        Surgical Techniques to Prevent Nipple-Areola Complex Malposition in Two-Stage Implant- Based Breast Reconstruction

        Background Appropriate position of the nipple-areolar complex (NAC) is crucial following nipple-sparing mastectomy (NSM). The prevention of NAC malposition in two-stage implant-based breast reconstruction has not been well described, and the efficacy of the techniques has not been evaluated. This study aimed to evaluate the efficacy of our technique to prevent NAC malposition in patients who underwent implant-based breast reconstruction after NSM. Methods Patients who underwent two-stage implant-based breast reconstruction with NSM between January 2012 and December 2019 were included. We used a surgical technique to fix the NAC to the rigid base, assuming a pocket-like appearance, with pectoralismajormuscle and lateral adipofascial flap at the time of tissue expander (TE) insertion. Patients were classified into two groups based on the performance of the technique for the prevention of NAC malposition. Results In 35 patients who underwent implant-based breast reconstruction after NSM, the clavicle-to-nipple distance ratio was 96.05.0% in thosewho underwent NAC fixation and 86.111.5% in those who did not undergo NAC fixation. Conclusions Using our technique, NAC malposition could be prevented in two-stage implant-based breast reconstruction. NAC fixation during TE insertion was found to be extremely effective. This procedure successfully prevented NAC malposition without the formation of extra scars.

        Overlapping of /o/ and /u/ in modern Seoul Korean

        Previous studies have reported on the overlapping of F₁ and F₂ distribution for the vowels /o/ and /u/ produced by young Korean speakers of the Seoul dialect. It has been suggested that the overlapping of /o/ and /u/ occurs due to sound change. However, few studies have examined whether speech rate influences the overlapping of /o/ and /u/. On the other hand, previous studies have reported that the overlapping of /o/ and /u/ in syllable produced by male speakers is smaller than by female speakers. Few reports have investigated on the overlapping of the two vowels in read speech produced by male speakers. In the current study, we examined whether speech rates affect overlapping of /o/ and /u/ in read speech by male and female speakers. Read speech produced by twelve young adult native speakers of Seoul dialect were recorded in three speech rates. For female speakers, discriminant analysis showed that the discriminant rate became lower as the speech rate increases from slow to fast. Thus, this indicates that speech rate is one of the factors affecting the overlapping of /o/ and /u/. For male speakers, on the other hand, the discriminant rate was not correlated with speech rate, but the overlapping was larger than that of female speakers in read speech. Moreover, read speech by male speakers was less clear than by female speakers. This indicates that the overlapping may be related to unclear speech by sociolinguistic reasons for male speakers.

        Protection of Kyo-yasai (heirloom vegetables in Kyoto) from extinction: a case of Sabaka-daikon (Japan's heirloom white radish, Raphanus sativus) in Maizuru, Japan

        As a general background, Sabaka-daikon, one of the heirloom vegetables in Kyoto, ceased its commercial cultivation in the late 1960s at north Kyoto, leading to an extinction crisis. We review the activities of the local residents to revive the commercial cultivation of Sabaka-daikon from 2010. We also discuss the significance of the information on the health benefits of local foods in motivating local residents to protect local foods from extinction. For methodology, through field interviews of administrative members, venders, and farmers in Maizuru city, we explore reasons for the success in reviving a local heirloom vegetable, Sabaka-daikon. We also apply chemical analysis to determine individual differences in the anticarcinogenic (cancer preventive) compound, 4-methylthio-3-butenyl isothiocyanate, in Sabaka-daikon to guarantee its added health benefits. As a result, the primary factors for the revival of Sabaka-daikon in Maizuru city include the passion of farmers, measures undertaken by the local administration, and the marketing strategies of venders. The secondary factor is the fact that one primary factor (passion of farmers) coincides on a time axis with the other two factors (administrative measures and marketing strategies of venders). Wide individual differences in the anticarcinogenic compound, 4-methylthio-3-butenyl isothiocyanate, were observed, likely due to the variable genome of Sabaka-daikon. In conclusion, the results imply that the information on the health benefits of daikon contribute to motivating the local residents (i.e., passion of farmers) in protecting Sabaka-daikon from extinction. Revival of this heirloom vegetable would contribute to the diversification of local foods, and to the preservation of those foods and the cooking knowledge for future generations. As Sabaka-daikon variety has some races in variation of 4-methylthio-3-butenyl isothiocyanate production, a race with a fixed amount of 4-methylthio-3-butenyl isothiocyanate should be established to guarantee added health benefits of Sabaka-daikon.

