This report presents the case of a patient demonstrating multicentric Castleman’s disease (MCD) with a lung lesion that was successfully treated with an anti-interleukin-6 receptor antibody, tocilizumab in combination with corticosteroid and tacrolimus. A 43-yr-old female with abnormal shadows on a chest X-ray was referred to the hospital for further examination. She was diagnosed as having MCD based on the characteristic pathology of inguinal lymph node, lung lesions, laboratory data, and undifferentiated arthritis. Corticosteroid and rituximab therapy did not fully ameliorate the symptoms; thus, the therapeutic regimen was changed to include tocilizumab, oral corticosteroid and tacrolimus. This regimen resulted in clinical remission and the dose of tocilizumab and corticosteroid could be tapered. Tocilizumab in combination with corticosteroid and tacrolimus may therefore be a beneficial treatment regimen for lung lesions associated with MCD.

Environments surrounding production sites have changed greatly in recent years. Accommodating environmental changes calls for the design and development of information systems that center on production lines. There is a need for a training program that teaches learners to understand the particulars of an operation and apply that knowledge to an information system. In this research, we used material requirements planning (MRP) as the subject for which basic skills are to be taught and developed an MRP exercise-based training program. The program is designed for 13 lectures of 90 minutes each, and it consists of MRP exercises, modeling methods to represent them, the use of a programming language for system development, and finally, evaluation of the exercises. Lecture materials are described in 505 lecture slides using Microsoft PowerPoint to allow visualization of topics through graphs and models. The developed training program was then delivered to 86 college students, and its results were measured through quizzes to verify educational effectiveness.

Discarding case-carburized gears with excessive case-depth causes financial and environmental losses. To save the losses, we experimentally investigated the applicability of case-depth modification by additional heat treatments (AHTs) to such gears in terms of bending fatigue and impact strength. The AHTs consisted of an intermediate softening treatment (IST) (vacuum normalizing, tempering, induction normalizing, or none) to case-carburized spur gears with an excessive case-depth, followed by induction heating and quenching. All the AHTs reduced the effective case-depth, maintained the surface residual stress, refined the microstructure near tooth root surfaces, and maintained the bending fatigue limit. Moreover, all the AHTs increased the impact limit because of the refinement of the microstructure, whereas omitting an IST reduced this increase. Thus, case-depth modification by AHTs is applicable in terms of bending fatigue and impact strength, and conducting an IST is highly recommended.
