        Learner-friendly textbooks: Chemistry texts based on a constructivist view of learning

        In this study, the use of inquiry methods, learning cycles, a conceptual change model and analogy in creating alternative science texts was discussed. An alternative text on the topic of acids and bases was created by integrating the methods and models discussed in this paper. The alternative text and a sample of a traditional text taken from a textbook, which is still used in Turkish high schools, were given to two groups, totaling 42 pre-service teachers-the alternative text was given to an experimental group and the traditional text to a control group-in an experimental setting and their understandings of acids and bases were compared. In addition, in the second step of the study, the pre-service teachers read both texts and indicated their preferences in terms of interest, understandability and helpfullness.

        Hybrid SNR‐Adaptive Multiuser Detectors for SDMA‐OFDM Systems

        Multiuser detection (MUD) and channel estimation techniques in space‐division multiple‐access aided orthogonal frequency‐division multiplexing systems recently has received intensive interest in receiver design technologies. The maximum likelihood (ML) MUD that provides optimal performance has the cost of a dramatically increased computational complexity. The minimum mean‐squared error (MMSE) MUD exhibits poor performance, although it achieves lower computational complexity. With almost the same complexity, an MMSE with successive interference cancellation (SIC) scheme achieves a better bit error rate performance than a linear MMSE multiuser detector. In this paper, hybrid ML‐MMSE with SIC adaptive multiuser detection based on the joint channel estimation method is suggested for signal detection. The simulation results show that the proposed method achieves good performance close to the optimal ML performance at low SNR values and a low computational complexity at high SNR values.

        A Rare Complication of Lumbar Spinal Surgery: Pneumocephalus

        A 25-year-old male patient with severe thigh and right side pain was presented. In the lumbar magnetic resonance images,there was a contrasting spinal cord tumor image with 1 cm in diameter at the level of the L3 vertebra. The patient was operated and the tumor was totally removed. All the pain of the patient recovered. The patient’s postoperative two days werevery comfortable and the patient was early mobilized. On the third postoperative day, the patient complained of severeheadache, nausea and vomiting. Immediate cranial computed tomography (CT) images was performed. Cerebral pneumocephalus was present in CT. The patient was treated with defnite bed rest and plenty of fluid replacement. After threedays, the patient’s complaints were completely improved. This improvement was confrmed by performing a cerebral CTscan. In the formation of pneumocephalus, air is compressed into the intradural space during operation and this passes tothe cerebral space. Defnite bed rest, abundant fluid replacement and, in addition, highly inspired oxygen therapy are suffcient to correct the condition.

        High-Efficiency Cationic Labeling Algorithm of Macroaggregated Albumin with 68Gallium

        Purpose The generator product radionuclide gallium-68(68Ga) is widely used for PET/CT imaging agents and the 68Ga-labeled MAA is an attractive alternative to 99mTc-labeled MAA. Using a commercially available MAA labeling kit for 99mTc, we presented a reliable synthesis protocol with a highly efficient, organic solvent-free cationic method in GMP conditions in the Scintomics automated synthesis unit. Methods The labeling process was performed by incubating for 7 min at 90 °C in the borax vial containing the generator product 68GaCl3 MAA-HEPES eluted from the PSH+ cartridge with 1.5 mL 5 molar NaCl. Quality control of the final product content was examined, and radiopharmaceutical production was carried out in accordance with GMP guidelines. Results 68Ga eluted from the generator was obtained in more than 99% radiochemical purity and efficiency. In this case, the labeling efficiency was found to be >99%. When the results of SEM-EDX analysis in the final product were examined, it was determined that most of toxic metals were no appreciable in the product content. Conclusions The radiochemical and chemical purity of the final product allows direct use without purification steps to remove “free 68Ga” or other toxic compounds.

        Scapular muscle endurance, shoulder pain, and functionality in patients with rotator-cuff-related shoulder pain: a matched, case-control study

        Background: Deficiency in scapular muscle endurance (SME) is a risk factor for rotator-cuff-related shoulder pain (RCRSP). However, the exact relationship among SME, pain, and functionality remains unclear. This study aims to compare SME, pain, and functionality in RCRSP patients to those in age-sex-matched healthy controls. Methods: Twenty-three patients with RCRSP and 23 age-sex matched healthy controls were included in the study. SME was measured using a 1-kg dynamometer. Self-reported pain level was assessed using a visual analog scale. The Functional Impairment Test-Hand, Neck, Shoulder, and Arm (FIT-HaNSA) was also used to assess functional impairment. Results: The control group had higher SME and total FIT-HaNSA scores than the patient group (P<0.05). There was a statistically significant and positive correlation between SME and FIT-HaNSA scores in both groups (P<0.05). Conclusions: SME was affected by RCRSP. Pain and functional impairment were correlated with low SME. Level of evidence: IV.