        PERCEPTION OF KOREAN STOPS BY JAPANESE SPEAKERS

        본고에서는 일본어 화자가 한국어의 파열음을 어떻게 지각하는지를 실험 결과를 통해 소개한다. 어두에서는 평음과 유기음이 무성파열음으로 지각되고 경음은 유성파열음으로 지각됐다. 또한 /Q/는 어두에서 거의 지각되지 않아 일본어 음운론과 일치한 결과를 보였다. 어중에서는 모음 사이에서 유성음이 되는 평음은 유성파열음으로 인식됐고 이 위치에서 주로 무성 촉음으로 인식된 경음과 유기음은 구별하기가 어려운것으로 밝혔다. 어미에서는 약 75%의 파열음이 인식됐고 받침 차열음이 탈락하는 차용어음성론과 일치 되지 않는 결과를 보였다. 전체적인 결과는 차용어음운론과 제2언어음운론 사이에서 일치되지 않는 형식을 보여주었다.

        Hepatitis B core-related antigen: A novel and promising surrogate biomarker to guide anti-hepatitis B virus therapy

        The current requirement for biomarkers to detect hepatitis B virus (HBV) infection is polarized. One is a fully-automated and highly sensitive measurement system; the other is a simple system for point-of-care testing (POCT) in resource-limited areas. Hepatitis B core-related antigen (HBcrAg) reflects intrahepatic covalently closed circular DNA and serum HBV DNA. Even in patients with undetectable serum HBV DNA or HBsAg loss, HBcrAg may remain detectable. Decreased HBcrAg levels are associated with reduction of the occurrence of hepatocellular carcinoma (HCC) in chronic hepatitis B. Recently, a fully-automated, novel high-sensitivity HBcrAg assay (iTACT-HBcrAg, cut-off value: 2.1 logIU/mL) has been developed. This attractive assay has been released in Japan very recently. iTACT-HBcrAg can be useful for monitoring HBV reactivation and prediction of HCC occurrence, as an alternative to HBV DNA. Moreover, monitoring HBcrAg may be suitable for determining the therapeutic effectiveness of approved drugs and novel drugs under development. Presently, international guidelines recommend anti-HBV prophylaxis for pregnant women with high viral loads to prevent mother-to-child transmission of HBV. However, >95% of HBV-infected individuals live in countries where HBV DNA quantification is not available. Worldwide elimination of HBV needs the scaling-up of examination and medication services in resource-limited areas. Based on this situation, a rapid and easy HBcrAg assay as a POCT is valuable. This review provides the latest information regarding the clinical use of a new surrogate marker, HBcrAg, in HBV management, based on iTACT-HBcrAg or POCT, and introduces novel agents targeting HBV RNA/protein.

        Promoting Education Regarding Conflict of Interest Management

        Even among highly educated health professionals, there is inconsistency in the knowledge and perception concerning both conflict of interest (COI) itself and COI disclosure. The key issue is the credibility of the manuscript, which relies heavily on transparency of COI for the reader. The tendency to disregard the importance of COI disclosure among journal editors has been recently highlighted. For all types of COI, the primary question is how it is managed. To ensure the enforcement of the declared journal COI policies, it is crucial that not only authors, but also those who are involved in the assessment of manuscripts, be educated and informed of the updated guidelines concerning COI disclosure.
