Confocal Microscopy in the Esophagus and Stomach

Probe-based confocal microscopy (pCLE) is actively being investigated for applications in the esophagus and stomach. The use of pCLE allows real-time in vivo microscopy to evaluate the microarchitecture of the mucosal epithelium. pCLE appears to be particularly useful in identifying mucosal dysplasia and early malignancies that cannot be clearly distinguished using high-definition white light endoscopy, chromoendoscopy, or magnification endoscopy. In addition, the ability to detect dysplastic tissue in real-time may shift the current screening practice from random biopsy to targeted biopsy of esophageal and gastric cancers and their precursor lesions. We will review the use of pCLE for detection and surveillance of upper gastrointestinal early luminal malignancy.

Use of synthetic fertilizers and pesticides on yards is increasingly scrutinized by government officials for health and environmental risks but has not been extensively analyzed by economists. In our model, estimated with data from a national survey, households choose initially how much, if any, money to spend on synthetic chemicals and subsequently how much time to spend on other yard tasks. Expenditures on synthetic chemicals are income inelastic. Time spent on subsequent yard care decreases as income increases but increases as prior chemical expenditures increase. These effects on time allocation and our theoretical model help us to characterize and qualitatively analyze the private benefits of synthetic chemical use and costs of a ban on that use. Data on prices and quantities are necessary to estimate these costs and determine the extent to which politically feasible financial incentives would encourage use of 'environmentally friendlier' alternatives. The non-effect of the number of children on expenditures underscores the importance of future research about expenditures on specific risk-classes of pesticides and household perceptions of risks.

Systemic lupus erythematosus (SLE, MIM152700) is an autoimmune disease characterized by self-reactive antibodies resulting in systemic inflammation and organ failure. TNFAIP3, encoding the ubiquitin-modifying enzyme A20, is an established susceptibility locus for SLE. By fine mapping and genomic re-sequencing in ethnically diverse populations, we fully characterized the TNFAIP3 risk haplotype and identified a TT>A polymorphic dinucleotide (deletion T followed by a T to A transversion) associated with SLE in subjects of European (P = 1.58 ? 10<SUP>??8</SUP>, odds ratio = 1.70) and Korean (P = 8.33 ? 10<SUP>??10</SUP>, odds ratio = 2.54) ancestry. This variant, located in a region of high conservation and regulatory potential, bound a nuclear protein complex composed of NF-觀B subunits with reduced avidity. Further, compared with the non-risk haplotype, the haplotype carrying this variant resulted in reduced TNFAIP3 mRNA and A20 protein expression. These results establish this TT>A variant as the most likely functional polymorphism responsible for the association between TNFAIP3 and SLE.

The laryngeal mask airway (LMA) ClassicTM and Air-QⓇ are supralaryngeal devices used for airway management in routine and difficult pediatric airways. We describe a novel two-stage technique of insertion of the LMA ClassicTM awake prior to induction of anesthesia, to assure oxygenation and ventilation, and after induction removal and placement of the Air-QⓇ for intubation using the flexible fiberoptic bronchoscope. The LMA Classic’sTM pliable design and relatively small size allow it to be easily placed in awake infants. In contrast, the Air-QⓇ is an excellent device for intubation because of its larger internal diameter and removable 9 mm adapter. Our goal was to reduce unpredictability and potentially increase the safety of induction of anesthesia and intubation in infants with Pierre Robin sequence. By using these devices in a two-stage approach we created a technique for consistent oxygenation, ventilation, and intubation in these infants.
