        Two Cases of Severe Type 2 Respiratory Failure Associated with Diffuse Idiopathic Skeletal Hyperostosis

        Diffuse idiopathic skeletal hyperostosis (DISH) is a non-inflammatory process characterized by hyperostosis at tendon insertions and around joint capsules and ossification of the anterior longitudinal ligament of the spine. The flexibility of the spinal column is reduced in DISH and affects the movement of the thorax, leading to restrictive ventilatory function. In this report, we describe the first two cases of severe type 2 (hypercapnic) respiratory failure associated with DISH. Two older men presented with histories of shortness of breath. Radiography of the spine revealed DISH with coexisting ankylosis of the costovertebral joints. The patients’ thoracic motion was severely restricted, reducing the mechanism of lung expansion to diaphragm contraction only. Both patients required non-invasive positive-pressure ventilation therapy to cope with their conditions. Our report sheds light on the risk of potentially life-threatening respiratory manifestations of DISH among older adults.

        Crystallization of a-Si Films with Smooth Surfaces by Using Blue Multi-Laser Diode Annealing

        Crystallization of ∼50-nm-thick amorphous Si (a-Si) with a smooth surface was achieved by using Blue Multi-Laser Diode Annealing (BLDA). The a-Si films were deposited by using RF sputtering with Ne gas or by using plasma enhanced chemical vapor deposition (PE-CVD) and were annealed by using BLDA with CW scanning. After the films had been annealed a relatively low laser power below 4 W, the root-mean-square (RMS) roughness deduced from the atomic force microscopy (AFM) results for the surfaces of the Si films was slightly increased, but smoothness was within 3 nm despite the conditions under which the films had been crystallized. BLDA has a potential to realize next-generation poly-Si thin film transistors (TFTs).

      • HCV Elimination in Japan: The Japanese Policy and Outcome

        In Japan, estimated number of chronic HBV infection was 1.1-1.4 million and that of chronic HCV was 1.9-2.3 million in 2000. The research on National Database (NDB) estimated number of chronic HBV infection on treatment was 0.16 million and that of chronic HCV was 0.47 million in 2015, respectively. The mortality of HCC had been increasing and hit the peak at around 2002, which subsequently started to decrease. Real-world clinical data have proven that direct anti-viral agents (DAAs) successfully eradicate HCV from more than 95% of the infected patients. In the clinical practice in Japan, optimization of DAAs treatment has been done based on HCV genotypes, stages of liver disease (chronic hepatitis or compensated/decompensated cirrhosis), prior experience of DAAs and the pattern of resistance-associated substitutions (RAS) on treatment. Of particular importance, meticulous care is needed for the treatment of patients with prior DAA failure and decompensated liver cirrhosis. Such tailored DAA treatment is guided by the Guidelines for the management of hepatitis C virus infection, updated and issued from Japan Society of Hepatology (JSH). Most of DAAs have been approved and registered in Japan, which enables us to choose multiple treatment options. In addition, for patients on treatment, drug prices of DAAs and examination expenses should be covered by special subsidy program for viral hepatitis. The national and local government cover the amount in excess of 100-200 USD of the cost of treatment. Japan has a national action plan for addressing viral hepatitis called, Basic Act on Hepatitis Measures, established in 2009. Basic Guidelines for Promotion of Control Measures for Hepatitis is issued in 2011 and was updated in 2016, comprising 9 principles in order to promote measures to prevent hepatitis B and C (Oza N, Kanto T et al., Hepatology Research, 2017). There are few countries like Japan that implement strategy against viral hepatitis targeting general population. According to these guidelines, national and local government share screening costs for testing hepatitis B and C for those who are over 40 years old residents. Thus, out-of-pocket expenses from examinees are free of charge or reduced to the minimum. From December 2018, special coverage program of medical expenses, shared by central and local government, has started for patients with HBV- or HCV-induced liver cancer and decompensated cirrhosis. However, in the cascade-of-care of viral hepatitis in Japan, significant gaps still remain in the diagnosis, treatment and their transition of patients in need. Questionnaire analysis for general people in Japan, which was performed in 2011 and 2017 by the government-funded research group including us, revealed that information regarding clinics and hospitals where testing is available for free has not been well disseminated. In addition, lack of correct knowledge on viral hepatitis in ordinally people sometimes induce stigma and discrimination against patients. Therefore, awareness raising in general people still in great demand for promoting hepatitis policy in Japan. The Hepatitis Information Center (HIC), established in the Research Center for hepatitis and Immunology, National Center for Global health and Medicine in 2008. The HIC has been collaborating with 71 regional core hospitals to promote hepatitis measures in Japan. According to the nationwide annual survey conducted by us, regional core centers have come to play varied roles in hepatitis treatment and expanded their programs, including awareness raising in general people, education of health care workers and hepatitis medical coordinators (HMC) (Setoyama H, Kanto T, et al., Hepatology Research, 2019). In order to achieve HCV elimination target by 2030, the strengthening of hepatitis care networks is needed that include regional core centers, specialized medical institutions, primary care physicians, HMCs and local governments. Several advantages have been prevailed in Japanese health care systems for patients with viral liver disease compared to those in countries in Asia and Pacific regions. Therefore, Japan should take a lead in helping the implementation of practical hepatitis action plan to each country where in need.

