        Acute cerebral infarction combined with a thyroid storm in a patient with both Moyamoya syndrome and Graves’ disease

        Moyamoya syndrome (MMS) associated with hyperthyroidism, such as Graves’ disease, is a rare condition that causes ischemic stroke with thyrotoxicity. A 43-year-old woman with symptoms of right hemiparesis was admitted. Brain magnetic resonance imaging revealed a small cerebral infarction in the left frontal lobe. Cerebral angiography revealed multi-vessel intracranial occlusive disease. Several days later, neurologic deterioration and aggravation of cerebral infarction developed due to a thyroid storm. A thyroid function test revealed the following: thyroid-stimulating hormone (TSH) <0.01 μunits/mL (reference range, 0.55–4.78 μunits/mL); triiodo-thyronine >8.0 ng/mL (reference range, 0.77–1.81 ng/mL); free thyroxine (T4) of 9.47 pmol/L (reference range, 11.4–22.6 pmol/L); and TSH receptor antibody of 37.10 U/L (reference range, 0–10 U/L). For thyroid storm management, we initiated treatment with methimazole, Gemstein’s solution, and hydrocortisone. Finally, the thyroid disease was controlled, and neurologic deficits improved. We describe a case of acute cerebral infarction combined with a thyroid storm in a patient with Moyamoya syndrome and Graves’ disease. Hyperthyroidism such as Graves’ disease should be considered in the differential diagnosis for patho-etiologic mechanisms associated with MMS. A cerebrovascular disease with a thyroid storm can lead to severe mortality and morbidity. Prompt diagnosis and strict treatment are important.

        뇌관류에 의한 선택적 뇌온도 하강이 허혈후 뇌수분 함량에 미치는 영향

        Background : Blood-brain barrier(BBB) permeability and intravascular hydrostatic pressure are main factors for developing brain edema. Selective cooling of the brain could attenuate the ischemia-induced increase of BBB permeability. Because the method can provide driving pressure for edema formation, a beneficial effect of hypothermic perfusion on reducing edema would be questionable. The goal of this study was to evaluate the effect of isolated cerebral perfusion during the cerebral ischemia on the formation of brain edema. Methods : Both vertebral arteries were cauterized, right carotid artery was cannulated to provide an infusion route. After left carotid artery was snared, cold saline was immediately perfused into the right carotid artery for 10 min in the perfusion group. To produce ischemia in no perfusion group carotid arteries were clamped bilaterally for 10 min. Brain water contents were measured using the kerosene/bromobenzene density gradient after reperfusion and compared with no perfusion and normal control group. Results : Brain water content of perfusion group measured at 90 min after reperfusion showed increased water content compared to no perfusion and normal control group(p$lt;0.05). However, at 180 min after reperfusion, there were no statistically significant differences between the perfusion and no perfusion group. Conclusions : Cerebral saline perfusion during the ischemia enhanced the formation of brain edema even though hypothermia could reduce BBB permeability. These results show driving force is more important than permeability for the development of brain edema in this type of experiments. (Korean J Anesthesiol 1997; 32: 19∼26)

      • 사고전류의 경사각 및 클램핑 장치를 이용한 방사형 MVDC 배전계통의 보호협조 운용방안

        본 논문에서는 사고전류의 경사각 특성과 전류 클램핑 장치를 이용한 방사형 MVDC 배전계통의 보호협조 운용방안을 제시한다. 여기서, 경사각 특성을 이용한 운용방안은 간선의 사고지점에 따라 비례적으로 변하는 경사각의 특성을 이용하여 차단기의 보호협조를 설정하여 빠른 시간 내에 사고구간을 선택적으로 분리하는 것이다. 또한 사고전류 클램핑 장치를 이용한 운용 방안은 분기선에 사고가 발생하는 경우 사고전류를 제한하여 분기선용 보호호기기가 사고구간을 분리할 수 있는 보호협조 시간을 확보하여 메인컨버터의 탈락을 방지하는 것이다. 한편 PSCAD/EDTDC를 이용하여 방사형 MVDC 배전계통을 모델링을 수행하여 보호협조 특성을 분석한 결과, 제시한 보호협조 운용방안에 따라 메인 컨버터의 탈락을 방지하고 사고구간을 분리할 수 있어서, 본 논문에서 제안한 보호협조 운용방안이 유용함을 알 수 있었다.

