      • Poster Session : PS 0446 ; Infectious Disease ; Fever of Unknown Cause Which is the Cause of Intestinal Tuberculosis

        Background: In developing countries, tuberculosis (tbc) incidence is reduced; but immunocompromised patients still remain at high risk for the disease. Malignancy and Crohn`s disease should be considered in the differential diagnosis of gastrointestinal (GIS) tbc in immunocompromised patients. We present here the clinical course of a patient with fever of unknown origin and rectal bleeding. Methods: A 31-year-old male cachectic patient was admitted to hospital with abdominal pain, fever and diarrhea. The history of the patient revealed systemic lupus erythematosus, lupus nephritis and left middle cerebral artery infarction. Right hemiparesis was present. He was on warfarin 5mg/day, cilazapril 2,5mg/day, methylprednisolone 4mg/day, mycophenolate mofetil 2g/day, hydroxychloroquine 200mg/ day, levodopa+benserazide 375mg/day, levetiracetam 1 g/day. Creatinine was 2,79mg/dL, and C-reactive protein was 10 mg/dL. No pneumonic infi ltration was shown. Blood, urine and faeces cultures, Chlamydia IgM, mycoplasma IgM, toxoplasma IgM, EBV IgM, CMV IgM and PPD test were negative. Transesophageal echocardiography excluded infective endocarditis. Empiric antibiotic treatment with ceftriaxone, piperacillin-tazobactam and moxifi oxacin, and antifungal fi uconazole was started. On the 15th day hematochezia occured. Colonoscopy revealed three different massive lesions straightening the lumen in caecum, hepatic fi exure of colon and transverse colon (figure-1). Results: Pathologic examination demonstrated granulomatous lesion. Considering the positive results of Tbc-PCR treatment, the patient was diagnosed as GIS tbc and isoniazid, rifampicin, pyrazinamide and ethambutol were started. Conclusions: The ileocecal region is the most frequent localization of intestinal tbc. Colonic tbc is often localized in proximal colon and caecum, and usually associated with ileal tbc. It is rarely seen in transverse colon. Our patient is a rare case of gastrointestinal tbc presenting without pneumonic infi ltration and with an unusual localization in colon.

        The effect of coloring liquids on the translucency of zirconia framework

        PURPOSE Translucency of all-ceramic restorations is an important factor which affects the final appearance and esthetic outcome of the restoration. The aim of this study was to evaluate the effect of the shade of coloring liquid on the translucency of zirconia framework. MATERIALS AND METHODS Thirty zirconium oxide core plate (15 × 12 × 0.5 mm) were divided into 6 groups of 5 plates each. Each group was classified according to the shade of coloring liquid based on Vita Classic Scale (A2, A3, B1, C2, and D2), and each sample was immersed in coloring liquid for 3 seconds as recommended by the manufacturer, except for the control group. Contrast ratio, as a translucency parameter, was calculated using a spectrophotometer and the data were analyzed with oneway analysis of variance (ANOVA) and Tukey's honestly significant differences (HSD) tests (α=.05). RESULTS Significant differences in translucency among the control and test groups, and the B1 shaded group and other shades was observed. There were no significant differences among A2, A3, C2, and D2 shaded groups. CONCLUSION The translucency of the zirconium oxide cores was affected by the coloring procedure and significant differences in the translucency measurements were identified between specific shades.

        FAMILY SIZE, HUMAN CAPITAL AND GROWTH: STRUCTURAL PATH ANALYSIS OF RWANDA

        This paper analyzes the macroeconomic role that different household groups play in human capital formation, sectoral growth and income distribution in Rwanda. Using a disaggregated SAM for Rwanda and, with the assistance of structural path analysis, the paper explores the macroeconomic implications of family size for human capital, sectoral growth and income distribution. The findings support the so-called quantity-quality trade-off hypothesis: the smaller the family size, the higher the investment in human capital. In particular, the human capital investment of households with 1-3 children tends to be more pronounced than that of households with more than 3 children. Moreover, households with 1-3 children act as an important intermediate pole transmitting the influence of human capital investment on agricultural production. As a result, promoting family planning programs seems to be a viable strategy for economic growth and poverty reduction.

        Evaluation of the resin cement thicknesses and push-out bond strengths of circular and oval fiber posts in oval-shapes canals

        PURPOSE The aim of this study was to evaluate whether the push-out bond strength varies between oval and circular fiber posts, and to examine the effect on the resin cement thicknesses around the posts. MATERIALS AND METHODS Eighteen mandibular premolar roots were separated into two groups for oval and circular fiber posts systems. Post spaces were prepared and fiber posts were luted to the post spaces. Roots were cut horizontally to produce 1-mm-thick specimens. Resin cement thicknesses were determined with a metallographic optical microscope and push-out tests were done. RESULTS No significant differences were observed in terms of push-out bond strength between the oval and circular fiber posts (P>.05) The resin cement thicknesses of the oval posts were greater than those of the circular posts group in the coronal, middle and apical specimens (P<.05). CONCLUSION In the light of these results, it can be stated that resin cement thickness does not affect the push-out bond strength.