        Rethinking the Social Contributions of Religions

        Until World War II, Faith based Organizations (FBOs) have been played the main role in the field of Japanese social welfare. After WWII, the social welfare became state responsibility. After that, religious groups in Japan tended to be excluded from the field of social welfare. However, from the late 1970s until today, the State’s role in the field of social welfare has changed drastically in the background of the crisis of welfare state. In this context, FBOs started to re-participate in the field of social welfare, in particular newly emerging social issues such as homeless problem. Some groups are very active in missionary work in supporting homeless people. On the other hand, some groups rather separate its missionary work and homeless support. In this paper I focus on FBOs that supporting homeless people. Also I clarify the social roles of FBOs by using FBO activities configuration matrix.

        Corrected empirical Bayes confidence intervals in nested error regression models

        In the small area estimation, the empirical best linear unbiased predictor (EBLUP) or the empirical Bayes estimator (EB) in the linear mixed model is recognized to be useful because it gives a stable and reliable estimate for a mean of a small area. In practical situations where EBLUP is applied to real data, it is important to evaluate how much EBLUP is reliable. One method for the purpose is to construct a confidence interval based on EBLUP. In this paper,we obtain an asymptotically corrected empirical Bayes confidence interval in a nested error regression model with unbalanced sample sizes and unknown components of variance. The coverage probability is shown to satisfy the confidence level in the second-order asymptotics. It is numerically revealed that the corrected confidence interval is superior to the conventional confidence interval based on the sample mean in terms of the coverage probability and the expected width of the interval. Finally, it is applied to the posted land price data in Tokyo and the neighboring prefecture.

      • A Flexible Power and Delay Modeling and Optimization for Small Size Systems Operated by Battery

        Due to the rapid popularization of portable equipments, a goal for designer becomes to make the battery lifetime longer without increasing the total system size. Especially for ubiquitous computing age, long battery lifetime in a tight size limitation will be required. A new flexible power and delay modeling for LSI are proposed in this paper. These models help the optimization for small size systems, is proposed in, to be more accurate and useful. The result of the optimization is shown in this paper.

      • Force Control Strategy for Massage Chair Based on Masseur’s Knowhow Data Base

        Recently, the massage chair is used by a lot of people because they were able to use it easily at home. However, a present massage chair only realizes the massage motion, and the physical condition of the user and the massage force is not considered. Then, the professional masseur’s massage process is that the physical condition of the patient is estimated, and the appropriate massage force is decided based on the estimated physical condition, and the decided massage force is realized. This paper proposes the method of applying masseur’s procedure for the massage chair. The realizability of the proposed method is verified by the experimental work using the massage chair.

        The Role of Heat Shock Response in Insulin Resistance and Diabetes

        The expansion of life-style related diseases, such as metabolic syndrome (MS) and type 2 diabetes mellitus (T2DM), appears to be unstoppable. It is also difficult to cease their complications in spite of many antidiabetic medications or intervention of public administration. We and our collaborators found that physical medicine using simultaneous stimulation of heat with mild electric current activates heat shock response, thereby reducing visceral adiposity, insulin resistance, chronic inflammation and improving glucose homeostasis in mice models of T2DM, as well as in humans with MS or T2DM. This combination therapy exerts novel action on insulin signaling, β-cell protection and body compositions, and may provide a new therapeutic alternative in diabetic treatment strategy.
