      • Efficacy and Tolerability of Weekly Docetaxel, Cisplatin, and 5-Fluorouracil for Locally Advanced or Metastatic Gastric Cancer Patients with ECOG Performance Scores of 1 and 2

        Background: Docetaxel, cisplatin, 5-fluorouracil (DCF) given every three weeks is an effective, but palliative regimen and significantly toxic especially in patients who have a low performance score. Here, we aimed to evaluate the efficacy and tolerability of a weekly formulation of DCF in locally advanced and metastatic gastric cancer patients. Materials and Methods: 64 gastric cancer patients (13 locally advanced and 51 metastatic) whose ECOG (Eastern Cooperative Oncology Group) performance status (PS) was 1-2 and who were treated with at least two cycles of weekly DCF protocol as first-line treatment were included retrospectively. The weekly DCF protocol included $25mg/m^2$ docetaxel, $25mg/m^2$ cisplatin, and 24 hours infusion of $750mg/m^2$ 5-fluorouracil, repeated every week. Disease and patient characteristics, prognostic factors, treatment response, grade 3-4 toxicity related to treatment, progression free survival (PFS) and overall survival (OS) were evaluated. Results: Of the patients, 41 were male and 23 were female; the median age was 63 (29-82) years. Forty-one patients were ECOG-1 and 23 were ECOG-2. Of the total, 81.2% received at least three cycles of chemotherapy. Partial response was observed in 28.1% and stabilization in 29.7%. Overall, the disease was controlled in 57.8% whereas progression was noted in 42.2%. The median time to progression was 4 months (95%CI, 2.8-5.2 months) and median overall survival was 12 months (95%CI, 9.2-14.8 months). The evaluation of patients for grade 3-4 toxicity revealed that 10.9% had anemia, 7.8% had thrombocytopenia and 10.9% had neutropenia. Non-hematologic toxicity included renal toxicity (7.8%) and thrombosis (1.6%). Conclusions: In patients with locally advanced or metastatic gastric cancer who were not candidates for DCF administered every-3-weeks, a weekly formulation of DCF demonstrated modest activity with minimal hematologic toxicity, suggesting that weekly DCF is a reasonable treatment option for such patients.

        The slenderness effect on wind response of industrial reinforced concrete chimneys

        There are several parameters affecting the response of industrial reinforced concrete (RC) chimneys, i.e., the severity of wind and earthquake loads acting to the structure, structural properties such as height and cross section of the chimney, the slenderness property of the structure etc. One of the most important parameter that should be considered while understanding the wind response of industrial RC chimneys is slenderness property. Although there is no certain definition for slenderness effect on these structures, some standards like ASCE-7 define slenderness from different aspects of the structural properties. In the first part of this study, general information about the definition of slenderness in the well-known standards and ten selected industrial RC chimneys are given. In the second part of the study, brief information about wind load standards that are used for calculating wind loads namely ACI 307/98, CICIND 2001, DIN 1056, TS 498 and Eurocode 1 is given. In the third part of the study, calculated wind loads for selected chimneys are represented. In the fourth part of this study, the internal forces obtained from load combinations that are applied to chimneys and some graphs presenting the effect of slenderness on chimneys are given. In the last part of the study, a conclusion and discussion part is taking place.

      • Prognostic Value of Baseline Neutrophil-Lymphocyte and Platelet-Lymphocyte Ratios in Local and Advanced Gastric Cancer Patients

        Background: We aimed to investigate the prognostic value of baseline neutrophil, lymphocyte, and platelet counts along with the neutrophil-lymphocyte ratio (NLR) and platelet-lymphocyte ratio (PLR) in local and advanced gastric cancer patients. Materials and Methods: In this retrospective cross-sectional study, a total of 103 patients with gastric cancer were included. For all, patient characteristics and overall survival (OS) times were evaluated. Data from a complete blood count test including neutrophil, lymphocyte, monocyte, white blood cell (WBC) and platelet (Plt) count, hemoglobin level (Hb) were recorded, and the NLR and PLR were obtained for every patient prior to pathological diagnosis before any treatment was applied. Results: Of the patients, 53 had local disease, underwent surgery and were administered adjuvant chemoradiotherapy where indicated. The remaining 50 had advanced disease and only received chemotherapy. OS time was $71.6{\pm}6$ months in local gastric cancer patients group and $15{\pm}2$ months in the advanced gastric cancer group. Univariate analysis demonstrated that only high platelet count (p=0.013) was associated with better OS in the local gastric cancer patients. In contrast, both low NLR (p=0.029) and low PLR (p=0.012) were associated with better OS in advanced gastric cancer patients. Conclusions: This study demonstrated that NLR and PLR had no effect on prognosis in patients with local gastric cancer who underwent surgery and received adjuvant chemoradiotherapy. In advanced gastric cancer patients, both NLR and PLR had significant effects on prognosis, so they may find application as easily measured prognostic factors for such patients.

