practitioners. Patients were grouped as compensated chronic renal failure, hemodialysis or renal transplanted. Of the 156 responders, 128 (82%) physicians recommended breast cancer screening and the most recommended subgroup was hemodialysis patients (15%). The most preferred methods of screening were combinations of mammography, self breast examination and physicianbreast examination. 112 (72%) physicians recommended cervix cancer screening, and the most preferred method of screening was pap-smear. Colon cancer screening was recommended by 102 (65%) physicians and prostate screening by 109 (70%) physicians. The most preferred methods of screening were fecal occult blood test and PSA plus rectal digital test, respectively. Conclusions: It is not obvious whether cancer screening in renal failure patients is different from the rest of society. There is a variety of screening methods. An answer can be found to these questions as a result of studies by a common follow-up protocol and cooperation of nephrologists and oncologists.

      • Multicenter Evaluation of Patients with Cutaneous Malignant Melanoma in Turkey: MELAS Study

        Background: Malignant melanoma is a cancer that demonstrates rapid progression and atypical clinically features with a poor prognosis. Aim: This study was performed to determine the clinical characteristics and treatment outcomes of patients with malignant melanoma in Turkey. Methods: The medical records of 98 patients between 2007-2012 at our centers were retrieved from the patient registry. Overall survival (OS) was calculated using the Kaplan-Meier method. Results: In our study, with the median follow-up of all patients with cutaneous MM of 46.3 months, the median OS rate of all cases was 43.6 months and 5-year OS was 48.6%. However, five-year OS rates of patients with localized disease (stage I-II) and node involvement (stage III) were 60.3% and 39.6%, respectively. The median OS of stage IV patients was 8.7 months and 1-year OS rate was 26.2%. We showed that advanced stage, male gender, and advanced age in all patients with MM were significant prognostic factors of OS. Conclusions: Compared with the results of current studies from Western countries, we found similar findings concerning demographical features, histological variables and survival analyses for our patients with cutaneous MM in Turkey.

        Low-shrinking composites. Are they reliable for bonding orthodontic retainers?

        Objective: To evaluate the shear bond strength (SBS), fracture mode, wire pull out (WPO) resistance and microleakage between low-shrinking and conventional composites used as a lingual retainer adhesive. Methods: A total of 120 human mandibular incisor teeth, extracted for periodontal reasons, were collected. Sixty of them were separated into two groups. To determine the SBS, either Transbond-LR (3M-Unitek) or Silorane (3M-Espe) was applied to the lingual surface of the teeth by packing the material into standard cylindrical plastic matrices (Ultradent) to simulate the lingual retainer bonding area. To test WPO resistance, 20 samples were prepared for each composite where the wire was embedded in the composite materialand cured. Then tensile stress was applied until failure of the composite occurred. The remaining 60 teeth were divided into two groups and multi-stranded 0.0215-inch diameter wire was bonded with the same composites. Microleakage was evaluated by the dye penetration method. Statistical analyses were performed by Wilcoxon, Pearson chi-square, and Mann-Whitney-U tests at p < 0.05 level. Results: The SBS and WPO results were not statistically significant between the two groups. Significant differences were found between the groups in terms of fracture mode (p < 0.001). Greater percentages of the fractures showed mix type failure (85%) for Silorane and adhesive (60%) for Transbond-LR. Microleakage values were lower in low-shrinking composite than the control and this difference was found to be statistically significant (p < 0.001). Conclusions: Low-shrinking composite produced sufficient SBS, WPO and microleakage values on the etched enamel surfaces, when used as a lingual retainer composite.

        Stimulation of bone formation by direct electrical current in an orthopedically expanded suture in the rat

