      • 초음파 가이드를 통한 맥락막상강출혈의 치료

        Purpose : Intraoperative suprachoroidal hemorrhage is one of the most serious complications. We report a case of suprachoroidal hemorrhage during phacoemulsification surgery, was managed properly. Methods : Suprachoroidal hemorrhage occurred during phacoemulsificaiton in other hospital. The patient was referred to our department immediately after closing the surgical wound. On initial examination, iris and vitreous were prolapsed through surgical wound. Ciliary body was closely place to anterior chamber, and retina was not visible. Results : Prolapased ocular tissue were reduced by emergency operation. We controlled the patient’s intraocular pressure and inflammation. Ultrasonography was repeatedly performed to check the degree of liquefaction of suprachoroidal hemorrhage. After 9 days of suprachoroidal hemorrhage onset, vitrectomy and drainage through sclera was performed. Postoperative state was stable with successful removal of suprachoroidal hemorrhage. After 2 weeks of secondary operation, retinal detachment has occurred on nasal retinal area, so we performed scleral buckling and endolaser photocoagulation. After 6 months, retina was attached well and intraocular pressure was in normal range. Conclusion : In suprachoroidal hemorrhage, drainage of suprachoroidal hemorrhage after liquefaction of thrombi is effective. After successful drainage, postoperative complications may accompany such as retinal detachment or elevation of intraocular pressure. 목적 : 술중 발생하는 맥락막상 출혈은 매우 심각한 합병증이다. 저자는 수정체 유화술 중에 발생한 맥락막상 출혈을 적절히 대처한 증례를 보고하는 바이다. 대상과 방법 : 수정체 유화술 중 발생한 맥락막상강 출혈환자가 수술창을 봉합 후 즉시 본원으로 전원되었다. 초진시 홍채와 유리체가 수술창을 통해 탈출되어 있었으며, 전방을 통해 모양체가 확인되었으나, 망막은 보이지 않았다. 탈출된 안구내 조직을 응급수술을 통해 복원 후 환자의 안압과 염증을 조절하였다. 초음파를 반복적으로 실시하여 맥락막상 출혈의 액화 정도를 확인하였으며, 출혈 9일 후 유리체 절제술과 공막을 통한 배액술을 시행하였다. 결과 : 맥락막상 출혈이 성공적으로 제거되었으며 술 후 상태는 안정적이었다. 수술 2주 후 비측 망막 박리가 발생하여 공막돌륭술과 안구내 삽관 레이저술을 시행하였다. 6개월 후 망막은 잘 붙어있었으며, 안압은 정상 범위를 유지하였다. 결론 : 맥락막상 출혈의 경우 출혈의 배액 시기를 결정하는 것이 어렵지만, 규칙적인 초음파를 통해 배액술의 시기를 결정할 수 있다. 또한 맥락막상강 출혈의 성공적인 배액 이후에도 망막박리 및 안압상승과 같은 합병증의 동반에 대해 유념해야한다.

        진동에 의한 혈관 내 폐 보조장치의 산소전달 특성

        본 연구는 급성호흡부전환자를 치료하기 위하여 사용되고 있는 혈관 내 폐 보조장치의 기체전달을 향상시키기 위하여 중공사막에 진동기법을 사용하여 기체전달을 향상시키고자 시도하였다. 그리고 혈관 내 폐 보조장치를 정맥에 삽입하기 전, 혈관 내 폐 보조장치의 설계조건을 실험적 모델을 통하여 기체전달을 예측할 수 있는 예측식을 만들고자 하였다. 실험결과, 본 연구에서는 진동기법이 기체전달을 향상시키는데 효과적임을 확인할 수 있었다. 뿐만 아니라, 충진율과 가징 주파수의 함수관계를 이용하여 기체전달을 예측 할 수 있었다. 실험에 의하여 얻어진 결과는 예측식에 의하여 얻어진 결과와 유사한 결과를 얻었다. 그러므로 본 연구에서는 충진율과 가진 주파수의 함수를 이용하여 VIVLAD의 기체전달을 예측할 수 있었다. In this paper, we tried to improve gas exchange of the vibrating intravascular lung assist device(VIVLAD) using vibrating method in the hollow fiber membrane, for the patients suffering from acute respiratory distress syndrome(ARDS). And we tried to formularize prediction equations to make a prediction about gas transfer for designing intravenous artificial lung assist device, and designed modules under various conditions were studied through an experimental modeling before inserted the artificial lung assist device into as venous. As a result, we are convinced that vibration method is very useful for the has transfer increasing. Also, we can estimate the gas transfer as a function of the packing density and excited frequencies. The gas transfer obtained from the experiment was similar to that from the prediction equation, confirming the usefulness prediction equation. Therefore, we can estimate the gas transfer of the VIVLAD as a function of the packing density and excited frequencies.

      • Sinc 기저함수를 이용한 CGFFT의 수렴률 개선

        The current induced on a perfectly conducting strip excited by incident fields which are transverse magnetic (TM) wave or transverse electric (TE) wave to the strip axis are solved by three methods. The one is analytic and the others are numerical method: SIT (Spectral Iteration Technique), CGFFT (Conjugate Gradient Fast Fourier Transform). It is estimated accuracy of current values, as that are compared results of the analytic method with that of the numerical methods. The procedure involves the incorporation of subdmain basis functions associated with the current representation of a conducting strip. It is shown that significant improvements are achieved in the convergences of the CGFFT method when using sinc basis functions.

