        Frequency and risk factors of severe postoperative bleeding after proctological surgery: a retrospective case-control study

        Purpose: The aim of this study was to assess frequency and risk factors of severe bleeding after proctological surgery requiring hemostatic surgery observed after publication of the French guidelines for anticoagulant and platelet-inhibitor treatment. Methods: All patients who underwent proctological surgery between January 2012 and March 2017 in a referral center were included. Delay, severity of bleeding, and need for blood transfusion were recorded. Patients with severe postoperative bleeding were matched to controls at a 2:1 ratio adjusted on the operator, and the type of surgery. Results: Among the 8,890 operated patients, 65 (0.7%) needed a postoperative hemostatic procedure in an operating room. The risk of a hemostatic surgery was significantly increased after hemorrhoidal surgery compared with other procedures (1.9% vs. 0.5%, P<10–4) and was most frequent after Milligan-Morgan hemorrhoidectomy (2.5%). Mean bleeding time was 6.2 days and no bleeding occurred after day 15. Blood transfusion rate was 0.1%. Treatment with anticoagulants and platelet inhibitors were managed according to recommendations and did not increase the severity of bleeding. The risk of severe bleeding was significantly lower in active smokers vs. non-smokers in univariate (16.9% vs. 36.2%, P=0.007) and multivariate (odds ratio, 0.31; 95% confidence interval, 0.14–0.65) analysis whereas sex, age, and body mass were not significantly associated with bleeding. Conclusion: Severe postoperative bleeding occurs in 0.7% of patients, but varies with type of procedure and is not affected by anticoagulant or antiplatelet treatment. These treatments given in accordance with the new guidelines do not increase the severity of postoperative bleeding.

      • Challenging issues in vitiligo research

        In the last five years we have studied prospectively a large cohort of children and adults with vitiligo. Clinical and pathological observations made in this group of patients have been of major importance to direct our bench research. In particular, the relation of segmental to nonsegmental vitiligo has been questioned, new forms of vitiligo have been delineated, and the importance of factors affecting immune responses (autoimmune and atopic diathesis) in patients and families have been studied. Recent data underline that vitiligo associated with thyroid autoantibodies has distinct clinical features from vitiligo without thyroid autoantibodies. In particular, patients with longer duration of disease and greater body surface involvement are more likely to present thyroid autoantibodies and should thus be monitored for thyroid function and anti-thyroid antibodies on a regular basis. Koebner`s phenomenon and its role in primary melanocytorrhagy vs relation to inflammatory progression of disease is important for disease assessment and prognosis. A simple clinical score for Koebner`s phenomenon was developed indicating that seven variables were independently associated with the presence of KP namely disease duration of more than 3 years, forehead + scalp areas, eyelids, wrists, genital + belt areas, knees and tibial crests. The score sustained adequate discrimination and calibration arguing that KP can be adequately predicted. Better nomenclature and outcome measures to perform muticenter randomized trials are future goals of our group within the Vitiligo European task force, now joined by other international colleagues since last year Vitiligo Global Issues Consensus Conferences held in Seoul and Bordeaux.

        The purpose of this paper is to prove some fixed point theorems using (𝜓, 𝜑)-contractions via the concept of C-class functions in G<sub>b</sub>-metric spaces. Moreover, an example is presented to illustrate the validity of our results.

      • Hierarchical structure parameters in three dimensional turbulence: She-Leveque model

        Hierarchical structure parameters, proposed in She-Leveque model, are investigated for velocity components obtained from different flow types over a large range of Reynolds numbers 255 < < 720. The values of intermittency parameter , with respect to a fixed velocity component, are observed nearly same for all four types of turbulence. The parameter , for streamwise velocity components is nearly the same but significantly different for vertical components in different flows. It is also observed that for both parameters, an obvious relation between the longitudinal and transverse components < (and < ) always holds. However, the difference between and is found very small in all types of turbulent flows, we studied here. It is evidenced that at low Reynolds numbers, the deviations from K41 scaling are mainly due to the most intense structures and slightly because of more heterogeneous hierarchy of fluctuation structures. However, at higher Reynolds numbers the deviations seem as a consequence of the most intense structures only. Over all, the study suggests that the hierarchy parameter may be consider as a universal constant.

      • Effect of recycled polypropylene fiber on high strength concrete and normal strength concrete properties

        An experimental study was undertaken to evaluate the performance of recycled polypropylene fiber (RPF) in concrete. The RPF materials were recycled from woven bags and used in concrete at various volume fractions corresponding to 0.1%, 0.2%, and 0.3%. Two different classes of strength, corresponding to normal and high strength concrete, were investigated. Fiber was used as substitution of coarse aggregate in concrete. The dosage of fiber was used at relatively lower dosages to avoid altering fluidity and to limit the reduction in coarse aggregate content. On the other hand, a commercial polypropylene fiber (PPF) was used at equivalent dosages than RPF for comparisons purposes. Test results indicated that optimized RPF volumes can secure comparable mechanical performance than those obtained with commercial PPF. On the other hand, the use of both fiber types resulted in lower compressive strength (10 to 20%), higher flexural strength (up to 27%), and lower elastic modulus (by 16%). Furthermore, the use of RPF type reduced the drying shrinkage (6 to 10%) of normal and high strength concrete types and increased the permeable pore void of both concrete types.