        Optimum design of RC shallow tunnels in earthquake zones using artificial bee colony and genetic algorithms

        The main purpose of this study is to perform optimum cost design of cut and cover RC shallow tunnels using Artificial bee colony and genetic algorithms. For this purpose, mathematical expressions of objective function, design variables and constraints for the design of cut and cover RC shallow tunnels were determined. By using these expressions, optimum cost design of the Trabzon Kalekapısı junction underpass tunnel was carried out by using the cited algorithms. The results obtained from the algorithms were compared with the results obtained from traditional design and remarkable saving from the cost of the tunnel was achieved.

        Predictive Value of the TRACK Questionnaire as a Measure of Asthma Control in Preschool Aged Children

        Assessment of asthma control in preschool children is important for therapeutic decisions. Aim of this study was to evaluate the predictive value ofTRACK questionnaire scores for subsequent clinical parameters and to investigate the validity and reliability of the Turkish version of the TRACKquestionnaire. We enrolled 100 children with asthma aged 4 years or younger in this cohort study. We recorded sociodemographic characteristicsand clinical severity parameters. A pediatric allergist filled in the asthma severity scale and the caregiver of the child filled in the TRACK questionnaire. We called in the children again at the end of one month and recorded the same parameters and administered TRACK again. Uncontrolled asthmawas defined as a TRACK score below 80. According to the TRACK score, 65% of the children had controlled asthma initially while at the end ofthe study 64.1% had controlled asthma. At the beginning of the study, all clinical parameters were worse in children with uncontrolled asthma accordingto TRACK score. Similarly, other objective clinical parameters during the following one month period were worse in children with initial uncontrolledasthma. Cronbach’s alpha score for the TRACK questionnaire was 0.84. Turkish TRACK questionnaire is a valid and reliable tool that is predictiveof short term asthma prognosis.

        Effect of introducing RC infill on seismic performance of damaged RC frames

        The main objective of this study was to investigate the seismic behavior of damaged reinforced concrete frames rehabilitated by introducing cast in place reinforced concrete infills. Four bare and five infilled frames were constructed and tested. Each specimen consisted of two (twin) 1/3-scale, one-bay and two-story reinforced concrete frames. Test specimens were tested under reversed-cyclic lateral loading until considerable damage occurred. RC infills were then introduced to the damaged specimens. One bare specimen was infilled without being subjected to any damage. All infilled frames were then tested under reversed-cyclic lateral loading until failure. While some of the test frames were detailed properly according to the current Turkish seismic code, others were built with the common deficiencies observed in existing residential buildings. The variables investigated were the effects of the damage level and deficiencies in the bare frame on the seismic behavior of the infilled frame. The deficiencies in the frame were; low concrete strength, inadequate confinement at member ends, 90 degree hooks in column and beam ties and inadequate length of lapped splices in column longitudinal bars made above the floor levels. Test results revealed that both the lateral strength and lateral stiffness increased significantly with the introduction of reinforced concrete infills even when the frame had the deficiencies mentioned above. The deficiency which affected the behavior of infilled frames most adversely was the presence of lap splices in column longitudinal reinforcement.

      • Cyclooxygenase-2 Expression is not a Marker of Poor Survival in Lung Cancer

        Objective: Cyclooxygenase-2 (COX-2) has been claimed to play role in carcinogenesis and be related to a bad prognosis in tumours. The aim of this study was to investigate the relationship between COX-2 expression and clinical and pathological parameters in early and advanced stage lung cancer patients. Materials and Methods: A total of 73 patients with lung cancer (27 adenocarcinomas, 33 squamous cell carcinomas, 4 large cell carcinomas and 9 small cell cancer) were analysed retrospectively. COX-2 expression was evaluated by immunohistochemistry in resection materials or lung biopsies. Tumor cells demonstrating more intense staining than smooth muscle and endothelial cells were recorded as COX-2 positive. We investigated the correlation between increased COX-2 expression and histological type of the tumor, the stage of the disease and survival. Results: COX-2 expression was observed in 55% of the adenocarcinomas, 45% of the squamous cell carcinomas and 22% of the small cell carcinomas. No correlation was apparent between COX-2 expression and disease stage, histological type and the survival. Conclusion: The results of this study do not support COX-2 expression as an independent prognostic factor in lung cancer. However, since results of the literature are different, further studies made in larger series are needed.

      • Effect of macro and micro fiber volume on the flexural performance of hybrid fiber reinforced SCC

        The aim of this study is to investigate the flexural performance of hybrid fiber reinforced self-compacting concrete (HFRSCC) having different ratio of micro and macro steel fiber. A total of five mixtures are prepared. In all mixtures, the sum of the steel fiber content is 1% and also water/binder ratio is kept constant. The amount of high range water reducer admixture (HRWRA) is arranged to satisfy the workability criteria of self-compacting concrete. Four-point bending test is carried out to analyze the flexural performance of the mixtures at 28 and 56 curing days. From the obtained load-deflection curves, the load carrying capacity, deflection and toughness values are investigated according to ASTM C1609, ASTM C1018 and JSCE standards. The mixtures containing higher ratio of macro steel fiber exhibit numerous micro-cracks and, thus, deflection-hardening response is observed. The mixture containing 1% micro steel fiber shows worst performance in the view of all flexural parameters. An improvement is observed in the aspect of toughness and load carrying capacity as the macro steel fiber content increases. The test results based on the standards are also compared taking account of abovementioned standards.
