Introduction: Ruptured sinus of valsalva aneurysm(RSVA) is a rare disease that fi rst described in 1840 by John Thurman. Symptoms vary depending on the rapidity of the rupture, the size of the ruptured origice and the chamber into which rupture occurs. Case report: A 54-year-old woman was admitted to our emergency department presenting with sudden-onset palpitation and chest discomfort while singing a song. ECG in the ambulance was compatible with wide QRS tachycardia but initial ECG in the emergency room was changed atrial fi brillation. A physical examination demonstrated grade 6 continuous murmur was auscultated on the left lower sternal border. Transthoracic echocardiogram was performed and revealed mild TR with increased RV systolic pressure of 67mmHg and a shunting fl ow over 4m/s from right coronary cusp to right ventricle. Also the 8mm-sized defect was detected on dilated aneurysm of right coronary sinus. There was no significant stenosis in coronary arteries and the ratio of the volume of pulmonary fl ow (Qp) and systemic fl ow (Qs) was 2.63 and oxygen saturation of RV(89.8%) was slightly higher than one of RA by 8%. She was still suffering from palpitation, and the 12-lead ECG showed RVOT VT same as before in ambulance with hemodynamic instability. A total of 8 times electrical cardioversion were immediately performed. Next, she underwent an emergency operation and was discharged without any complications including recurrent VT. Discussion: It would be the fi rst case that ruptured sinus of Valsalva aneurysm presenting with RVOT VT. We thought the mechanism triggering RVOT VT in this case is abrupt volume stretch caused by signifi cant amount of left to right shunt followed by rupture of the aneurysm.

<P>In this paper, we propose a near maximum likelihood (ML) scheme for the decoding of multiple-input-multiple-output (MIMO) systems. By employing the metric-first search method, Schnorr-Euchner enumeration, and branch-length thresholds in a single frame systematically, the proposed technique provides efficiency that is higher than those of other conventional near-ML decoding schemes. From simulation results, it is confirmed that the proposed scheme has computational complexity lower than those of other near-ML decoders while maintaining the bit error rate (BER) very close to the ML performance. The proposed scheme, in addition, possesses the capability of allowing flexible tradeoffs between the computational complexity and BER performance.</P>

<P><I>Cudrania tricuspidata</I> fruit extract contains a rich source of prenylated flavonoids with potential antiatherosclerotic, hepatoprotective, and anti-inflammatory properties. However, the effect of <I>C. tricuspidata</I> fruit extracts and its active compounds on the high-affinity IgE receptor (FcεRI)-mediated signaling remains unknown. In the present study, the effect of methanol extract from the fruits of <I>C. tricuspidata</I> (MFC) and its active compound, 5,7,3′,4′-tetrahydroxy-6,8-diprenylisoflavone (THDPI), on FcεRI-mediated signaling in mast cells was investigated. MFC and THDPI suppressed mast cell degranulation and Ca<SUP>2+</SUP> influx. MFC also interfered with IgE–FcεRI interaction and decreased FcεRIβ mRNA expression in mast cells. Furthermore, MFC and THDPI inhibited the phosphorylation of Syk, LAT, and PLCγ and F-actin redistribution. An Automatic Loop Gain Control Algorithm for Bang-Bang CDRs

<P>An automatic loop gain control algorithm (ALGC) for a bang-bang (BB) clock and data recovery (CDR) is proposed. The proposed algorithm finds the optimum loop gain using the autocorrelation of a BBPD output signal for minimum MSE performance. Mathematical proof of the algorithm is presented for both rotator-based and VCO-based CDRs with finite loop delay. A 25 Gb/s transceiver IC is fabricated using a 40 nm CMOS process to validate the performance of the algorithm. The power consumptions of TX and RX are 37.8 mW and 46.8 mW, respectively and the synthesized area implementing a digital loop filter together with the proposed ALGC occupies 140 μm × 170 μm.</P>
Growth Pattern and Prognostic Factors of Untreated Nonfunctioning Pituitary Adenomas

Objective : Pituitary adenomas (PAs) are often detected as incidental findings. However, the natural history remains unclear. The objective of this study was to evaluate the natural history and growth pattern of untreated PAs. Methods : Between 2003 and 2014, 59 PAs were managed with clinico-radiological follow up for longer than 12 months without any kind of therapeutic intervention. Tumor volumes were calculated at initial and last follow-up visit, and tumor growth during the observation period was determined. Data were analyzed according to clinical and imaging characteristics. Results : The mean initial and last tumor volume and diameter were $1.83{\pm}2.97mL$ and $13.77{\pm}6.45mm$, $2.85{\pm}4.47mL$ and $15.75{\pm}8.08mm$, respectively. The mean annual tumor growth rate was $0.33{\pm}0.68mL/year$ during a mean observation period of $46.8{\pm}32.1months$. Sixteen (27%) PAs showed tumor growth. The initial tumor size (HR, 1.140; 95% confidence interval, 1.003-1.295; p=0.045) was the independent predictive factor that determined the tumor growth. Six patients (11%) of 56 conservatively managed non-symptomatic PAs underwent resection for aggravating visual symptoms with mean interval of 34.5 months from diagnosis. By Cox regression analysis, PAs of last longest diameter over 21.75 mm were a significant prognostic factor for eventual treatment. Conclusion : The initial tumor size of PAs was independently associated with the tumor growth. Six patients (11%) of conservatively managed PAs were likely to be treated eventually. PAs of last follow-up longest diameter over 21.75 mm were a significant prognostic factor for treatment. Further studies with a large series are required to determine treatment strategy.