        A comparative study of multi-objective evolutionary metaheuristics for lattice girder design optimization

        The geometric nonlinearity has been successfully integrated with the design of steel structural system. Thus, the tubular lattice girder, one application of steel structural systems have already been optimized to obtain an economic design following the completion of computationally expensive design procedure. In order to decrease its computing cost, this study proposes to employ five multi-objective metaheuristics for the design optimization of geometrically nonlinear tubular lattice girder. Then, the employed multi-objective optimization algorithms (MOAs), NSGAII, PESAII, SPEAII, AbYSS and MoCell are evaluated considering their computing performances. For an unbiased evaluation of their computing performance, a tubular lattice girder with varying size-shape-topology and a benchmark truss design with 17 members are not only optimized considering the geometrically nonlinear behavior, but three benchmark mathematical functions along with the four benchmark linear design problems are also included for the comparison purpose. The proposed experimental study is carried out by use of an intelligent optimization tool named JMetal v5.10. According to the quantitative results of employed quality indicators with respect to a statistical analysis test, MoCell is resulted with an achievement of showing better computing performance compared to other four MOAs. Consequently, MoCell is suggested as an optimization tool for the design of geometrically nonlinear tubular lattice girder than the other employed MOAs.

        Multiobjective size and topolgy optimization of dome structures

        The size and topology of geometrically nonlinear dome structures are optimized thereby minimizing both its entire weight & joint (node) displacements and maximizing load-carrying capacity. Design constraints are implemented from provisions of American Petroleum Institute specification (API RP2A-LRFD). In accordance with the proposed design constraints, the member responses computed by use of arc-length technique as a nonlinear structural analysis method are checked at each load increment. Thus, a penalization process utilized for inclusion of unfeasible designations to genetic search is correspondingly neglected. In order to solve this complex design optimization problem with multiple objective functions, Non-dominated Sorting Genetic Algorithm II (NSGA II) approach is employed as a multi-objective optimization tool. Furthermore, the flexibility of proposed optimization is enhanced thereby integrating an automatic dome generating tool. Thus, it is possible to generate three distinct sphere-shaped dome configurations with varying topologies. It is demonstrated that the inclusion of brace (diagonal) members into the geometrical configuration of dome structure provides a weight-saving dome designation with higher load-carrying capacity. The proposed optimization approach is recommended for the design optimization of geometrically nonlinear dome structures.

        Theoretical Study of the N-(2,5-Methylphenyl)salicylaldimine Schiff Base Ligand: Atomic Charges, Molecular Electrostatic Potential, Nonlinear Optical (NLO) Effects and Thermodynamic Properties

        Optimized geometrical structure, atomic charges, molecular electrostatic potential, nonlinear optical (NLO) effects and thermodynamic properties of the title compound N-(2,5-methylphenyl)salicylaldimine (I) have been investigated by using ab initio quantum chemical computational studies. Calculated results showed that the enol form of (I) is more stable than keto form. The solvent effect was investigated for obtained molecular energies, hardneses and the atomic charge distributions of (I). Natural bond orbital and frontier molecular orbital analysis of the title compound were also performed. The total molecular dipole moment (${\mu}$), linear polarizability (${\alpha}$), and first-order hyperpolarizability (${\beta}$) were calculated by B3LYP method with 6-31G(d), 6-31+G(d,p), 6-31++G(d,p), 6-311+G(d) and 6-311++G(d,p) basis sets to investigate the NLO properties of the compound (I). The standard thermodynamic functions were obtained for the title compound with the temperature ranging from 200 to 450 K.

        Estimation of compression strength of polypropylene fibre reinforced concrete using artificial neural networks

        In this study, Artificial Neural Networks (ANN) analysis is used to predict the compression strength of polypropylene fibre mixed concrete. Polypropylene fibre admixture increases the compression strength of concrete to a certain extent according to mix proportion. This proportion and homogenous distribution are important parameters on compression strength. Determination of compression strength of fibre mixed concrete is significant due to the veridicality of capacity calculations. Plenty of experiments shall be completed to state the compression strength of concrete which have different fibre admixture. In each case, it is known that performing the laboratory experiments is costly and time-consuming. Therefore, ANN analysis is used to predict the 7 and 28 days of compression strength values. For this purpose, 156 test specimens are produced that have 26 different types of fibre admixture. While the results of 120 specimens are used for training process, 36 of them are separated for test process in ANN analysis to determine the validity of experimental results. Finally, it is seen that ANN analysis predicts the compression strength of concrete successfully.

        An Evaluation of Glucose Tolerance in Essential Hypertension

        Purpose: This study aimed to determine the impaired glucose tolerance and diabetes prevalence in patients with essential hypertension (HT) and to compare the developed microvascular complications of these groups. Materials and Methods: An oral glucose tolerance test (OGTT) was performed on 338 essential hypertensive cases and glucose tolerances were classified according to ADA-2002 criteria. Results: Of the 338 cases, 32 people had diabetes (DM, 9.46%), 78 people had glucose intolerance (IGT, 23.1%), and 228 people had only hypertension but not IGT and DM (67.4%). Both the mean ages of the DM group (56.9 ± 6.7 years, p = 0.002) and IGT group (56.3 ± 8.4 years, p = 0.003) were older than the mean age of the control group (51.1 ± 6.4 years). The risk of IGT development was found to be four times greater in male cases than female cases when compared to the control group (p = 0.004, add ratio = 4.194). There were no significant differences in the body mass indexes (BMI’s), hypertension durations, and microvascular complications between the groups. Conclusion: In conclusion, the risk of IGT and DM development in hypertensive cases increases with aging and longer hypertension duration. The risk of IGT development in hypertensive cases is four times more in males.