        납동위원소비법에 의한 영광 수동유적 청동기의 산지추정

        본 연구는 전남 영광군 화평리 수동마을 움무덤에서 출토된 새무늬 청동기 및 본뜬거울(방제경)을 주조하기 위해서 어디에서 원료를 가져다 썼는지 그 산지를 추정한 결과이다. 청동기의 원료산지를 추정하는 방법중의 하나로서 납동위원소비법을 사용하였다. 즉, 청동기에 함유된 납의 동위원소비를 분석한 다음 기 확보된 한국·중국·일본 방연광의 납동위원소비 분포도를 비교하여 어느 군(group)에 소속되는지를 살펴보았다. 또한 다변수분석법중 선형판별식분석으로 작성한 방연광 분포도를 사용하여 위의 결과와도 잘 일치하는지를 확인하였다. 납동위원소비 분석결과 새무늬 청동기는 중국 북부산 방연광 분포 범위에 포함되었는데 이는 선형판별식분석 결과와도 일치하였다. 본뜬거울은 중국 남부산의 방연광에 속하는 것으로 생각되나 한국 남부산 방연광 일부와도 인접해 있어서 명확하게 판단하기에는 어려움이 있다. 또한 위의 결과를 종합하면 동일 움무덤에서 출토된 청동기이지만 원료의 출처는 서로 다르다는 것을 반영하고 있다. 청동기 원료의 산지추정을 위해서는 성분조성비 분석도 함께 이루어져야 하나 대부분 부식되어 분석이 불가능하였다. 그러나 이들 납동위원소비 데이터는 차후 호남 문화권의 청동기연구에 있어서 좋은 참고 자료가 되리라 판단된다. Five lead isotope ratios(206/204, 207/204, 208/204, 207/206, 208/206) of two bronze artifacts excavated from Sudong site, Youngkwang-gun were analyzed by TIMS(thermal ionization mass spectrometer). Their lead isotope ratio data obtained were compared with those of galena of Korea, Japan and China, which data were being used for reference raw material data set. And multivariate mapping method such as statistical linear discriminant analysis(SLDA) was applied to classify the sources of raw materials. The lead isotope data of bronze artifact decorated with bird design was included in the area of galena of northern China and this result was accorded with SLDA result. Imitated bronze mirror was of southern China but very difficult to differentiate with area of south Korea. Consequently, the result of lead isotope ratios of two bronze artifacts suggested that the sources of raw materials were different origin each other.

      • OenNtn 합성수지를 이용한 Sm, Co, U의 흡착에 관한 연구

        The ion exchange resins have been synthesized from chloromethyl styrene-1,4-divinylbenzene(DVB) with 1%, 5%, and 20%-crosslinking and macrocyclic ligand of cryptand type by copolymerization method and the adsorption characteristics of uranium(Ⅵ), cobalt(Ⅱ) and samarium(Ⅲ) metallic ions have been investigated in various experimental conditions. The correlation between the adsorption characteristics of rare earths and transition metal on the resins and stability constants of complexes with macrocyclic ligand have been examined. The UO_2^(2+) aqueous solutions are not adsorbed on the resins below pH 3.0 but the power of adsorption of UO_2^(2+) increased rapidly above pH 4.0. The adsorption power was in the order of 1%, 5% and 20%-crosslinked resin, but adsorptive characteristics of resins decreased in proportion to the order of dielectric constants of solvents.

      • 평판 및 쉘 구조물의 유한 요소해석을 위한 프로그램 개발

        The original Mindlin-type degenerated plate and shell elements perform reasonably well for moderately thick shell structure. However, for thin shells when full integration is used to evaluate the stiffness matrix, overstiff solutions are often produced due to shear and membrane locking. To overcome the locking phenmena of the degenerated shell element, the assumed strain method is introduced. Using this approach, the finite element analysis program has been developed for the plate and shell under elastic conditions. The program are accommodated in a simple and effective manner by using a layered approach, where in two Gauss point integration scheme is adopted for each layer. Numerical tests for linear elastic problems show an improved performance of this program.

        측두하악 관절의 재건을 위한 늑연골 이식의 효과

        We performed costochondral grafting in mmandibular reconstruction including condyle replacement in 5 patients which have 4 cases of ameloblastoma, 1 case of resorption of left condyle due to osteomyelitis. We harvested the 6 th costochondral cartilage of unaffected side. No specific complications were appeared with unexpected fracture of costochondral junction and infection of operation site, These patients show 30-45mm at month opening, near normal of affected side, near normal or normal of unaffected side at lateral excursion. Postoperative coureses was uneventful with improved cosmetic and functional results. Our report is indicated that the costochondral graft is satisfactory to reconstruct mandibular defect including condyle replacement, especially in growing patients.

      • 부유대역 용융응고법으로 제조된 YBa_2Cu_3O_x 산화물초전도체의 미세구조와 전기적 특성

        The YBa_2Cu_3O_x-20 wt% Y_2BaCuO_5 composition designed in YBCO system was synthesized by solid state reaction method. The optimum condition of hot zone temperature and growth rate in floating-zone melt were 1080 ℃ and 2.5 mm/hour, respectively. In samples growth as these condition, the low angel boundaries in grain direction and small amount of amorphous phase known as decreasing characteristics of superconducting materials between grain boundaries in microstructure are investigated by polarized light microscopy on the whole. The critical current density(Jc) at 77k of sample using powder fabricated with no CeO_2 and doped 5 wt%CeO_2 were 8,000 A/㎠(Jc : 561A) and 12,000 A/㎠(Jc: 826A) at 0 T by direct current transport method, respectively.

Primary school students(n=7) participated in the aerobic exercise and agility Ladder exercise program(HRmax 60~80%, 55min/day, 5days/week).