        이 실험적 연구는 직류 전류 자극(direct electrical current stimulation, DECS)을 백서의 확장된 전상악골간 봉합에 가하였을 시 골 재생에 미치는 영향을 평가하기 위해 시행되었다. 60마리의 웅성 Wistar rats를 연구에 사용하였으며 대조군과 실험군의 2개의 군으로 나누었다. 상악 절치 사이에 30-gram의 힘을 발휘하는 helical-spring을 적용하여 상악골의 확장을 두 군 모두에서 시행하였다. 실험군에서는 상악골 측면부에 금속 스크류를 식립하였고 스크류에 전극을 연결하였다. 장치는 지속적으로 $10{\mu}A$의 전류를 흘러 보내도록 조절하였으며 전류는 확장기와 초기 유지기 동안 매일 점검하였다. 골 재생을 조직형태계측학적으로 평가하기 위해 신생골 면적(${\mu}m^2$), 골 둘레길이(${\mu}m$), feret's diameter (${\mu}m$) 와 신생골(%)을 계측하였다. 결과는 유의수준 p < 0.05에서 Kruskal-Wallis rank와 Mann-Whitney U tests를 이용하여 통계 분석하였다. 실험 결과 조사한 모든 조직형태계측적 수치들에서 통계적으로 유의한 차이가 양 군 사이에서 나타났다. 신생골 면적 (p = 0.002), 골 둘레길이 (p = 0.002), feret's diameter (p = 0.002)와 신생골 비율 (p = 0.002)의 모든 계측치가 실험군에서 대조군에 비해 높게 나타났다. 이런 결과는 DECS 군에서 골 형성이 더 양호함을 의미한다. 결론적으로 악정형적으로 확장시킨 전상악골 봉합부에 DECS를 적용하면 초기 유지 기간 중에 신생골 형성이 촉진된다고 할 수 있다. Objective: The aim of this experimental study was to evaluate the effects of direct electrical current stimulation (DECS) on bone regeneration in response to an expansion of the inter-premaxillary suture in the rat. Methods: Sixteen 50 - 60 days old Wistar male rats were separated into two equal groups (control and experimental). Both groups were subjected to expansion, and 30-gram of force was applied to the maxillary incisors with helical-spring. In the experimental group, two metallic-screws were placed at lateral parts of the maxillary segments. Electrodes were connected to the screws. The device was activated with current adjustment to measure $10{\mu}A$ continuously and the current was monitored daily during the expansion and early-retention phase. Bone regeneration in the sutural area was histomorphometrically evaluated including new-bone area (${\mu}m^2$), bone perimeter (${\mu}m$), feret's diameter (${\mu}m$) and newly formed bone (%) parameters. Kruskal-Wallis rank and Mann-Whitney U tests were used for statistical evaluation at p < 0.05 level. Results: Statistical analysis showed significant differences between groups for all investigated histomorphometric parameters. New bone area (p = 0.002), bone perimeter (p = 0.004), feret's diameter (p = 0.002) and newly formed bone percentage (p = 0.002) measurements were significantly higher in the experimental group than the control group. Bone histomorphometric measurements revealed that bone architecture in the DECS group was improved. Conclusions: The application of DECS to an orthopedically expanded inter-premaxillary suture area during the early retention phase stimulated the formation of new bone.

        Applying nano-HA in addition to scaling and root planing increases clinical attachment gain

        Purpose: This study evaluated the efficacy of treating periodontitis using subgingival nano-hydroxyapatite powder with an air abrasion device (NHAPA) combined with scaling and root planing (SRP). Methods: A total of 28 patients with stage III periodontitis (grade B) were included in this study, although 1 was lost during follow-up and 3 used antibiotics. The patients were divided into a test group and a control group. All patients first received whole-mouth SRP using hand instruments, and a split-mouth approach was used for the second treatment. In the test group, the teeth were treated with NHAPA for 15 seconds at 70% power per pocket. Subgingival plaque samples were obtained from the 2 deepest pockets at the test and control sites before treatment (baseline) and 3 months after treatment. The full-mouth plaque index (PI), gingival index (GI), papillary bleeding index (PBI), bleeding on probing (BOP), probing depth (PD) and clinical attachment level (CAL) were recorded at baseline and at 1- and 3-month post-treatment. Real-time polymerase chain reaction was used to determine the colonisation of Treponema denticola (Td), Porphyromonas gingivalis (Pg), and Aggregatibacter actinomycetemcomitans in the subgingival plaque. Results: From baseline to the first month, the test group showed significantly larger changes in BOP and CAL (43.705%±27.495% and 1.160±0.747 mm, respectively) than the control group (36.311%±27.599% and 0.947±0.635 mm, respectively). Periodontal parameters had improved in both groups at 3 months. The reductions of PI, GI, BOP, PD, and CAL in the test group at 3 months were greater and statistically significant. The total bacterial count and Td and Pg species had decreased significantly by the third month in both groups (P<0.05). Conclusions: Applying NHAPA in addition to SRP improves clinical periodontal parameters more than SRP alone. Subgingival NHAPA may encourage clot adhesion to tooth surfaces by increasing surface wettability.

        In this paper, we present some recent results on weighted pointwise convergence and the rate of pointwise convergence for the family of nonlinear double singular integral operators in the following form: $$T_{\eta}(f;x,y)={\int}{\int\limits_{{\mathbb{R}^2}}}K_{\eta}(t-x,\;s-y,\;f(t,s))dsdt,\;(x,y){\in}{\mathbb{R}^2},\;{\eta}{\in}{\Lambda}$$, where the function $f:{\mathbb{R}}^2{\rightarrow}{\mathbb{R}}$ is Lebesgue measurable on ${\mathbb{R}}^2$ and ${\Lambda}$ is a non-empty set of indices. Further, we provide an example to support these theoretical results.
