Objective: To determine the safety and usefulness of a two-tiered approach to balloon-occluded retrograde transvenous obliteration (B-RTO) as a treatment for large gastric varices after portal hypertension. Materials and Methods: 50 patients were studied who underwent B-RTO for gastric varices between October 2004 and October 2011 in our institution. The B-RTO procedure was performed from the right femoral vein and the B-RTO catheter was retained until the following morning. Distribution of sclerotic agents in the gastric varices on fluoroscopy was evaluated in all patients on days 1 and 2. When distribution of sclerotic agents in the gastric varices on day 1 had been none or very scanty even though the volume of the sclerotic agent infused was above the acceptable level, a second infusion was administered on day 2. When distribution was satisfactory, the B-RTO catheter was removed. Results: In 8 (16%) patients, little or no sclerotic agent infused on day 1 was distributed in the gastric varices. However, on day 2, sclerotic agents were distributed in all gastric varices. Mean volume of ethanolamine oleate-iopamidol infused on day 1 was 24.6 mL and was 19.4 mL on day 2. Gastric varices were well obliterated with no recurrence. Complications caused by the sclerotic agent such as pulmonary edema or renal insufficiencies were not seen. Conclusion: When gastric varices are very large, a strategy involving thrombosis of only the drainage vein on the first day followed by infusing the sclerotic agent on the following day might be effective and feasible.

Purpose: Pelvic exenteration (PE) is a highly invasive procedure with high morbidity and mortality rates. Promising options to reduce this invasiveness have included laparoscopic and transperineal approaches. The aim of this study was to identify the safety of combined transabdominal and transperineal endoscopic PE for colorectal malignancies. Methods: Fourteen patients who underwent combined transabdominal and transperineal PE (T group: 2-team approach, n = 7; O group: 1-team approach, n = 7) for colorectal malignancies between April 2016 and March 2020 in our institutions were included in this study. Clinicopathological features and perioperative outcomes were compared between groups. Results: All patients successfully underwent R0 resection. Operation time tended to be shorter in the T group (463 minutes) than in the O group (636 minutes, P = 0.080). Time to specimen removal was significantly shorter (258 minutes vs. 423 minutes, P = 0.006), blood loss was lower (343 mL vs. 867 mL, P = 0.042), and volume of blood transfusion was less (0 mL vs. 560 mL, P = 0.063) in the T group, respectively. Postoperative complications were similar between groups. Conclusion: Combined transabdominal and transperineal PE under a synchronous 2-team approach was feasible and safe, with the potential to reduce operation time, blood loss, and surgeon stress.

Euglena gracilis abruptly changes its swimming direction after a sudden increase or decrease in incident light intensity, that is, step-up or step-down photophobic responses, resulting in photoavoidance or photoaccumulation, respectively. To identify the photoreceptor molecules for these UV-A/blue-light type photobehaviors, we purified a flavoprotein from isolated putative photosencory organelles (PFBs) of Euglena. The purified flavoprotein, which noncovalently bound flavin adenine dinucleotide (FAD), seemed to be a heterotetramer of alpha- and beta-subunits. Predicted amino acid sequences of each of the subunits were similar to each other and contained two FAD-binding domains each followed by an adenylyl cyclase catalytic domain. The purified flavoprotein actually showed adenylyl cyclase activity, being drastically elevated by blue-light irradiation. Suppression of gene expression of the flavoprotein (Photoactivated Adenylyl Cyclase, PAC) by RNA interference (RNAi) caused loss of the step-up photophobic response, demonstrating that PAC actually mediates photoavoidance of Euglena.

The coercivity of a single 27 nm-spherical barium ferrite (S-BaFe) particle was simulated using three models: 1) Gibbs free energy (GFE), 2) Landau-Lifshitz-Gilbert (LLG), and 3) Stoner-Wohlfarth (S-W). Spherically and hexagonally shaped particles were used in the GFE and LLG simulations to investigate coercivity with the different shape anisotropies. The effect of shape was not included in the S-W model. It was found that the models using a spherical shape resulted in a coercivity higher than the models using the hexagonal shape with both shapes having the same diameter. The coercivity estimated with the S-W model was approximately the same as that for the spherical-shape models, which indicates that spherical shape has no significant effect on the particle’s coercivity at nanoscale.