        물리치료사와 작업치료사의 필라테스에 대한 인식도 조사

        Background: This study is intended as an investigation of the recognition for physical and occupational therapist on the Pilates. Methods: Thirty three therapist were participated in instruction of C-college, completed questionnaire form and then analyzed data. Results: Subjects who don't instruct or practice on the Pilates was 90.9% and physical therapist(PT) was 83.3%, occupational therapist(OT) 100%. 57.6% subjects was well aware of the Pilates by the media and was a most higher respondents. Therapists knowing the exact effect of the Pilates was 54.5%. PT was 33.3%, OT 80.0% and between PT and OT groups were significant difference(p<.05). Therapists knowing the exact practice with Pilates method of applicable a patient was 54.5%. In relation to Pilates and rehabilitation, a lot was 18.2%, a yes 69.7% and a little 12.1%. Therapists have applied the Pilates in rehabilitation was 12.1%. In minded therapists applying the Pilates after instruction, a lot was 24.2%, a yes 69.7%, a little 6.1% and a none 0%. Conclusion: Judging from the above, the knowledge and recognition of the Pilates for PT and OT was low and the will applying for therapy was high. Therefore, therapist is in need of instruction of the Pilates and further research on the Pilates and therapist would clarify the development of PT or OT.

        The Effect of Brain Hypothermia on Brain Edema Formation after Transient Ischemia

        BACKGOUND: When ischemia reduces blood supply, hypothermia remains the sine qua non for reducing demand. An alternative to whole body deep hypothermia is an isolated cerebral hypothermia via perfusion of cooled blood through one internal carotid artery. The goal of this study was to evaluate the effect of isolated cold hemisphere perfusion during the cerebral ischemia on the formation of brain edema. METHODS: The studies were designed to perfuse a saline solution into both carotid arteries with a different temperature (left 15degreesC, right 38degreesC) in the same animal. Cerebral ischemia was produced by a combination of the both carotid artery saline perfusion and systemic hypotension to a mean arterial blood pressure of 40 mmHg for 10 minutes. Ninety minutes after reperfusion, brain water contents were measured using the kerosene/bromobenzene density gradient and compared with warm saline perfusion and normal control group.         일부 대학생의 감각처리능력과 대인관계문제와의 관계