        Evaluation Expression of the Caspase-3 and Caspase-9 Apoptotic Genes in Schizophrenia Patients

        Objective: Apoptosis is programmed cell death that occurs by several pathways. Caspase-3 is induced by active caspase-9 via the intrinsic pathway. The aim of this research was to explore the expression of caspase-3 and caspase-9 in schizophrenia patients and healthy samples. Methods: RNA was isolated from the peripheral blood of 39 schizophrenia patients’ and healthy samples. After cDNA synthesis, real time PCR (RT-PCR) was used to analyse caspase-3 and caspase-9 gene expression. The severity of psychopathological symptoms of schizophrenia was evaluated using the Positive and Negative Symptoms Scale for schizophrenia (PANSS) and Clinical Global Impressions (CGI). Results: The expression of caspase-3 and caspase-9 genes was higher in schizophrenia patients than in healthy samples (p = 0.012, p = 0.002, respectively). The increase in caspase-3 gene expression was significant with being male, smoking and with a duration of less than 6 years (p = 0.047, p = 0.049, p = 0.034, respectively). On the other hand, the increase in caspase-9 gene expression was significant in patients who is smoke, have children, and are under 33 years old (p = 0.040, p = 0.043, p = 0.045, respectively). A significant positive correlation was detected between the caspase-3 and caspase-9 gene expression (r = 0.3218, p = 0.049). Conclusion: Our findings indicate that caspase-3 and caspase-9 gene expression may activate cell death mechanisms by intrinsic apoptotic genes. Furthermore, caspase-3 and caspase-9 may play essential roles in different ways in schizophrenia. Hence there is a need to further study the apoptotic mechanism with expanded patient populations.

        Management and Outcome of Intracranial Dural Arteriovenous Fistulas That Have Caused a Hemorrhage in the Posterior Fossa : A Clinical Study

        Objective : We evaluated the diagnosis, treatment, and long-term results of patients with dural arteriovenous fistula (dAVF), which is a very rare cause of posterior fossa hemorrhage. Methods : This study included 15 patients who underwent endovascular, surgical, combined, or Gamma Knife treatments between 2012 and 2020. Demographics and clinical features, angiographic features, treatment modalities, and outcomes were analyzed. Results : The mean age of the patients was 40±17 years (range, 17–68), and 68% were men (11/15). Seven of the patients (46.6%) were in the age group of 50 years and older. While the mean Glasgow coma scale was 11.5±3.9 (range, 4–15), 46.3% presented with headache and 53.7% had stupor/coma. Four patients (26.6%) had only cerebellar hematoma and headache. All dAVFs had cortical venous drainage. In 11 patients (73.3%), the fistula was located in the tentorium and was the most common localization. Three patients (20%) had transverse and sigmoid sinus localizations, while one patient (6.7%) had dAVF located in the foramen magnum. Eighteen sessions were performed on the patients during endovascular treatment. Sixteen sessions (88.8%) were performed with the transarterial (TA) route, one session (5.5%) with the transvenous (TV) route, and one session (5.5%) with the TA+TV route. Surgery was performed in two patients (14.2%). One patient (7.1%) passed away. While there were nine patients (64.2%) with a Rankin score between 0 and 2, the total closure rate was 69.2% in the first year of control angiograms. Conclusion : In the differential diagnosis of posterior fossa hemorrhages, the differential diagnosis of dAVFs, which is a very rare entity, should be considered, even in the middle and elderly age groups, in patients presenting with good clinical status and pure hematoma. The treatment of such patients can be done safely and effectively in a multidisciplinary manner with a good understanding of pathological vascular anatomy and appropriate endovascular treatment approaches.

        Effects of Trimetazidine on T Wave Alternans in Stable Coronary Artery Disease

        Background and Objectives: Studies reveal that the microvolt T wave alternans (MTWA) test has a high negative predictive value forarrhythmic mortality among patients with ischemic or non-ischemic cardiomyopathy. In this study, we investigate the effects oftrimetazidine treatment on MTWA and several echocardiographic parameters in patients with stable coronary artery disease. Subjects and Methods: One hundred patients (23 females, mean age 55.6±9.2 years) with stable ischemic heart disease were included inthe study group. Twenty-five age- and sex-matched patients with stable coronary artery disease formed the control group. All patientswere stable with medical treatment, and had no active complaints. Trimetazidine, 60 mg/day, was added to their current treatment for aminimum three months in the study group and the control group received no additional treatment. Pre- and post-treatment MTWAvalues were measured by 24 hour Holter testing. Left ventricular systolic and diastolic functions were assessed by echocardiography. Results: After trimetazidine treatment, several echocardiographic parameters related with diastolic dysfunction significantly improved. MTWA has been found to be significantly improved after trimethazidine treatment (63±8 μV vs. 53±7 μV, p<0.001). Abnormal MTWA waspresent in 29 and 11 patients pre- and post-treatment, respectively (p< 0.001). Conclusion: Trimetazidine improves MTWA, a non-invasive determinant of electrical instability. Moreover, several echocardiographic parametersrelated with left ventricular functions also improved. Thus, we can conclude that trimetazidine may be an effective agent to prevent arrhythmiccomplications and improve myocardial functions in patients with stable coronary artery disease.
