          The Vasodilator Effect of a Cream Containing 10% Menthol and 15% Methyl Salicylate on Random- Pattern Skin Flaps in Rats

          Background It is still difficult to prevent partial or full-thickness flap necrosis. In this study, the effects of a cream containing menthol and methyl salicylate on the viability of randompattern skin flaps were studied. Methods Forty female Sprague-Dawley rats were divided into two equal groups. Caudally based dorsal random-pattern skin flaps were elevated, including the panniculus carnosus. In the study group, 1.5 mL of a cream containing menthol and methyl salicylate was applied to the skin of the flap, and saline solution (0.9%) was used in the control group. Upon completion of the experiment, flap necrosis was analyzed with imaging software and radionuclide scintigraphy. Histopathological measurements were made of the percentage of viable flaps, the number of vessels, and the width of the panniculus carnosus muscle. Results According to the photographic analysis, the mean viable flap surface area in the study group was larger than that in the control group (P=0.004). According to the scintigrams, no change in radioactivity uptake was seen in the study group (P>0.05). However, a significant decrease was observed in the control group (P=0.006). No statistically significant differences were observed between the groups in terms of the percentage of viable flaps, the number of vessels, or the width of the panniculus carnosus muscle (P>0.05). Conclusions Based on these results, it is certain that the cream did not reduce the viability of the flaps. Due to its vasodilatory effect, it can be used as a component of the dressing in reconstructive operations where skin perfusion is compromised.

          Innovation in the planning of V-Y rotation advancement flaps: A template for flap design

          Local flaps exhibit excellent color matching that no other type of flap can compete with. Moreover, surgery using a local flap is easier and faster than surgery using a distant or free flap. However, local flaps can be much more difficult to design. We designed 2 templates to plan a V-Y rotation advancement flap. The template for a unilateral V-Y rotation advancement flap was used on the face (n=5), anterior tibia (n=1), posterior axilla (n=1), ischium (n=1), and trochanter (n=2). The template for a bilateral flap was used on the sacrum (n=8), arm (n=1), and anterior tibia (n=1). The causes of the defects were meningocele (n=3), a decubitus ulcer (n=5), pilonidal sinus (n=3), and skin tumor excision (n=10). The meningocele patients were younger than 8 days. The mean age of the adult patients was 50.4 years (range, 19–80 years). All the donor areas of the flaps were closed primarily. None of the patients experienced wound dehiscence or partial/total flap necrosis. The templates guided surgeons regarding the length and the placement of the incision for a V-Y rotation advancement flap according to the size of the wound. In addition, they could be used for the training of residents.

          Malicious Relay Detection Using Sentinels: A Stochastic Geometry Framework

          Next generation wireless networks are under high riskof security attacks due to increased connectivity and informationsharing among peer nodes. Some of the nodes could potentially bemalicious, intending to disrupt or tamper sensitive data transfer inthe network. In this paper, we present a detailed analysis of the sentinel based data integrity attack detection of malicious relays usinga stochastic geometry framework. We assume a practical channelmodel for each wireless link and apply a stochastic geometry approach to interference modeling. Two detection schemes depending on the level of connectivity between sentinel devices are proposed: isolated and co-operative detection. For both schemes, attack detection probability is derived as a function of important network parameters, and the minimum density of sentinels to achievea given detection probability is calculated. It will be shown that areasonable attack detection probability can be achieved even whenthe sentinel node density is much lower than the relay node density.

          Contingent Negative Variation Is Associated with Cognitive Dysfunction and Secondary Progressive Disease Course in Multiple Sclerosis

          Background and Purpose The relationship between contingent negative variation (CNV),which is an event-related potential, and cognition in multiple sclerosis (MS) has not been examined previously. The primary objective of the present study was thus to determine the association between CNV and cognition in a sample of MS patients. Methods The subjects of this study comprised 66 MS patients [50 with relapsing-remittingMS (RRMS) and 16 with secondary progressive MS (SPMS)] and 40 matched healthy volunteers. A neuropsychological battery was administered to all of the subjects; CNV recordingswere made from the Cz, Fz, and Pz electrodes, and the amplitude and area under the curve(AUC) were measured at each electrode. Results RRMS patients exhibited CNVs with lower amplitudes and smaller AUCs than thecontrols at Pz. SPMS patients exhibited CNVs with lower amplitudes and smaller AUCs thanthe controls, and CNVs with a smaller amplitude than the RRMS patients at both Cz and Pz. After correcting for multiple comparisons, a lower CNV amplitude at Pz was significantly associated with worse performance on measures of speed of information processing, verbal fluency, verbal learning, and verbal recall. Conclusions CNV may serve as a marker for disease progression and cognitive dysfunctionin MS. Further studies with larger samples and wider electrode coverage are required to fullyassess the value of CNV in these areas.