        이 연구는 성인 대학생의 감각처리 능력과 대인관계 문제와의 관련성을 조사하였으며 연구 대상자 177명이 작성한 청소년/성인 감각 프로파일 점수와 대인관계문제 검사 점수의 관련성을 분석한 결과 ‘낮은 감각 등록’영역은 KIIP 모든 척도와 관련성이 있는 것으로 나타났다 ‘감각찾기’ 영역은 통제지배(r=0.150, p<0.05), 자기희생(r=0.175, p<0.05), 과관여(r=0.218, p<0.01), 대인적과민성(r=0.196, p<0.01), 사회적인정욕구(r=0.149, p<0.05)가 상관관계를 보였 다. ‘감각민감성’영역은 KIIP 모든 척도와 관련성이 있는 것으로 나타났다. ‘감각회피’ 영역은 KIIP 척도 중 통제지배 (r=0.403, p<0.01), 자기중심성(r=0.420, p<0.01), 대인적비수용성(r=0.452, p<0.01), 공격성(r=0.417, p<0.01)과 비교적 높은 상관관계를 보였다. 본 연구를 통해 감각처리 능력과 대인관계 문제와는 전반적으로 관련이 있는 것으로 나타 났으며 특히 낮은 감각등록과 감각민감성 영역이 대인관계 문제와 높은 상관관계가 있는 것으로 나타났다. This research was conducted to find out the relation between sensory processing ability and Interpersonal Problem of university students. The subjects were 177 university students whose curricula were similar. The researchers made the subjects fill out the Adolescent/Adult sensory profile and Korean inventory of interpersonal problem(KIIP). The analysis of the relevancy between the Adolescent/Adult profile and the Interpersonal problem showed that 'Low Registration' is related to all aspects of KIIP. 'Sensation Seeking' showed relation with Domineering/Controlling(r=0.150, p<0.05), self sacrificing(r=0.175, p<0.05), Intrusive (r=0.218, p<0.01), Interpersonal sensitivity(r=0.196, p<0.01), and Need for social approval(r=0.149, p<0.05). 'Sensation Sensitivity' showed relation with all aspects of KIIP. ‘Sensation avoiding' showed relatively high relation with Domineering/ Controlling(r=0.403, p<0.01), Vindictive(r=0.420, p<0.01), Interpersonal ambivalence (r=0.452, p<0.01), and aggression(r=0.417, p<0.01). Through this research, it turned out that 'Sensory Processing ability' and the Interpersonal Problem are generally related, especially 'Low Registration' and 'Sensation Sensitivity‘ are highly related to the Interpersonal Problem.

        Modification of the characteristics of silkworm powder by treatment with alkaline protease

        Enzymatic modification of proteins is often used to increase the biological activity of materials. Silkworm powder has been investigated as a functional food resource, but no study has been performed on its modification by commercial food enzyme. Therefore, this study aimed to determine the feasibility of such modification of silkworm powder by alkaline protease. The activity of the enzyme was confirmed using an azocasein assay. Subsequently the silkworm powder was hydrolyzed by enzymatic treatment. UV visible spectrometry showed that the supernatant of silkworm powder subjected to enzymatic treatment had a stronger absorption band than the untreated powder. SDS-PAGE electrophoresis showed that the molecular weight of silkworm powder decreased on enzymatic treatment. Thus the results indicate that commercial enzymes might be used to modify the characteristics of silkworm powder.

        DSP 보드를 이용한 가상의 리더와 태그 에뮬레이터 시스템 구현

        신호를 생성할 수 있는 상용 신호 생성 장치 그리고 데이터 수집 장비와 DSP 보드를 이용하여 가상의 리더 와 태그를 모델링 한 에뮬레이터 시스템을 구현했다. 개발한 가상의 UHF RFID(860~960[MHz]) 리더/태그 모듈을 이용하여 개발한 RFID 리더와 태그의 프로토콜 및 RF 특성 등이 국제 표준 규격(ISO 18000-6 Type C, EPCglobal C1G2)에 적합 여부를 검증할 수 있는 방법을 제공해 줄 수 있게 하였다. 본 논문에서 제안한 가상의 리더 모델과 태그 모델을 구현하기 위해 DSP 보드를 이용하여 Visual DSP로 구현하였으 며, 시스템의 신호발생기, 신호 분석기, 성능 검증 대상 리더 또는 태그, RFID 에뮬레이터 제어 컴퓨터 및 제어 프로그램으로 구성하였다. Modeling a virtual reader and tags, the emulator system is realized by using a commercial signal generation device to make signal, a data collection equipment, and DSP board. By using a Virtual UHF RFID (860 ~ 960 [MHz]) reader/tags module, a developed RFID reader, protocol of tag, and properties of RF support to provide the way how to verify the suitability to international standards (ISO 18000-6 Type C, EPCglobal C1G2). In this paper, to implement a proposed model reader and tag model, Visual DSP is applied by using DSP board, composing the system's signal generators, signal analyzers and performance verification, the target readers or tags, RFID emulator control computesr and control programs