        Preparation and Non Isothermal Crystallization Kinetic of Acetylsalicylic Acid-Poly(vinylalcohol-co-ethylene) Blend. Application in Drug Delivery Domain

        A series of acetylsalicylic acid-poly(vinylalcohol-co-ethylene) blends (AcSa/PVAE) of different compositions were prepared by dissolution and solvent evaporation then characterized by the solubility test, differential scanning calorimetry (DSC), X-ray diffractometer (XRD) and by scanning electron microscopy (SEM)analyses. The results revealed that AcSa was perfectly soluble in PVAE up to a certain composition. A non-isothermal study of the crystallization kinetic of AcSa/PVAE materials was carried out by the DSC method. It was observed that the Ozawa equation perfectly describes the primary process of non-isothermal crystallization of this material. There is a strong dependence of AcSa/PVAE composition on the crystallization parameters. In general,at any cooling rate the variation in t1/2 of crystallization vs. the AcSa/PVAE composition passed by a minimum then a maximum of 7 and 5 wt% of AcSa, respectively. The crystallization activation energy (Ea) calculated from the Kissinger model reached a maximum when the concentration of AcSa in the AcSa/PVAE system was 8.5 wt%. The release dynamic of AcSa from AcSa/PVAE material was tested at body temperature during 92 h. On the light of the data obtained, it was found that at pH 5 and 7, AcSa/PVAE 15 presented the best performance because this material was capable of releasing more than 50 wt% of the AcSa amount uniformly (1.30±0.8 wt%/h) during 38-39 h.

        Impact of a 12-week high-intensity interval training without caloric restriction on body composition and lipid profile in sedentary healthy overweight/obese youth

        Although High-intensity interval training (HIIT) has shown its effective-ness in improving body composition, cardio-respiratory fitness and lipid profile in obese adults, evidences remain limited in overweight/obese youth. This study was conducted to investigate the effect of a 12-week HIIT program without caloric restriction on body composition and lipid profile among young overweight/obese men. Twenty healthy obese youth were randomly allocated into two groups; experimental group (HIIT) and control group. The HIIT program consisted in 3 exercises sessions per week (30 sec of work at 100% maximal aerobic velocity [MAV]) interspersed by 30 sec of active recovery at 50% MAV, starting by 15 repetitions to reach 27 by the end of the program. Aerobic capaci-ty (MAV and maximum oxygen uptake [VO2max]), body composition (body mass index [BMI], waist circumference [WC], and fat mass percent) and lipid profile (triglycerides [TG] and total, high-density lipoprotein [HDL] and low-density lipoprotein [LDL] cholesterol) were determined before and after the HIIT program. Following 12 weeks of HIIT, WC, BMI (P<0.01), and fat mass percent (P<0.05) were significantly decreased. MAV and VO2max were significantly improved in the HIIT group, only. To-tal cholesterol (P<0.05) and TG (P<0.05) decreased significantly in the HIIT group, while LDL and HDL cholesterol levels remained unchanged in both groups. HIIT may be particularly useful in overweight/obese youth to improve body composition, aerobic fitness and lipid profile.

        Genotypic and Phenotypic Characteristics of Tunisian Isoniazid-Resistant Mycobacterium tuberculosis Strains

        Forty three isoniazid (INH)-resistant Mycobacterium tuberculosis isolates were characterized on the basis of the most common INH associated mutations, katG315 and mabA -15C→T, and phenotypic properties (i.e. MIC of INH, resistance associated pattern, and catalase activity). Typing for resistance mutations was performed by Multiplex Allele-Specific PCR and sequencing reaction. Mutations at either codon were detected in 67.5% of isolates: katG315 in 37.2, mabA -15C→T in 27.9 and both of them in 2.4%, respectively. katG sequencing showed a G insertion at codon 325 detected in 2 strains and leading to amino acid change T326D which has not been previously reported. Distribution of each mutation, among the investigated strains,showed that katG S315T was associated with multiple-drug profile, high-level INH resistance and loss or decreased catalase activity; whereas the mabA -15C→T was more prevalent in mono-INH resistant isolates,but it was not only associated with a low-level INH resistance. It seems that determination of catalase activity aids in the detection of isolates for which MICs are high and could, in conjunction with molecular methods,provide rapid detection of most clinical INH-resistant strains.

        Evaluation of heavy metal kinetics through pyridine based Th(IV) phosphate composite cation exchanger using particle diffusion controlled ion exchange phenomenon

        In this study, the ion exchange kinetics of selected heavy metals such as Cd, Cu, Pb and Zn through thepyridine based thorium(IV) phosphate composite cation exchange material was studied. The ionexchange kinetics over this composite cation exchanger followed the particle diffusion controlled ionexchange phenomenon. Kinetics parameters also revealed that the mechanism of ion exchange for heavytoxic metal ion is feasible to explore the wastewater treatment.