          Increased Serum Hepcidin Levels in Children and Adolescents with Attention Deficit Hyperactivity Disorder

          Objective: In this study, we aimed to evaluate the serum hepcidin levels in attention deficit hyperactivity disorder (ADHD) patients that were newly diagnosed with no history of psychotropic drugs. Methods: A total of 70 ADHD patients and 69 healthy controls were enrolled in our study. During the diagnosis, the Schedule for Affective Disorders and Schizophrenia for School-Age Children-Present and Lifetime version were applied. The sociodemographic data form, Turgay DSM-IV-Based Child and Adolescent Behavior Disorders Screening and Rating Scale, and Conners' Rating Scales-Revised: Long Form were used for the clinical evaluation. Serum hepcidin levels were measured and compared between the groups. Results: No significant difference between the groups in terms of age (p =0.533) and gender (p =0.397) was determined. In addition, the groups did not differ significantly for the other sociodemographic variables recorded. Serum hepcidin levels were found to be significantly higher in the patients with ADHD than healthy controls (p =0.019). Conclusion: To the best of our knowledge, this study is the first to evaluate the total serum hepcidin levels in ADHD patients. Our study findings may suggest that high levels of hepcidin may cause iron dysregulation in ADHD patients. However, further studies are required to establish a definite conclusion.

        • Lack of any Prognostic Role of Insulin-Like Growth Factor-1 Receptor in Non-Small Cell Lung Cancer

          Background: The purpose of this study is to determine whether the IGF1R expression has a prognostic role in non-small cell lung cancer. Materials and Methods: Forty-seven patients histopathologically diagnosed with small cell lung cancer upon bronchoscopic biopsy or resection materials were included in the study. IGF1R expression was examined via immunohistochemical methods. In samples, >10% staining were assessed as positive and ${\leq}10%$ as negative. Information about demographic datas and treatments was obtained by retrospective searches of patient files. Results: IGF1R expression was determined as positive in 38 (80.9%) and as negative in 9 (19.1%) patients. There was no significant relation between IGF1R expression and histological sub-type, local invasion, lymph node and metastasis status (p=0.842, p=0.437, 0.064, 0.447, respectively). There was also no correlation with IGF1R expression and survival (p=0.141). Conclusions: There are conflicting results between IGF1R and its prognostic effects in the various studies. It has been claimed in some studies it is not related to prognosis as in our study, and in some studies it has been claimed that it is a good prognostic factor whereas in some studies it has been claimed as being a factor for worse prognosis. We think that IGF1R expression in non-small cell lung carcinoma patients deserves further analysis, because of its potential prognostic and predictive roles.

          Background: Metabolic syndrome is a systemic disorder and manifests a group of conditions such as abdominal obesity, dyslipidaemia, hypertension, coronary artery diseases. The importance of epicardial adipose tissue recognized its contribution to inflammation by pro-inflammatory cytokines discharge has been proved. Several investigations were performed on vitamin D receptor in different tissues except bone. In this study the epicardial adipose tissue thickness (EATT) and the levels of vitamin D were measured and compared with healthy control group. Methods: A total of 148 patients (84 patients who had metabolic syndrome without diabetes and 64 healthy individuals) were enrolled into the study. In all patients, the EATT was calculated by ecocardiography and the level of serum 25(OH) vitamin D was monitored. Results: It has been observed that the EATT in patients with metabolic syndrome increases significiantly compared with healthy control group (p<0.001). No significant difference between patients and control group was found on the levels of 25 (OH) vitamin D (p=0.507). There was no correlation between 25 (OH) vitamin D and the EATT (p=0.622). Conclusion: We observed that the EATT increased in patients with metabolic syndrome. In contradiction to the literature; the levels of 25(OH) vitamin D has not been found high in patients with metabolic syndrome. The reason of this can be small number of patients in our study. A significant correlation was not found between the EATT and the levels of 25 (OH) vitamin D. Further studies with a larger patient population are required assess the relationship.
